I. Origin of Slow Convulsion
Zhuang Zaitian stated: The syndrome of slow convulsion (Man Jing, a type of pediatric convulsive disorder characterized by chronic onset and deficiency patterns) is mostly caused by vomiting and diarrhea in infants; or it may result from prolonged malaria or dysentery, or follow the eruption of smallpox or measles; or it may be due to cold-food stagnation, where excessive use of purgative drugs damages the Spleen and Stomach; or it may stem from innate constitutional deficiency with the mistaken use of cooling medicines; or it may result from overly aggressive purging in the treatment of fast convulsion (Ji Jing); or it may arise from neglect in care, leading to extreme deficiency that generates internal wind. All these conditions can lead to slow convulsion. Jiang Bihua stated: What is commonly referred to as slow convulsion in folk medicine is actually "wind generated by Spleen deficiency." If an infant suffers from vomiting or diarrhea for a long time, the Spleen becomes deficient, allowing Liver Wood to overact upon Earth (Spleen). This presents with a bluish-white complexion and slight twitching of the hands and feet. This is internal wind harassing Earth (Spleen), not external wind; Yang Qi is declining and spirit is fatigued, with shortness of breath, indicating depletion of Middle Qi, not fright. Thus, it can be known that the origin of slow convulsion lies in internal deficiency, which is completely different from the excess patterns of fast convulsion. Therefore, fast convulsion causes locked jaw and clenched teeth due to obstruction of orifices, whereas slow convulsion causes locked jaw and clenched teeth due to Yang decline and Yin usurpation; fast convulsion causes opisthotonos due to pathogenic factors following the spine and neck, whereas slow convulsion causes opisthotonos due to deficiency of the spine and brain; fast convulsion causes limb spasms due to heat invading the sinews and collaterals, whereas slow convulsion causes limb spasms due to emptiness of the sinews and collaterals; fast convulsion causes fluttering eyes due to Liver heat generating wind, whereas slow convulsion causes fluttering eyes due to Liver deficiency generating wind; fast convulsion causes throat rattling due to heat-phlegm obstruction, whereas slow convulsion causes throat rattling due to body fluids transforming into saliva/phlegm; fast convulsion causes cold limbs due to deep heat causing deep reversal (heat exhaustion), whereas slow convulsion causes cold limbs due to decline of Yang Qi; fast convulsion causes loose stools due to diarrhea accompanying heat, whereas slow convulsion causes loose stools due to Spleen damage and fire decline. The distinction between excess and deficiency is very clear. Furthermore, among pediatric convulsions, some present as slow convulsion from the onset, mostly due to failure of Yuan Qi (Original Qi) to recover after vomiting, diarrhea, malaria, dysentery, or exanthematous diseases; some start as fast convulsion and transform into slow convulsion, mostly due to congenital insufficiency where pathogens are uncontrolled, gradually forming internal injury. Cases caused by mis-treatment are also numerous.
II. Pathology of Slow Convulsion
Since the origin of pediatric slow convulsion is clear, its pathology can be deduced, essentially involving non-transportation of Spleen Yang, exhaustion of Heart and Liver, decline of spirit and cowardice of Qi, and wind generation due to internal deficiency. Therefore, the harm of slow convulsion does not lie in the invasion of pathogenic factors, but in the weakness of Zheng Qi (Upright Qi). If Zheng Qi can gradually grow, pathogenic factors will naturally be eliminated; if Zheng Qi cannot expand, pathogenic factors will never cease. Although the disease progression may resemble fast convulsion, the distinction between deficiency and excess is fundamentally different. Let us compare them: Fast convulsion's locked jaw is due to obstruction of orifices, so nasal stimulation rarely induces sneezing, and sneezing reduces the locking; slow convulsion's locked jaw is due to Yang decline and Yin usurpation, so the jaw occasionally loosens spontaneously, not due to obstruction, and nasal stimulation often induces sneezing; fast convulsion's opisthotonos is due to pathogens following the spine and neck, so the body arches stiffly and forcefully; slow convulsion's opisthotonos is due to deficiency of the spine and brain, so the traction is weak and powerless; fast convulsion's limb spasms are due to heat invading sinews and collaterals, so movements are rapid and retraction is severe; slow convulsion's limb spasms are due to emptiness of sinews and collaterals, so movements are slower with slight twitching; fast convulsion's fluttering eyes are due to Liver heat generating wind, so the movement is swift, either upward rolling or trembling; slow convulsion's fluttering eyes are due to Liver deficiency generating wind, so the movement is slower, either squinting or staring straight ahead; fast convulsion's throat rattling is due to heat-phlegm obstruction, so when heat surges upwards, there is a sensation of suffocation; slow convulsion's throat rattling is due to body fluids transforming into phlegm, so the sound is like sawing without expectoratable phlegm; fast convulsion's cold limbs are due to deep heat causing deep reversal, so there must be high fever and foul breath; slow convulsion's cold limbs are due to decline of Yang Qi, so there is often a bluish face, sweating on the forehead, and slightly warm body; fast convulsion's loose stools are due to diarrhea accompanying heat, so the stool is foul-smelling and yellow; slow convulsion's loose stools are due to Spleen damage and fire decline, so the stool contains undigested food and is green. From the pathological aspect, the vast difference in deficiency and excess is evident. As for cases progressing to cold body, sticky sweat, and complete clouding of consciousness, this indicates extreme internal injury and floating of True Yang, which belongs to incurable syndromes.
