Pediatric Tian Diao (Sky Hook) is caused by pathogenic heat and phlegm-fluid obstructing the chest, preventing free circulation. Upon onset, it presents with palpitations, high fever, upward deviation of the eyes, convulsions of the hands and feet, and cyanosis of the nail beds. This condition resembles Jing Feng (Startle Wind), but differs slightly in that the eyes are predominantly fixed in an upward gaze. The prescription consists of twenty fresh tips of Bamboo Leaf (Zhu Ye Xin), three qian of Tiansu Zhu (Indian Hemp Seed/Amomum fruit substitute for phlegm), one qian of Lianqiao Xin (Forsythia Heart), one qian of Sheng Huang Qin (Raw Scutellaria Root), five fen of Chuan Huang Lian (Sichuan Coptis Rhizome), three qian of Gua Lou Shuang (Trichosanthes Powder), one and a half qian of Chuan Bei Mu (Sichuan Fritillaria Bulb), three fen of Sheng Gan Cao Shao (Raw Licorice Root Tip), two qian of Xian Sheng Di (Fresh Rehmannia Root), and ten drops of Zhu Li Zhi (Bamboo Sap), which are mixed with the decoction for administration.
Pediatric Nei Diao (Internal Hook) often results from pre-existing liver pathology combined with external cold invasion. Symptoms include greenish stools, tidal convulsions, flexed posture with abdominal pain, and spitting of frothy saliva. Although similar to Jing Xian (Startle Epilepsy), it is distinguished by the presence of red blood vessels and spots in the eyes. If the limbs become cold, nails turn cyanotic, and lips/mouth turn black, the prognosis is poor. For early-stage treatment, the prescription includes one and a half qian of Gou Teng Gou (Uncaria Ramulus cum Uncis), one qian of Guang Mu Xiang (Costus Root), five fen of Dan Gan Jiang (Mild Dried Ginger), five fen of Gan Cao (Licorice Root), one and a half qian of Chao Bai Shao (Stir-fried White Peony Root), two qian of Shuang Sang Ye (Mulberry Leaves After Frost), and three fen of Chao Xiao Hui Xiang (Stir-fried Fennel Fruit) as the guiding drug.
The pediatric epilepsy syndrome (Xian Zheng) resembles Jing Feng (Startle Wind) and Jing Feng (Spasm Wind) in that upon onset, the child falls unconscious, experiences convulsions, has abundant phlegm-fluid, rapid breathing with audible sounds, thus appearing similar to these two conditions. However, the limbs remain soft, and the child awakens after merely the time it takes to eat one meal, returning to a state indistinguishable from health. This contrasts with Jing Feng, where mental status remains unstable, and with Spasm Wind, characterized by whole-body rigidity and prolonged unconsciousness. Since manifestations vary, therapeutic methods differ; precise differentiation during clinical practice is essential before administering medication.
This syndrome belongs to Cold of the Viscera, often occurring after Slow Startle Wind, where phlegm enters the Pericardium. Onset features cold extremities, supine position with limb stiffness, bluish-white complexion, spitting of saliva, faint voice, and a deep, fine pulse. Mild cases are traditionally treated with Xing Pi Tang (Spleen-Awakening Decoction; see Slow Startle Wind section), while severe cases are treated with Gu Zhen Tang (Solidify True Decoction; see Slow Spleen Wind section).
[Commentary by Zhen] In the initial stage of epilepsy, administer equal parts of Bamboo Sap (Zhu Li Zhi) and Ginger Juice (Jiang Zhi) according to the child's age, administered slowly. Once the child regains consciousness, further medication can be considered. This method applies to all types of epilepsy. This specific presentation is Yin Epilepsy (Yin Xian). The prescription consists of one qian of Zhujie Ba Fu Zi (Bamboo-joint Typhonii Rhizoma), two qian of Tiansu Zhu (Indian Hemp Seed/Amomum fruit substitute for phlegm), three fen of Jiu Dan Xing (Aged Star Phellodendri Cortex/Fallopiae Semen processed with bile), one fen of Shi Chang Pu (Acori Tatarinowii Rhizoma), eight fen of Chuan Yu Jin (Curcuma Longa Rhizoma), two qian of Bao Fu Shen (Poria with Hostwood), one and a half qian of Guang Ju Hong (Citrus Reticulatae Pericarpium Rubrum), three fen of Chao Chuan Lian (Stir-fried Sichuan Coptis Rhizome), and five fen of Zhi Gan Cao (Honey-fried Licorice Root).
