Whenever taking emetic agents, it is best to administer them from Chen hour (7:00–9:00 AM) to Si hour (9:00–11:00 AM). Taking purgative agents is best done after Ren Ding hour (9:00–11:00 PM) just before sleep; the same applies to diuretic agents. During the day at noon, human affairs are chaotic and Yuan Qi (Original Qi) dissipates; at night, when resting peacefully, Jing Qi (Essence Qi) descends. Therefore, medicines that promote unblocking and draining are most suitable for administration before sleep.
Children who often have picky eating habits, becoming increasingly emaciated with abdominal distension, suffer from toxins of rich foods steaming and steaming the intestines and stomach. Thus, they present with abdominal distension, muscle wasting, and abnormal food preferences. The treatment method should be to expel toxins from the intestines and stomach and circulate Jin Ye (Body Fluids). The ancient use of Xiaoguan Tang (Eliminate Malnutrition Decoction) was no more than this intent. However, this condition mostly belongs to untreatable cases.
Those who usually have hearty appetites but suddenly develop body stiffness or paralysis/inability to move limbs should not take medicine recklessly; simply reducing food intake will lead to spontaneous recovery. (Ning Gu said: Diseases often arise from food toxicity. Here, specifically mentioning emetics, yet stating that for this condition one should not take medicine recklessly, shows insight surpassing predecessors.)
For patients with epistaxis (nosebleed) where various medicines are ineffective, adding 2 qian of Jingjie (Schizonepetae Herba) to Sanhuang Xiexin Tang (Three Yellow Heart-Purging Decoction) yields miraculous effects. (Note: *Weisheng Jiabao* treats reckless flow of Qi and Blood, bleeding like a gushing spring from mouth and nose. Cebai San (Cypress Leaf Powder), composed of Cebaiye (Platycladi Cacumen), Renshen (Ginseng Radix), and Jingjiesui (Schizonepetae Spica), also demonstrates the efficacy of Jingjie. Although the patterns treated are opposite in deficiency/excess, both coexist and are valid.)
Fukushima Shindokken inherited the medical lineage of Matsubara Ikkan-sai. He lived in seclusion in Fangye for decades, his aspirations unconstrained by etiquette, his demeanor open and magnanimous, holding few strong opinions on praise or blame. Therefore, his medical methods were naturally flexible and adaptable, unlike the rigid adherence to ancient formulas prevalent at the time. His disciple Nakagawa recorded his clinical achievements and wrote *Fangweng Yitan* (Medical Talks of Old Man Fang), which can be considered loyal service to his master.
For any masses in the abdomen accompanied by spasms, urgency, and shortness of breath, regardless of whether they are blood stasis accumulations, Qifei Wan (Revive the Wasted Pill) is effective.
If there is a mass in the abdomen with internal tension and pain, and the body is emaciated, it is called Ganxue Lao (Dry Blood Consumption); long-term use of Qifei Wan is appropriate.
Fanwei (Regurgitation/Vomiting of ingested food) is difficult to treat, but if stagnant fluids are expelled and Stomach Qi is harmonized, recovery is possible. Long-term use of Xiao Banxia Jia Fuling Tang (Minor Pinellia plus Poria Decoction) is suitable, occasionally supplemented with Dahuang Gancao Wan (Rhubarb and Licorice Pill) to eliminate putrid filth.
Zhongfeng (Wind Stroke) sudden collapse is difficult to treat; intermittent use of Fuzi Xiexin Tang (Aconite Heart-Purging Decoction) can yield results.
For hemiplegia with slurred speech, Maimendong Tang (Ophiopogon Decoction) added with Shigao (Gypsum Fibrosum) is indicated. For pure hemiplegia, Xuming Tang (Restore Life Decoction) is given. In this condition, Shigao is paramount, with a dose of up to 5 qian per prescription. (The use of Shigao for hemiplegia originates from Yamawata Toyo’s interpretation of Xuming Tang and Fengyin Tang; Master Fukuda shared this view simultaneously, highlighting the vitality of ancient formulas.) (Zhuoxuan said: Maimendong Tang with Shigao seems contrary to Zhang Zhongjing’s original intent, yet it often produces miraculous effects. The subtlety of ancient formulas is inconceivable.)
Hemiplegia, paralysis, and flaccid numbness are all due to the decline and dysfunction of Yang Qi; thus, even using herbs like Wutou (Aconiti Lateralis Radix Praeparata) and Fuzi (Aconiti Radix Praeparata) fails to produce efficacy.
Xiuxili (Resting Dysentery/Dysentery with remissions) occurs because foul matter remains incomplete; it is suitable to administer Duoluo Wan (Fallen Dust Pill) to purge, combined with Banxia Xiexin Tang (Pinellia Heart-Purging Decoction).
Chronic diarrhea that does not stop, presenting similarly to resting dysentery but without pus and blood, only watery stools coming and going, abdominal fullness with intermittent pain, relief after defecation, progressive emaciation, sallow complexion, nausea, or acid regurgitation, cannot be effectively treated except with Baidu (Crotonis Fructus). Therefore, using Duoluo Wan combined with Banxia Xiexin Tang is ideal. Ziwan (Purple Pill) for chronic dysentery follows this same principle.
