(This syndrome arises from extreme heat combined with external contraction of wind-cold, or triggered by fright from objects. When convulsive wind manifests with spasms, one must not forcibly restrain the child's limbs; instead, allow the pathogenic force to circulate through the meridians naturally. Once the heat reaches its peak, it will subside on its own, preventing injury to the body.)
Qingxin Lingshen Tang (Clear Heart and Calm Spirit Decoction) (Treats acute convulsive wind and various heat-induced fright syndromes. Can be prepared as a paste or pill and decocted into a soup. Children often experience fright; this formula is suitable to dispel pathogens in the Heart and Liver channels.)
Ophiopogonis Radix (Ouyangdong, remove heart, 7 fen) Poria cocos with Hostwood (Baifushen, 5 fen) Ginseng Radix et Rhizoma (Renshen, 2.5 fen) Bupleuri Radix (Chaihu, honey-fried) Saposhnikoviae Radix (Fangfeng, 5 fen each) Cinnabar (Zhusha, water-triturated, 2.5 fen; cold natured, do not use long-term; substitute with Scutellariae Radix (Huanglian) and wine-processed Zhusha, 3 fen total) Moschus (Shexiang, 1 zi)
If frightened by loud noises, indicating Yin deficiency, purgation is indicated. Add Strychni Semen (Heiqiu) head powder (contraindicated in deficiency), Akebiae Caulis (Mutong, remove nodes), 5 fen each; if fearing Gallbladder Qi deficiency, add Gentianae Macrophyllae Radix (Caolongdan, wine-washed) and Alpiniae Officinarum Fructus (Yizhiren, shell removed, salt-water fried), 5 fen each.
If frightened by strange sights, indicating weak Heart Qi and empty Shen, add Angelicae Sinensis Radix (Dangguishen, wine-washed) and Ziziphi Spinosae Semen (Suanzaoren, skin and tip removed, stir-fried separately), 5 fen each, Carthami Flos (Honghua, wine-washed), 1.5 fen.
If frightened by wind contact, indicating concurrent exterior contraction, double the amount of Chaihu, add Puerariae Lobatae Radix (Gegen) and Peucedani Radix (Qianhu), 5 fen each, Notopterygii Rhizoma et Radix (Qianghuo) and Glycyrrhizae Radix et Rhizoma (Gancao, fried), 3 fen each.
If frightened by cold air contact (Lung resides above the five zang organs and suffers first, leading to qi contraction and heat binding), add Astragali Membranacei Radix (Sheng Huangqi), 5 fen, Cimicifugae Rhizoma (Shengma) and Perillae Folium (Ziyè), 3 fen each, Zingiberis Rhizoma (Ganjiang, fried), 1.5 or 2 fen.
If frightened by stepping or pressure, causing blood and qi stagnation due to fright, add Suanzaoren (skin and tip removed, crushed and fried), 5 fen, Linderae Radix (Wuyao, skin and core removed, wine-washed), 3 fen, and Cinnamomi Cortex (Guipi, 2.5 fen; this material can unblock blood vessels).
If frightened by human intimidation, indicating Qi deficiency, double Renshen, add Suanzaoren (stir-fried separately, 7 fen) and Caolongdan (wine-washed, 5 fen).
If frightened by fire, indicating Shen insufficiency, double Zhusha, add Dangguishen (wine-washed) and Suanzaoren (stir-fried separately), 5 fen each, Polygalae Radix (Yuanzhi, bone removed, dry-fried), 3 fen, and Honghua (wine-washed), 1.5 fen.
If frightened by water, indicating Jing (Essence) deficiency, double Renshen (tonifies Lung Qi, as Metal generates Water), add Corni Fructus (Zhuyu, wine-steamed, pitted), 5 fen, Cynomorii Herba (Roucongrong, wine-soaked, scale removed) and Yizhiren (shell removed, salt-fried), 3 fen each.
If frightened with head raised and trance-like state, add Gastrodiae Rhizoma (Tianma, coarse skin removed) and Suanzaoren (stir-fried separately), 5 fen each, Bombyx Batryticatus (Baijiangcan, mouth removed, ginger-fried), 3 fen.
