Heatstroke manifests as fever without aversion to cold, body pain, vomiting, dark urine, and diarrhea. This indicates that the pathogenic factor is distributed throughout the Sanjiao (Triple Burner), affecting both upper and lower parts simultaneously. It is appropriate to use Liu Yi San (Six-to-One Powder) combined with Huang Qin Tang (Scutellaria Decoction), added with Xing Ren (Apricot Seed), Hou Po (Magnolia Bark), Chi Huang Qin (Red Scutellaria Root), Dou Juan (Soybean Sprout), Fen Ge Gen (Kudzu Root), Lian Qiao (Forsythia Fruit), etc., to clear and drain the Sanjiao.
This condition, like the previous one, involves the Upper Jiao, but the presence of dark urine and diarrhea indicates that the pathogen has spread to the Sanjiao. Its mechanism is similar to the concept of "combined disease of the three Yang meridians" causing diarrhea in the *Shanghan Lun* (Treatise on Cold Damage). Since sweating fails to resolve the condition and the Qi-level pathogen remains unresolved, the pathogen has no exit and must transmit downward, leading to diarrhea. Therefore, by imitating Liu Yi San combined with Huang Qin Tang and adding Xing Ren, Ge Gen, Hou Po, Lian Qiao, Chi Huang Qin, and Dou Juan, the pathogenic factors in the exterior and interior of the Sanjiao can be resolved simultaneously, ensuring a comprehensive treatment.
In heatstroke, there is sweating, but the body temperature is not significantly high; however, the tongue coating is yellow and greasy, accompanied by mental restlessness, oppression, and a desire to vomit. This indicates that the pathogen occupies the Lung and Stomach and lingers unresolved. It is appropriate to use Huang Lian Wen Dan Tang (Coptis Warm Gallbladder Decoction). Using bitter herbs to descend rebellious Qi and acrid herbs to unblock Yang, employing mobile agents, the goal is still to resolve the pathogen through sweating.
The presence of sweating without significant heat indicates that the pathogen in the Wei (Defensive) level has been resolved. However, the yellow tongue coating and desire to vomit indicate that the pathogen obstructs the Lung and Stomach, and the Qi level remains turbid. The acrid nature of Wen Dan Tang unblocks Yang, while the addition of Huang Lian uses bitterness to descend rebellion. Sweet, sour, and cloying herbs are avoided to prevent the pathogen from lingering indefinitely.
After six or seven days of heatstroke, the pulse is surging and rapid, with thirst and a dry tongue with red coating. This signifies that heat has depleted the Stomach fluids, causing the Qi and Blood to be scorched and steamed. It is appropriate to use Yu Nu Jian (Jade Woman Decoction), added with fresh Shi Hu (Dendrobium Stem), Hua Fen (Trichosanthes Root), Mai Dong (Ophiopogon Tuber), Li Zhi (Pear Juice), and Zhe Jiang (Sugarcane Juice), to cool both the Qi and Blood levels.
In the previous case, the exterior pathogen was resolved, leaving residual pathogens lingering. In this case, the exterior pathogen has transmitted inward, moving from the Qi level to the Blood level. Excessive Yang flares up, consuming fluids. Hence, there is thirst, a dry tongue, red color with little coating, indicating that dampness has fully transformed into heat. Combined with the surging and rapid pulse, mental irritability, thirst, and preference for drinking, it is evident that the excessive heat exists not only in the Qi level but also in the Blood level. Therefore, Yu Nu Jian is used to cool both Qi and Blood, and Hua Fen, Shi Hu, Mai Dong, Li Zhi, and Zhe Jiang are added. These sweet and cold substances generate fluids, aiming to prevent the progression to black tongue and coma.
