Zhenlu (a skin eruption disease) presents with fever, aversion to cold, chest tightness, abdominal pain, irritability and heat sensation with a desire to vomit. This indicates pathogenic factors stagnating in the Lung and Stomach. It is appropriate to use Xingren (Prunus armeniaca Seed), Jiegeng (Platycodon Root), Zhike (Aurantii Fructus), Yujin (Curcuma Longa Rhizome), Danshichi (Fermented Soybean), Heishanzhi (Gardeniae Fructus processed with charcoal), Bohe (Menthae Haplocalycis Herba), Nidali (Radix Acanthopanacis Senticosi), Can'erzi (Xanthii Fructus), Lianqiao (Forsythiae Fructus) and other herbs, to employ light and ascending actions to diffuse Lung Qi.

Zhen belongs to Wind-Heat of the Lung Meridian, while Luo (referring to the larger eruptions here) belongs to Damp-Heat in the Stomach. Therefore, Zhen particles are fine like mosquito bites and appear on the skin surface; Luo particles are large like coins and adhere to the muscle layer. This is because the Lung governs the skin and hair, and the Stomach governs the muscles. The onset must be preceded by chest and diaphragm fullness and colicky abdominal pain, caused by the obstruction of Qi mechanism due to congestion of Wind-Heat, leading to incessant vomiting in severe cases. However, if vomiting or diarrhea occurs alongside Zhenlu, it signifies that the stagnant pathogenic factors are being vented, and there is no need to stop them forcibly. Thus, Xingren, Yujin, Zhike, and Jiegeng are used to open the Upper Jiao; Shanzhi and Danshichi dispel stagnation; Bohe and Nidali expel Wind; Lianqiao and Can'erzi resolve Heat. Consequently, the pathogens are released, and the eruptions resolve spontaneously.

Zhenlu may appear intermittently or remain hidden, accompanied by unbearable itching, indicating internal steaming of Wind-Heat. It is appropriate to use Bohe, Nidali, Huanglian (Coptidis Rhizoma), Huangqin (Scutellariae Radix), Jingjie (Schizonepetae Spica), Fangfeng (Saposhnikoviae Radix), Chanyi (Bombyx Batryticatus), Danpi (Moutan Cortex), Lianqiao, Heishanzhi, Xingren, Difuzi (Kochiae Fructus) and other herbs, to clear Heat and expel Wind.

Once Zhenlu has appeared, sometimes hiding and sometimes showing, with generalized itching, it is largely due to the accumulation of Wind-Heat causing turbidity in Qi and Blood. Therefore, Jiuchao Qin (Scutellariae Radix fried with wine) and Jiuchao Lian (Coptidis Rhizoma fried with wine) are combined with Lianqiao and Heishanzhi to clear Fire; Bohe, Nidali, Fangfeng, and Jingjie expel Wind; Chanyi, Danpi, Xingren, and Difuzi clear both Qi and Blood levels. When Heat is cleared, the eruptions transform and disappear.

Before Baiyin (white miliary rash) appears, symptoms include fever, body pain, dull and sallow complexion, sticky tongue coating, chest and epigastric discomfort, nausea and vomiting, loose stools, and a large yet slow pulse. This indicates Heat accumulation and steaming. It is appropriate to use Huanglian Wendan Tang (Coptis Phlegm-Calming Decoction) added with Xingren, Loupi (Trichosanthis Pericarpium), Tongcao (Tetrapanacis Medulla), Doujuan (Sojae Semen Praeparatum) and other ingredients, to unblock and drain the Sanjiao.

The initial onset of Yin is invariably caused by the transformation of Damp-Heat into steaming, resulting in the indistinction between Qi and Dampness. Dampness inevitably obstructs Qi, and Heat damages Qi. Once Dampness transforms into Heat, it steams the Lung and Stomach. The pathogen moves from the Stomach to the Lung, emerges from the Lung to the exterior, and is excreted through sweat. Thus, the particles resemble fine millet grains and have a crystal-clear color. Therefore, Baiyin is most common in the humid and hot regions of the Southeast, whereas Maculae et Petechiae (spotted rashes) prevail in the dry and windy regions of the Northwest. Ancient medical texts record only Maculae et Petechiae but not Baiyin. Only with the emergence of Southern physicians in recent times has this condition been discussed, albeit briefly and incompletely. One should realize that south of the Yangtze River, the terrain is low and the climate damp. During summer and autumn, when暑秽 (summer heat and filth) steams, one feels persistent fever, dull complexion, and greasy tongue coating, which are signs of Damp-Heat. Internal pathology manifests externally. Moreover, Dampness is a heavy and turbid pathogen, and Heat is a steaming qi; they first injure the Qi level and easily transform into Heat. Hence, chest and abdominal stuffiness and a large, slow pulse are inevitable. Furthermore, nausea, vomiting, and loose stools are symptoms of pathogens pervading the Sanjiao and loss of clarity in Qi transformation. Therefore, Banxia (Pinelliae Rhizoma) and Fuling (Poria) are used to circulate the Middle Jiao; Xingren and Loupi are added to diffuse the Upper Jiao; Tongcao and Doujuan are used to leach out the Lower Jiao; Huanglian and Zhishi (Aurantii Fructus Immaturus) are combined to purge fullness; Zhuru (Bambusae Caulis in Taenia) and Chenpi (Citri Reticulatae Pericarpium) regulate Qi. This ensures the smooth flow of Qi mechanism in the Sanjiao, allowing the Damp-Heat pathogens to be dispersed and eliminated separately.