If startled during sleep with nightmares, constant fear and restlessness, add Suanzaoren (stir-fried separately) and Rehmanniae Radix Praeparata (Shudi, ginger-fried), 5 fen (to prevent stomach damage), and Yizhiren (salt-fried), 3 fen.
If phlegm is abundant with sawing-like wheezing, add Arisaematis Cum Bile (Danningxing) and Fritillariae Thunbergii Bulbus (Beimu, heart removed, dried), 5 fen each, Wuyao (skin and core removed, wine-washed), 3 fen. When qi flows smoothly, phlegm transforms and spasms stop spontaneously. First administer Suhé Wán (Suhe Pill) mixed with Bohe Tang (Mint Decoction), 1-2 pills, followed by this formula for good effect.
If convulsions with phlegm-wheezing cause suffocation nearing death, add Danningxing and Tianma, 5 fen each, Baijiangcan (dry-fried), 2 fen, Scorpio (Quanxie, remove head, wings, feet, and earth from abdomen, dry-fried), 2.5 fen.
If eyes roll upward with tidal saliva, add Danningxing and Citri Reticulatae Pericarpium Rubrum (Juhong), 5 fen each, Aucklandiae Radix (Muxiang), 3 fen, and Quanxie (dry-fried), 1 piece.
Generally, the physiological principle of children originates from the wonder of "Heaven generates One, Water." For treating fetal heat, erysipelas, convulsive wind, phlegm-heat, metamorphosis fever, fever, vomiting, and dysentery, promoting free flow of the water passages is the most expedient method, thus clearing Heart heat and promoting urination to dissipate fire. Double Renshen, add Junci Medulla (Dengxin), 3 fen each, Polyporus (Zuling) and Alismatis Rhizoma (Zexie), 2.5 fen each. Tianyi Wan also treats these conditions (formula see below, others follow this pattern).
For various heat-induced fright, Qingxin Wan (Clear Heart Pill) is primary (Chaihu, Fangfeng, Baifushen, Renshen, Zhusha in equal parts, made into pills, coated with Gold Leaf).
For acute convulsive wind caused by evil heat, Xiao'er Anshen Wan (Children's Calming Spirit Pill) is primary (Da Anshen Wan formula see below).
For acute convulsive wind due to Liver heat, Xieqing Wan (Drain Green Pill) is primary.
For dizziness, confusion, and trance, Baoming Dan (Life-Preserving Pill) is primary.
For Qi stagnation with phlegm-induced spasms, Danning Bao Long Wan (Phlegm-Nine Dragon Guarding Pill) is primary (formula see below).
When convulsive wind is critical and difficult to save, moxa three burns on each nipple, Baihui (GV20) and Shangxing (GV23) (acupoints located one cun above the anterior hairline). Another method: moxa three burns between the eyebrows, highly effective. Convulsive wind is specifically the contention between Heart Fire and Liver Wind, chaotic blood and merged qi, obstructed phlegm-saliva, blocked orifices unable to vent, causing violent sudden spasms. After the attack subsides, if the child returns to normal consciousness, it is acute convulsive wind; if continuous and lingering, it is chronic convulsive wind. Refer to the treatment of chronic convulsive wind below.
Kaiguan San (Open Gate Powder) (Treats acute/chronic convulsive wind with locked jaw)
Centipede (Wugong, one piece; those with golden heads and red legs can be wine-soaked and roasted) Baijiangcan (mouth removed, dry-fried) Arisaema cum Bile (Nanxing, processed, dried, 1 qian each) Shexiang (1 zi) Gleditsiae Sinensis Fructus (Zhuya Zaojiao, three pods, slightly burned to retain properties)
Grind all into powder. Dip finger in Ginger Juice, apply small amount of medicine to rub teeth, or use a tool to drip 2-3 drops into the mouth. Saliva discharge opens the gate. I observe that although this formula contains toxic materials, it is not for frequent consumption. However, for children with this symptom, application is harmless. (Children preserve pure Yang Qi and have not broken Yin, so even small doses of toxic agents do not cause major damage.)
For pediatric trismus (locked jaw), moxa Ran Gu (KI2) (acupoint one cun directly below the medial malleolus, below the large bone) three burns, highly effective.
