A young puerpera, half a month postpartum, suffered from metrorrhagia (profuse vaginal bleeding) and syncope. People generally attribute this to "evil blood rushing to the heart," but little do they know it is due to imprudent sexual intercourse! Although more than half a month has passed since delivery, unlike the critical first two or three days, the Qi and Blood have just begun to regenerate and are not yet fully restored. Even if the lochia has cleared, the damage to the Chong and Ren vessels (uterus/reproductive system) has not healed; one must absolutely avoid sexual activity to prevent further injury to the "gate" (the root of vitality).
When Qi and Blood have just recovered but proper care is neglected, leading to metrorrhagia and syncope, both the Heart and Kidney are injured, not merely the reproductive organs. With the leakage of Essence and the dissipation of Shen (Spirit), there is no better method than massive tonification of Qi and Blood.
The formula used is Jiubai Qiusheng Tang (Rescue for the Defeated, Seek Life Decoction).
Ren Shen (Ginseng Radix et Rhizoma, 2 liang), Dang Gui (Angelicae Sinensis Radix, 2 liang, washed with wine), Bai Zhu (Atractylodis Macrocephalae Rhizoma, 2 liang, stir-fried with soil), Shu Di Huang (Rehmanniae Radix Praeparata, steamed nine times, 1 liang), Shan Yu Rou (Corni Fructus, 5 qian, steamed), Shan Yao (Dioscoreae Rhizoma, 5 qian, stir-fried), Suan Zao Ren (Ziziphi Spinosae Semen, 5 qian, raw), Fu Zi (Aconiti Lateralis Radix Praeparata, 1 fen or 1 qian, self-processed).
Decoct in water and take orally. After one dose, the Shen stabilizes; after two doses, the syncope stops; after three doses, the bleeding ceases. If improvement is seen after one dose, continue for three to four doses, then reduce the dosage by half, and take ten more doses to celebrate recovery. This formula tonifies Qi to restore Yuan Yang (Original Yang) at its source; as Yang returns, Qi follows, naturally grasping the Blood and returning it to the Shen, reviving life.
Commentary: There are cases where the Middle Qi (Spleen/Stomach Qi) is inherently deficient, leading to immediate, unstoppable metrorrhagia postpartum, with Qi collapsing simultaneously. This is a critical condition where eight or nine out of ten patients die. Only Du Shen Tang (Single Ginseng Decoction) may save one or two. Use Liao Ren Shen (Liaoning Ginseng, removing the radicle) 5 qian, crushed, decoct urgently; delay will result in fatal Qi collapse. Administer the decoction slowly. Once Qi recovers, prepare another dose. For other treatments, refer to the Metrorrhagia section. However, postpartum, Hang Shao Yao (Paeoniae Radix Alba Carbonisata) and other cooling herbs must be avoided. Note that these cases are caused by sexual intercourse one or two days before delivery; beware!
Verse:
Half a month postpartum, the young mother experiences sexual arousal and intercourse.
Metrorrhagia and syncope occur, accompanied by hallucinations; such illness is self-inflicted.
Jiubai Qiusheng Tang uses Ren Shen and Bai Zhu, along with Dang Gui, Shan Yao, and Shu Di Huang.
With Shan Yu Rou, Suan Zao Ren, and processed Fu Zi, this miraculous formula tonifies Qi and restores Yang.