Xianzheng (Epilepsy) presents in myriad ways, impossible to list exhaustively. However, for symptoms such as frequent eyelid twitching and rapid, forced breathing resembling sobbing, Sanhuang Xiexin Tang is most effective. If rebellious Qi is severe with spontaneous sweating, add Muli (Concha Ostreae) to the previous formula. If strange manifestations appear, combine with Chensha Wan (Cinnabar Pill).
Generally, epilepsy is best treated with Wendan Tang (Warm the Gallbladder Decoction) from *Qianjin Fang*. For any symptoms that vary unpredictably, this is due to stagnation of Liver and Gallbladder Qi; one should prioritize this formula and not change it rashly due to symptom fluctuations.
I once treated a boy purchased in the market who suddenly presented with cold extremities, eyes fixed upward (Daiyan), and unconsciousness, diagnosing him with epilepsy. I administered Sanhuang Jia Mangxiao Tang (Three Yellow plus Nitre Decoction). After three days without improvement, I consulted Master Songyuan Bai. He prescribed Fengyin Tang (Wind-Guiding Decoction) for three doses, resulting in complete recovery. An eighteen-year-old man, previously suffering from oral ulcers and redness/swelling, suddenly became unable to speak or look straight, with a rock-hard sensation below the heart. Upon waking, he relapsed. I applied the previous treatment with Fengyin Tang for ten doses, restoring consciousness, followed by Sanhuang Tang for full recovery.
For epilepsy patients with scorched tongue coating or slippery white coating like soaked water, internal administration of Maimendong Tang is suitable, while external application of Huanglian (Coptidis Rhizoma) and Shigao powder to the area leads to cure.
Lord Tarao had an irritable and constrained temperament, suffering from seminal emission for years. He was unaware of leakage even while sitting with others; various treatments failed. I diagnosed this as epilepsy and prescribed Sanhuang Xiexin Tang, resulting in complete recovery.
Internal hemorrhoids are difficult to cure due to accumulated toxins internally. It is necessary to thoroughly expel these toxins; Yupi (Cornus Officinalis Bark - *Note: Context implies a specific herb for hemorrhoids, likely Diyu/Sanguisorbae Radix or similar, but text says Yupi*) is most effective; the same applies to hemorrhoidal fistulas. Long-term use of purgatives can cleanse all toxins; do not indiscriminately apply external remedies seeking quick fixes.
There are diseases that must be treated, such as sweating, vomiting, and purging. If the timing is missed, the disease becomes untreatable. There are conditions where treatment is less effective than non-treatment, such as Crural Phthisis (Knee-Joint Swelling) and Liuzhu (Flowing Abscesses). Why? Meridians have structure but are invisible externally. Reckless acupuncture often impairs flexion and extension. If one takes tonifying drugs to support the interior, the toxic vapors exit outward, eventually suppurating and ulcerating spontaneously, leaving no future complications. Hence I say: non-treatment is preferable. Those treating sores and swellings must understand this.
Regarding dog bites and rat bites, ancient and modern texts discuss treatments extensively, but cat bites are rarely mentioned. I was once bitten by a domestic cat, experiencing indescribable pain and distress. I searched for formulas for venomous animal bites and decocted Shuijing (Rock Crystal) alone to drink; the illness vanished instantly. When it recurred, I prepared Huanglian Jiedu Tang (Coptis Detoxification Decoction) with Hu Jinggu (Tiger Tibia Bone - *Note: Modern ethics prohibit this, historical context noted*) taken concurrently, achieving full recovery after dozens of days.
I once observed a person polishing an ancient mirror using Shiliupi (Pomegranate Peel); the silver luster peeled away revealing copper color, revealing what Mercury fears. The world uses Shiliupi exclusively to detoxify Qingfen (Calomel), which is indeed justified.
For edema with rebellious attack, or Hongzhangjiao (Gout/Lower Limb Swelling) attacking the heart near death, take one piece of Baidu (Croton Seed), remove skin and crush, stir-fry together with Chixiaodou (Adzuki Bean), then discard the Baidu. Decoct the Chixiaodou alone and drink; it works instantaneously. Alternatively, adding this preparation to Chixiaodou Tang is also good.
For unbearable toothache, Taohé Chengqi Tang (Peach Kernel Draining-Chi Decoction) is suitable. (For caries, dental abscesses, noma, alveolar pyorrhea, and other unbearable toothaches, I have used this formula with frequent success, largely because these cases often involve rebellious Qi and Blood.)
A patient suffered from hiccups for about fifty days; many doctors were helpless. I examined his abdominal signs and prescribed Jianzhong Tang (Establish the Middle Decoction) for two doses, stopping the hiccups completely. (Note: Western theory attributes hiccups to diaphragm spasm; Jianzhong Tang’s efficacy here suggests Master Fukuda’s precise diagnostic thinking achieved this result, rather than belief in Western theories.)
*Xiwai Mi Yao*’s Xipi Tang treats Pi accumulation turning into consumption, known generally as accumulations and masses. For those with abdominal pain, this formula often yields results.
