On the day of birth, or the following day, if an infant passes stool, it is colloquially called "passing umbilical meconium" (Xia Qishi). This indicates that the intestines and stomach are harmonious and unobstructed, and the pylorus (Youmen) is moist. If the infant does not pass stool after two or three days, it is named "locked belly" (Suodu), caused by heat-toxin from pungent and hot substances acquired in the womb, leading to Qi stagnation and obstruction. Such infants will necessarily exhibit a red face, abdominal distension, refusal to nurse, and frequent crying. Ancient methods first employed Cinnabar Honey (Zhusha Mi) for treatment; if ineffective, Yi Nian Jin (One Pinch Gold) would be酌情 administered (see formula under "Refusal to Nurse"). Subsequently, a woman should rinse her mouth with warm water and suckle seven areas: the anterior and posterior chest/abdomen, both palms, both soles, and below the navel. The procedure continues until the skin turns reddish, which can relieve constipation.
[Chen Shouzhen's Note] Cinnabar (Zhusha) has a heavy-sinking property that damages the Spirit (Shen) and should not be used lightly; Yi Nian Jin is even less advisable. Only the method of suckling the seven areas (anterior/posterior chest, palms, soles, and below navel) can be faithfully followed. Alternatively, one may use Powder of Raw Glycyrrhiza (Sheng Gancao) mixed with a small amount of Powder of Coptis (Huanglian), steeped in boiling water for a moment, and administered orally without harm.
When an infant fails to pass both urine and stool upon birth, it is a most critical condition, resulting from heat-toxin in the fetus. If this persists for seven days, termed "one lunar fortnight" (Yi La), accompanied by a hard, distended abdomen and constant moaning, recovery becomes difficult. Ancient emergency protocols included oral suction at the five centers (two palms, two soles, and heart region) and below the navel, followed by administration of Tongxie Fang (Wood Pass Decoction) to clear heat (ingredients include Akebiae Caulis [Mutong], Plantaginis Semen [Cheqianzi], Polygoni Citini Herba [Bianxu], Stellariae Herba [Qumai], Poria Cocos [Chiling], Gardeniae Fructus [Shanzhi], Talcum [Huashi], Scutellariae Radix [Huangqin], Rheii Radix et Rhizoma [Dahuang], Raw Glycyrrhizae Radix et Rhizoma [Sheng Gancao], using Juncus effusus [Dengxin] as a guide). Alternatively, Zi Shuang Wan (Purple Frost Pill) could be used to open obstructions (ingredients include Haematitum [Daizheshi], Halloysit Rubra [Chishizhi], Armeniacae Semen Amarum [Xingren], Crotonis Fructus [Badou]) as an emergency measure. Another method involved grinding several fresh Earthworms (Di Long) with a little honey and applying the paste to the penis, while internally administering ash from burned Cicada Slough (Chan Tui) paper mixed with small amounts of Cinnabar (Zhusha), Borneolum Syntheticum (Longnao), and Moschus (Shexiang), prepared with Ophiopogonis Radix (Maidong) and Juncus effusus (Dengxin) decoction, poured into the mouth to restore passage.
[Chen Shouzhen's Note] Infants possess fragile and tender constitutions. Tongxie Fang clears heat too aggressively, and Zi Shuang Wan opens obstructions even more excessively; neither should be tried lightly. Even the external application of fresh Earthworms with honey is not entirely appropriate. Borneolum Syntheticum (Longnao) and Moschus (Shexiang) are acrid and dispersing drugs, particularly unsuitable. One should simply have a woman suckle the seven areas (anterior/posterior chest, including both palms and soles, and below the navel). If abdominal distension and Qi oppression persist, urgently mix milk with a small amount of Juice of Allii Fistulosi (Congbai) and apply it to the infant's mouth for suckling. Then, gently insert a firm stalk of green onion into the anus; passage will be restored shortly.
