I. Causes of Umbilical Tetanus (Qi Feng)
After birth, the umbilical cord must be cut with extreme caution. If improper cutting allows water-dampness and cold pathogens to invade through the navel, or if surgical instruments are unclean, introducing microorganisms into the blood; or if post-cutting bandaging is negligent, allowing pathogenic factors to enter; or if the umbilical cord falls off too early causing injury, all these conditions can lead to Umbilical Tetanus (Qi Feng). If not treated promptly, pathogenic toxins will circulate throughout the body, leading to rapid death. However, even with vigorous treatment at the onset, preservation of life is often difficult, hence the folk saying that "Umbilical Tetanus is hard to treat," which underscores the necessity of caution in midwifery and cord cutting.
II. Symptoms of Umbilical Tetanus (Qi Feng)
Before the onset of Umbilical Tetanus (Qi Feng), the infant may cry inexplicably and appear restless. Examination of the abdomen reveals slight distension and swelling of the navel, which are signs. At this stage, the corners of the eyes and the nasal tip (Bi Zhun) may show a yellowish hue, and sucking strength during breastfeeding weakens, indicating further progression of the disease. Variations in symptoms include: excessive phlegm-saliva accumulation with high-pitched, rapid breathing; high fever with red face and dry mouth; crying with bent waist and vomiting of green bile; severe cases present with Mouth Closure (Cuo Kou - lips tightly pursed like a purse string), Jaw Locking (Jin Kou - trismus), convulsions of hands and feet, a cyanotic vein on the lower abdomen branching from the navel towards the chest, and cyanosis around the umbilicus. These indicate critical, difficult-to-treat conditions.
III. Treatment Methods for Umbilical Tetanus (Qi Feng)
For early-stage Umbilical Tetanus (Qi Feng) with fever and frequent crying, treatment should focus on dispersing pathogenic heat using Qu Feng San (Wind-Dispersing Powder). For excess syndromes with constipation and high fever, purgatives such as Da Huang (Rhubarb Rhizome), Xuan Ming Fen (Mirabilite), and Hei Bai Chou (Black and White Morning Glory Seeds) may be used. In cases of excessive phlegm-saliva, umbilical swelling, limb convulsions, incessant crying, and inability to suckle, Tian Ma Wan (Gastrodia Pill), Ding Ming Dan (Life-Determining Elixir), or Zhu Yin Wan (Cinnabar-Silver Pill) should be considered based on the specific condition. For excess syndromes resembling Acute Convulsion (Ji Jing Feng), with closed eyes, locked jaw, silent crying, and foaming at the mouth, Chen Sha Gao (Cinnabar Paste) should be administered in appropriate amounts. This is an internal formula and must be used according to syndrome differentiation. External treatments are also available, with moxibustion being particularly suitable: One method for critical cases involves applying crushed garlic paste to the navel and performing moxibustion on top of the garlic until the patient smells garlic, followed by dripping garlic juice into the nose. Another method uses fire-lighting moxibustion; applied early, it prevents criticality, and when critical, it can save lives. This method was transmitted by the Xia family. Xia Yuzhu stated: When Umbilical Tetanus (Qi Feng) first appears, feeding becomes slightly looser than usual, or crying is intermittent. Quickly examine the eye corners and glabella (Mei Xin) in bright light; if yellow discoloration is present, immediate treatment leads to easy recovery. If yellow spreads to the nasal tip, treatment is still relatively easy. If it reaches the Ren Zhong (GV26) and Cheng Jiang (CV24) points, treatment becomes more difficult. If there is no mouth closure but slight puffing of cheeks, it is still treatable. However, if lips and mouth are tightly locked and the tongue is stiff, the prognosis is fatal. Upon seeing yellow discoloration in eye corners or nasal tip without lip locking, bend the infant's little finger and massage the Wai Lao Gong (Ex-UE8) point (located on the dorsum of the hand opposite the palm). Then, ignite a thick rush pith dipped in good sesame oil and perform moxibustion on the Fontanelle (Baihui/Danzhong area), Mei Xin (Yintang), Ren Zhong (GV26), Cheng Jiang (CV24), and Shao Shang (LU11) on both thumbs, one moxa cone each. Perform six cones on the umbilical rim. If the cord is intact, burn at the stump; if fallen, burn at the umbilical center. This stops Umbilical Tetanus (Qi Feng) and clears the yellow discoloration. Continue previous treatments until recovery. The efficacy of this fire therapy is unparalleled (Editor's Note: Umbilical Tetanus (Qi Feng) is often caused by traditional midwives lacking disinfection knowledge, leading to improper cord cutting. Modern obstetricians rarely cause this, but negligence cannot be entirely ruled out; thus, supervision of cord cutting is essential. Moxibustion works by using thermal energy to warm meridians, promote circulation, and sterilize, making it quite suitable. However, if pathogenic toxins spread extensively, cure becomes difficult).
Appendix: Mouth Closure and Jaw Locking Wind (Cuo Kou Jin Feng)
Mouth Closure and Jaw Locking Wind (Cuo Kou Jin Feng) belongs to the category of Umbilical Tetanus (Qi Feng), commonly seen after contracting Umbilical Tetanus (Qi Feng), or arising from fetal toxins combined with wind-cold exposure after birth. Mouth Closure (Cuo Kou) manifests as straight crying, tongue stiffness, and refusal to feed. Jaw Locking (Jin Feng) manifests as trismus and tightly clenched teeth. Both are critical conditions. For early Mouth Closure (Cuo Kou), use Chen Sha Jiang Can San (Cinnabar and Silkworm Powder); for severe limb convulsions, use Cuo Feng San (Wind-Catching Powder). For early Jaw Locking (Jin Feng), use Long Dan Tang (Gentian Decoction); if medicine cannot be swallowed, use Kai Guan San (Gate-Opening Powder) to open the jaw. Excess syndromes may benefit from mild purgation. Treatments should refer to formulas for Umbilical Tetanus (Qi Feng), and moxibustion methods are also generally applicable.
