Zhengchan (Normal Delivery) refers to the condition where the parturient experiences intermittent abdominal pain or cessation thereof, along with soreness and pain in the lower back and hypochondrium. If the labor force is urgent but the amniotic sac has not yet ruptured, it is called "Nongtai" (False Labor). Taking Bazhen Tang (Eight-Treasure Decoction) with added Cyperi Rhizoma (Xiangfu) will bring relief spontaneously. If the amniotic sac has ruptured for several days but contractions remain slow, the same medicine should be taken while awaiting delivery.
Shangchan (Traumatic Delivery) occurs when the fetus is not yet full-term and is disturbed by injury or agitation, resulting in abdominal or umbilical pain. It may also result from taking parturition-inducing medicines too early, or from the mother exerting herself excessively, forcing the fetus into an abnormal position, preventing normal delivery. Therefore, near term, movements should be leisurely; one should avoid excessive sleep, overeating, or drinking alcohol. Whenever fetal movement is felt within the abdomen, the mother should lie supine with her body straightened, waiting for the fetus to turn properly. It is better to be cautious beforehand than to struggle with difficulty at the moment of delivery.
Diaochan (Preparation for Delivery) involves selecting a midwife, preparing utensils, and ginseng-based medicines before term. During labor, there should be no noise from many people; two attendants should support the body, or the mother may stand leaning against an object. If the mother feels irritable, mixing one spoonful of white honey with boiling water is effective, and Duhuo Tang (Angelicae Pubescentis Decoction) is even better. If hungry, consume a small amount of millet porridge; do not let the mother suffer from hunger or thirst. If the fetal breathing pattern is irregular, claim it is a twin pregnancy or retained placenta to prevent the mother from becoming frightened.
Cuisheng (Induction of Labor) addresses difficult labor caused by entering the delivery room too early, leading to fatigue. Use Bazhen Tang (Eight-Treasure Decoction), slightly supplemented with Cyperi Rhizoma (Xiangfu) and Boswelliae Resina (Ruxiang), to assist Qi and Blood. If the amniotic sac ruptures early and the amniotic fluid and blood have dried up, use Bazhen Tang (Eight-Treasure Decoction) as well.
Dongchan (Cold-induced Delivery Difficulty) occurs when cold weather causes stagnation of Qi and Blood, delaying rapid delivery. Therefore, clothing should be thick, and the delivery room should be warm, especially keeping the back and lower body covered.
Rechan (Heat-induced Delivery Difficulty) requires appropriate temperature control during summer months. If the delivery room is crowded and heat becomes oppressive, causing symptoms such as headache, red face, and fainting, drink a small amount of clear water to resolve it. However, avoid exposure to cool drafts from wind and rain.
Hengchan (Breech/Transverse Presentation) refers to the fetus being head-down and foot-up in the womb. During delivery, if the head is down but the mother exerts force prematurely, the fetus may turn sideways halfway through. The mother should be instructed to lie peacefully on her back to allow the fetus to self-correct. The midwife should use the middle finger to hold the shoulder, preventing entanglement by the umbilical cord. Using parturition-inducing medicine and exerting effort will lead to immediate birth.
