Pediatric Heart Gan (Gan Syndrome of the Heart) presents with symptoms such as a red face and red eyes, high fever with sweating, frequent palpitations and irritability, teeth grinding and tongue protrusion, dry mouth and tongue, sores caused by thirst-driven drinking, red and dark urine, chest and diaphragm fullness and oppression, a preference for sleeping in a prone position, loss of appetite leading to emaciation, or various conditions such as vomiting or diarrhea. When heat is excessive, ancient methods primarily prescribe Xie Xin Dao Chi Tang (Drain Heart and Guide Out Red Decoction), containing Tong Cao (Akebia Caulis), Sheng Di Huang (Rehmanniae Radix Recens), Huang Lian (Coptidis Rhizoma), and Sheng Gan Cao (Glycyrrhizae Radix et Rhizoma Recens), with Deng Xin Cao (Junci Medulla) used as an guiding herb. If heat excess generates fright (convulsions), ancient methods primarily prescribe Zhen Zhu San (Pearl Powder), composed of Zhen Zhu (Margarita), Mai Dong (Ophiopogonis Radix), Tian Zhu Huang (Calami Tuber), Jin Bo (Folium Aurum), Niu Huang (Bovis Calculus), Hu Huang Lian (Scutellariae Barbatae Radix), Sheng Gan Cao (Glycyrrhizae Radix et Rhizoma Recens), Ling Yang Jiao (Capris Cornu), Da Huang (Rhei Radix et Rhizoma), Dang Gui (Angelicae Sinensis Radix), Zhu Sha (Cinnabaris), Xiong Huang (Realgar), Fu Shen (Poria cum Radice), and Xi Jiao (Rhinocerotis Cornu), ground into a fine powder. This should be mixed evenly with Yin Chen Hao Tang (Artemisiae Scopariae Decoction) and taken cold. For prolonged illness resulting in Heart deficiency, ancient methods primarily prescribe Fu Shen Tang (Poria Decoction), containing Fu Shen (Poria cum Radice), Dang Gui (Angelicae Sinensis Radix), Ren Shen (Ginseng Radix), and Zhi Gan Cao (Glycyrrhizae Radix et Rhizoma Preparata), with Long Yan Rou (Longan Aril) used as a guiding herb, decocted and taken orally. For symptoms of irritability and thirst, add Mai Dong (Ophiopogonis Radix).
[Author's Note] The Heart belongs to Fire, its color is red, and it governs the blood vessels; thus, phenomena such as high fever and frightful irritability occur. The Heart is connected to the Pericardium (Xin Bao Luo). When the Heart is hot, one must urgently prevent the pathogen from invading the Pericardium, which could lead to complications. The sedative/heavy nature of Pearl Powder and the acrid/dispersing nature of Dang Gui are both unsuitable in this context. Therefore, for excessive heat, only use fifteen fresh leaves of Ye Zhu (Bambusa Textilis Folium), one qian of Xi Mu Tong (Akebiae Caulis), two qian of Tian Zhu Huang (Calami Tuber), eight fen of Sheng Bai Shao (Paeoniae Radix Alba Recens), and three fen of Sheng Gan Cao Shao (the tip of Glycyrrhizae Radix et Rhizoma Recens). After decoction, mix with two fen of juice ground from Ling Yang Jian (the tip of Capris Cornu) and take orally.
If the illness has been prolonged causing Heart deficiency, the formula uses San Zi Shen (Pseudostellariae Radix) three qian, Zhu Sha Ban Fu Shen (Poria coated with Cinnabaris) half a qian, Shu Di Huang (Rehmanniae Radix Preparata) half a qian, Sheng Bai Shao (Paeoniae Radix Alba Recens) two qian, Yuan Zhi Rou (Polygalae Radix Flesh) one qian, Sheng Gan Cao (Glycyrrhizae Radix et Rhizoma Recens) eight fen, Suan Zao Ren (Ziziphi Spinosae Semen) five fen, with Shi Chang Pu (Acori Tatarinowii Rhizoma) three fen and Long Chi (Dent Draconis Os) half a qian used as guiding herbs.
Pediatric Gan Thirst often arises from the accumulation of heat due to excessive consumption of rich and sweet foods, which scorch and consume the Spleen and Stomach, leading to a depletion of body fluids. Consequently, there is frequent intense thirst requiring large amounts of water intake, accompanied by mental irritability and heat. Ancient methods primarily prescribe Qing Re Gan Lu Yin (Clear Heat and Generate Fluids Decoction), containing Sheng Di Huang (Rehmanniae Radix Recens), Mai Dong Qu Xin (Ophiopogonis Radix decored of heart), Shi Hu (Dendrobii Caulis), Sheng Zhi Mu (Anemarrhenae Rhizoma Recens), Mi Zhi Pi Pa Ye (Loquat Leaf prepared with honey), Shi Gao (Gypsum Fibrosum), Sheng Gan Cao (Glycyrrhizae Radix et Rhizoma Recens), Yin Chen Hao (Artemisiae Scopariae Herba), and Huang Qin (Scutellariae Radix), with Deng Xin Cao (Junci Medulla) used as a guiding herb.
[Author's Note] Children who indulge in rich and sweet foods damage their fluids and consume their essence, causing Stomach Qi to sink downward, thereby preventing the generation of body fluids. Although the treatment method must focus on generating fluids, if the Stomach is not tonified, the Clear Yang cannot ascend, and how can fluids be generated? Therefore, the method should involve tonifying the Stomach while simultaneously clearing accumulated heat. The formula uses Dan Zhu Ye (Lophatheri Herba) half a qian, Xi Sheng Di (Rehmanniae Radix Recens) two qian, Jin Cha Shi Hu (Dendrobii Nobile Caulis) half a qian, Yuan Mai Dong (Ophiopogonis Radix) one qian, Zhi Tian Dong (Asparagi Radix preparata) one qian, Xi Yang Shen (Panacis Quinquefolii Radix) one qian, Sheng Bai Zhu (Atractylodis Macrocephalae Rhizoma Recens) one qian, Fu Ling Pian (Poria Slice) two qian, Fen Ge Gen (Puerariae Lobatae Radix) five fen, and Sheng Gan Cao (Glycyrrhizae Radix et Rhizoma Recens) five fen, with Yin Jin Hua Lu (Silver Flower Dew) one liang used as a guiding herb.
