People in ancient times did not suffer from smallpox, while those in middle antiquity all suffered from it; this relates to desires and lifestyle, which in turn relate to prenatal constitution. People in the northern regions did not suffer from smallpox, while those in the central plains all suffered from it; this relates to geography, specifically the relationship of cold, cool, warm, and hot climates. Toxins have varying depths, so the eruption varies in severity. At the onset of pediatric smallpox, it resembles Taiyang disease (Exterior Syndrome due to Cold Damage). Symptoms include body fever, occasional palpitations and fright, coarse breathing through the mouth and nose, fixed gaze, coldness isolated to the middle finger, and warmth in the ears and sacrum. Further observation of red veins behind the ears confirms the signs of smallpox eruption. For instance: yawning and restlessness indicate the toxin emerging from the Liver Meridian; cough with phlegm, sneezing, and runny nose indicate emergence from the Lung Meridian; palpitations, irritability, and red face indicate emergence from the Heart Meridian; preference for sleep, spontaneous diarrhea, and heat in the four limbs indicate emergence from the Spleen Meridian; only coldness in the ears and sacrum indicates emergence from the Kidney Meridian, representing Fire failing to overcome Water.
Currently, the method of variolation (smallpox inoculation) is prevalent both domestically and internationally. All aspects regarding the site of eruption, the smooth or adverse nature of pustule shape and color, the maturity and density of the rash, the differentiation of deficiency and excess based on form, spirit, voice, breath, and diet, the differentiation of deficiency and excess in Qi, Blood, Yin, Yang, Interior, and Exterior, the smooth or adverse nature of constitutional endowment, and the smooth or adverse nature of heavenly timing and human affairs, are unrelated to variolation. Therefore, they are briefly omitted without detailed discussion.