Niuhuang Bao Long Wan (Cattle Gallstone Dragon-Guarding Pill) (Based on Danning Bao Long Wan plus Niuhuang, Zhenzhu, Hupo, Jinbo. Name implies Niuhuang Bao. Excellent formula for hundred diseases in children. Use first to calm when acute/chronic types are undifferentiated. 'Bao' means protect; 'Long' refers to Liver. Liver is Mother, Heart is Child; if Mother is safe, Child is safe. Heart houses Shen, Liver houses Hun; if Shen and Hun are guarded internally, how can fright arise?)
Arisaematis Cum Bile (Danningxing, 1 liang) Tianshu Huang (Tianzhuhu, 5 qian) Realgar (Xionghuang) Cinnabar (Chensha, water-triturated, 2.5 qian each) Shexiang Zhenzhu Hupo (1 qian each) Niuhuang (5 fen) Gold Leaf (Jinbo, 10 pieces)
Mix with Licorice decoction paste to form pills size of Euryale Seed (Qianshi), coat with Gold Leaf. Half-year-old infant takes 1/3 of a pill; three-year-old takes one pill. Take with Mint Decoction. This is an excellent formula for calming fright, reducing heat, and resolving phlegm. However, its nature is extremely cold, lacking Yuan-tonifying power, so do not overuse. If heat reduction is urgently needed, adding 2-3 fen of Renshen taken with tea is acceptable. After fright stops and normalcy returns, switch to Spleen-tonifying regulators (such as Buyuan Tang). Generally, convulsive wind is a disease of Heart Fire, Liver Wind, and Spleen Phlegm. If Metal is prosperous within Earth, Wood naturally declines and Fire retreats, preventing contention. Thus, regulating Spleen and Stomach after severe illness is the King's Way of medicine, a profound truth. Niuhuang Bao Long Wan is rare and expensive, unaffordable for commoners. Using Renshen, Beimu, Baijiangcan, Fangfeng, Suanzaoren, Zhusha, Niuhuang in equal parts as powder, taken with Mint Decoction, yields swift efficacy comparable to the pill. Observing laypeople, they only know using precious materials for critical conditions, unaware that extremely cold medicines aggressively attacking high heat may trap pathogens internally rather than expelling them externally. After taking good medicine, the patient appears normal, deemed cured, and neglected. Later, when relapse occurs due to internal deficiency and external contraction, the root is solidified and harder to treat. Acute convulsive wind becomes epilepsy; chronic becomes mania. Families raising young children should beware.
Jiefeng Wan (Wind-Cutting Pill) (Treats acute/chronic convulsive wind, phlegm spasms, and acute transforming into chronic; use to cut off the condition)
Gastrodiae Rhizoma (Tianma) Baijiangcan (mouth removed, dry-fried) Wugong (one piece) Bai Fuzi (processed according to Materia Medica to remove toxicity) Fangfeng Zhusha Quanxie (dry-fried, 1 qian each) Shexiang (1 zi)
Make honey pills size of Sophora Seed (Wutongzi), take with Mint Decoction. This is a key medicine for stopping fright, resolving phlegm, and cutting wind, but contains many toxic materials. Although symptoms reduce, it damages nature and depletes Yuan Qi. Remove Wugong, reduce Bai Fuzi by 5 fen, replace Nanxing with Beimu, add Renshen 2 qian and Xionghuang 5 fen to moderate it.
Da Anshen Wan (Great Calming Spirit Pill) (Treats acute/chronic convulsive wind. Calms spirit and stops fright, also treats Heart heat night crying)
Ziziphi Spinosae Semen (Suanzaoren, skin removed, powdered, fried) Gancao (fried, 5 fen each) Baijiangcan Jiegeng Wei (tips, 1.25 qian each) Renshen Chi Fuling Baizhu Zhusha Ouyangdong (remove heart) Muxiang Daizheshi (vinegar-boiled, 2.5 qian each; authentic product hard to find, substitute with 1 qian Xionghuang) Quanxie (ginger-fried, 3 pieces) Gold and Silver Foil (3 pieces each)
Make honey pills size of Sophora Seed, coat with Gold/Silver Foil, dissolve 1-2 pills in Mint Decoction. This formula has mild nature, combining tonification and draining. It can be taken regularly as pills or decoction after recovery from convulsive wind.