III. Symptoms of Slow Convulsion
The symptoms of slow convulsion initially often appear after vomiting and diarrhea, with persistent fever, restless sleep, gradually developing dizziness and head distension, and limb spasms. Subsequently, coma, panting, eye rolling, and convulsions occur. There may be alternating chills and fever, or bluish-white urine and feces. In severe cases, there is locked jaw, clenched teeth, opisthotonos, abdominal borborygmus, and intrathoracic rattling. Although the lips may crack and bleed, the breath expelled from the mouth is cold. The gaze is dim. In severe cases, the face is blue, the forehead sweats, and the limbs are ice-cold. The patient may shake their head, protrude their tongue, keep their eyes closed, or sleep with eyes partially open. If one observes a cold body, sticky sweat, lying straight like a corpse, panting and coughing, soft head, rigid back, locked jaw, head shaking, lip retraction, coarse breathing with rattling, and a face lacking luster, it mostly indicates an incurable condition. This is a general overview of the symptoms of slow convulsion.
IV. Treatment Methods for Slow Convulsion
In the early stage of slow convulsion, when symptoms are not fully manifested, presenting only with vomiting and diarrhea and occasional signs of fright, treatment should focus on harmonizing the Spleen and calming the Liver. Xingxing San (Wonderful Powder) is suitable. If vomiting and diarrhea persist, the Spleen is困 (trapped/dysfunctional) with loss of appetite, and body fluids transform into phlegm with loud rattling sounds, Tang Shi's Xingpi San (Spleen-Awakening Powder) is suitable. However, drugs such as Quanxie (Scorpion), Wugong (Centipede), Bingpian (Borneol), Shexiang (Musk), and mineral stones should be used with caution. If the situation is urgent and necessitates their use, they should be administered only once. Subsequently, Sijunzi Tang (Four Gentlemen Decoction) or Lizhong Tang (Regulate the Middle Decoction) should be used for regulation. If phlegm sounds do not subside and both deficiency and excess are present, Guntan Wan (Rolling Phlegm Pill) should be used first to treat the branch (symptoms), followed by modified Sijunzi Tang for regulation. If slow convulsion progresses, presenting with deficiency-cold or false-heat signs, Zhuhan Dangjing Tang (Expel Cold and Shake Off Fright Decoction) is suitable. For severe cases, Jiawei Lizhong Dihuang Tang (Modified Regulate the Middle and Rehmannia Decoction) should be advanced. If slow convulsion persists with deficiency-cold of Spleen Earth and decline of Yang Qi, Liujunzi Tang (Six Gentlemen Decoction) with Paojiang (Prepared Ginger) and Muxiang (Aucklandia) is suitable, or Fuzi (Aconite) can be added to restore Yang. For abundant phlegm, Qingzhou Baiwanzi (Qingzhou White Pills) is suitable. For depleted Yuan Qi leading to confusion, moxibustion at Baihui (GV20) acupoint should be performed urgently, or Haizang Fanhun Dan (Sea Storehouse Soul-Returning Pill) can be used. However, modern children have increasingly thin constitutions and fragile bodies. When encountering slow convulsion, many are incurable. Therefore, rather than panicking during illness, it is better to be cautious when vomiting and diarrhea or colds first begin, seek good physicians, and manage medication carefully for better outcomes.
