Pediatric Pi Disease (abdominal mass due to food stagnation and qi/blood stasis) initially presents with tidal fever in the afternoon, thirst with a preference for cold drinks, abdominal distension with visible subcutaneous veins, gradually progressing to hardness and mass formation, accompanied by intermittent pain. Ancient methods advocate combined internal and external treatment: Internally, administer Qianjin Xiao Pi Wan (Thousand-Gold Pill for Dissolving Masses). Formula ingredients include Lu Hui (Aloe), A Wei (Ferula resin) prepared separately into a paste; Qing Dai (Indigo Naturalis), Mu Xiang (Vladimiria Root), Hou Po (Magnolia Bark), Bing Lang (Areca Seed), Chen Pi (Citrus Peel), Sheng Gan Cao (Raw Licorice Root) each 1 qian; Shi Jun Zi Rou (Seed of Quisqualis), Hu Huang Lian (Coptis Rhizome), Shan Zha Rou (Hawthorn Flesh), Cu Chao Xiang Fu (Cyperi Rhizome stir-fried with vinegar), San Leng (Sparganium Rhizome), E Zhu (Curcuma Rhizome) each 2 qian; Shui Hong Hua Zi (Polygoni Multiflori Caulis Seed), Chao Shen Qu (Medicated Leaven stir-fried), Chao Mai Ya (Malt stir-fried) each 4 qian; Ren Shen (Ginseng Root), Tu Chao Bai Zhu (Atractylodes Macrocephala Rhizome stir-fried with earth), Fu Ling (Poria) each 3 qian. All are ground into fine powder. Take 1 qian of A Wei, mix with water and wheat flour to make a paste as binder, form pills the size of mung beans. Administer with rice water, dosage adjusted according to the child's age.
Externally, apply Hong Hua Gao (Red Flower Ointment). Formula ingredients include Mo Yao (Myrrh) 5 qian, Xue Jie (Dragon's Blood), She Xiang (Musk), A Wei (Ferula resin) each 3 qian, Dang Gui (Angelica Sinensis Root), Chi Shao (Red Peony Root) each 1 qian, Shui Hong Hua material one bundle. Decoct the material, remove dregs, and boil down to one bowl of ointment. Grind the above six herbs into fine powder, mix into the ointment evenly, spread on blue cloth, and apply to the affected area.
If there is no fever or thirst, ancient methods advise first using Mu Xiang Wan (Aucklandia Pill). Formula ingredients include Mu Xiang (Vladimiria Root), Peng E Zhu (Curcuma Phaeocaulis Rhizome), Suo Sha Ren (Amomum Fruit), Qing Pi (Immature Bitter Orange Peel), Zhu Sha (Cinnabar) each 2 qian. Grind into fine powder, mix uniformly, bind with refined white flour paste, form pills the size of hemp seeds. Take 2-3 pills per dose. If caused by breast milk injury, administer with breast milk; if caused by food injury, administer with decoction of the ingested food.
[Commentary] Pediatric Pi disease is mostly caused by irregular diet leading to fullness in the intestines and stomach, overflow of turbid fluids, and subsequent condensation due to cold pathogen invasion. Alternatively, it may result from blood membranes transforming into water retention, which over time forms masses beside the hypochondrium, causing intermittent pain, tidal fever, or alternating chills and fever resembling malaria. Generally, children with Pi disease are predominantly deficient in constitution; purgative drugs should not be used frequently. For those with weak constitutions, use Xi Dang Shen (Codonopsis Root), Chao Bai Zhu (Atractylodes Macrocephala Rhizome stir-fried), Zheng Guang Pi (Tangerine Peel from Zhejiang), Bai Yun Ling (Poria), Ru Zheng Hang Qing Pi (Cyperi Rhizome steamed and stir-fried with vinegar), Chuan Hou Po (Magnolia Bark processed with ginger juice), Xiao Zhi Shi (Immature Bitter Orange Fruit stir-fried with wine), Fa Ban Xia (Pinellia Tuber processed), Xi Sha Ren (Amomum Fruit stir-fried with wine), Liu Shen Qu (Medicated Leaven stir-fried), Chen Mai Ya (Malt stir-fried) each 2 qian, Zhi Jiu Lei Bie Jia (Testudinis Carapax et Plastrum honey-roasted) 3 qian, Jing San Leng (Sparganium Rhizome roasted) 1 qian. Grind into fine powder, bind with morning rice flour paste to form pills. Take 1-2 qian per dose. Swallow with rice water according to the child's age, until the mass resolves.
For those with robust constitutions, use Dai Zhe Shi (Limonitum), calcined in fire and quenched in vinegar until brittle, ground into powder and washed with water (water-floating method); Hang Qing Pi (Cyperi Rhizome stir-fried with vinegar), Peng E Zhu (Curcuma Phaeocaulis Rhizome roasted and stir-fried), Nan Mu Xiang (Vladimiria Root shaved into flakes), Qing Hua Gui (Cassia Twig) each 3 qian, Ba Dou Shuang (Croton Seed Frost, oil removed completely) 1 qian, Chuan Da Huang (Rhei Radix et Rhizoma) 2 qian. Grind into fine powder, bind with vinegar-boiled flour paste to form pills the size of radish seeds. Take 5 pills per dose, send down with light ginger soup. Externally, can combine application of Hong Hua Gao to alleviate symptoms. Follow this method for prescription.