For mania, administer Sanhuang Jia Mangxiao Tang, combined with the "Waterfall Spring" irrigation therapy for optimal effect. The irrigation method involves laying the patient flat, binding them to a ladder with hemp rope, covering their head with a hand towel, then pouring water over Baihui (GV20). Use hands to shield the forehead, eyes, and nose, then pour over Tinggong (EX-HN18/Tinggong area - *Text says Tingting, likely Tinggong or similar anterior point*), then down to the chest at Danzhong (CV17). This makes the procedure bearable and effective. (Muddy water is poor; choose clear, cold water.)
For diffuse swelling that is hard, with unchanged skin color but intense heat, fumigate with Fanshi Tang (Alum Decoction) to dissipate it; this is most effective for Xuan Yong (Perineal Abscess), Lin Lou (Leaking Fistula), and hemorrhoidal toxins. It also treats persistent paralysis and Hongzhangjiao attacking the heart.
When a prostitute first enters a brothel and has sexual contact for over ten days, she inevitably develops chills, fever, and abdominal pain, commonly called Yin Abdominal Pain. Hairuo (Sargassum/Hairy Seaweed) can treat this. If chills and fever persist, Xiao Chaihu Tang (Minor Bupleurum Decoction) added with Hairuo is suitable. (Note: *Lanxian Yitan* records successful cases of Hairuo Tang. Generally, seaweed avoids damp-miasma, hence prostitutes in the capital often eat Qing Haitai (Green Seaweed). *Dahe Bencao* states that patients with Syphilis/Kanyame Chuang eating Kelp prevent facial eruptions, which is another proof.)
Ren Zhongbai (Urine Sediment) treats Blood Syncope. Whether postpartum, prenatal, or from traumatic injury, taking a small amount dissolved in Jinghua Shui (Well Water) stops syncope immediately. A woman suffered from facial deviation and hemiplegia after childbirth. I prescribed Guiling Wan material added with Chenxiang (Agarwood) and Ren Zhongbai, curing her. Since this was a disorder of the Blood aspect, Ren Zhongbai, which enters the Blood aspect, was effective. (For severe redness, ulceration, and pain of the mouth/tongue pre/post-partum, applying Ren Zhongbai externally is effective due to its entry into the Blood aspect.)
For traumatic bleeding that is difficult to stop, tightly bind with paper strips, sprinkle Danhongfen (Light Red Powder) between layers, binding and sprinkling simultaneously. Finish wrapping; if movement causes bleeding to resume, it will stop. For more severe cases, apply Fanshi Fen (Alum Powder); pain and bleeding will cease.
Epilepsy has several syndromes. Those belonging to Fire-Heat and those belonging to Blood Stasis require differentiation via tongue coating. If the coating is yellow or black, with frequent rebellious Qi sensation, and pulse is rapid and forceful, it is Fire-Heat syndrome: Maimendong Tang added with Shigao, or Chaihu Jia Shigu Tang (Bupleurum plus Gypsum Decoction), selecting Waterfall Spring therapy. If accompanied by bleeding symptoms, it is Blood Stasis syndrome: Sanhuang Xiexin Tang added with Xijiao (Rhinoceros Horn - *Note: Substitute with Buffalo Horn in modern practice*), Mangxiao (Mirabilite), or Chenxiang, Jianghuang (Curcuma Longa Rhizoma). If there are convulsions of hands and feet, Tianma (Gastrodiae Rhizoma) is suitable. Occasionally, women recover spontaneously after old age, similar to epileptic women recovering spontaneously postpartum without medication; the principle is the same.
Jinkouli (Closed-Mouth Dysentery) may suit Banxia Xiexin Tang added with Binglang (Areca Seed), or Zhenwu Tang (True Warrior Decoction); treatment cannot be generalized.
For women with amenorrhea forming Zheng Jia (Masses) or developing Drum Distension (Ascites), moxa the Shu points of Kidney, Large Intestine, Small Intestine, Bladder, and Yaoyan (Lumbar Eye points) to over 100,000 moxa cones; this will certainly be effective.
For Huangpan (Yellow Puffy Disease/Iron-deficiency anemia-like condition) where Iron Powder is ineffective, Chensha (Cinnabar) is suitable.
A patient suffered from Cold Damage (Shanghan). After recovery, he could not eat for a long time; other doctors failed. I diagnosed palpitations in the abdomen and prescribed Guizhi Jia Longgu Muli Tang (Cinnamon Twig plus Dragon Bone and Oyster Shell Decoction); appetite suddenly returned to normal.
Doctors are classified into Superior (Shanggong) and Inferior (Xiagong). Those who fixate on formulas hoping for efficacy against a disease are Inferior. Those who observe the pathogenic mechanism clinically and ensure herbal harmony are Superior. Observing mechanism and ensuring harmony seems slow but is actually swift; this is gained by the wise and lost by the foolish.
Human life follows natural laws, and disease does not exist outside human physiology. Therefore, physicians must examine these principles to treat; otherwise, treatment becomes more erroneous. Thus, the writings of Changsha (Zhang Zhongjing) aim to correct these abuses; should we not learn from them?
