To treat obstruction in the diaphragm and pharynx, with rebellious Qi rising and internal urgency, accompanied by constipation.
Qingpi (Cortex Citri Reticulatae), Huaizi (Fructus Sophorae Tonkinensis) (each 2 fen), Shengdihuang (Radix Rehmanniae Preparata? No, Fresh: Radix Rehmanniae Recens), Shudihuang (Radix Rehmanniae Preparata), Huangbai (Cortex Phellodendri) (each 3 fen), Dangguishen (Radix Angelicae Sinensis, root body), Gancaoshao (Radix Glycyrrhizae Preparata, tip) (1 qian), Taoren (Semen Persicae) (10 seeds, ground separately).
The above herbs are chopped like hemp or bean seeds, generally taken as one dose. Add the ground Taoren paste and two zhan of water, decoct to one zhan, strain, and take warm before meals.
To treat wind-type or blood-deficiency-type constipation with frequent dryness and hard stools.
Dangguishaoo (Radix Angelicae Sinensis, root tip) (1 qian), Fangfeng (Radix Saposhnikoviae) (3 qian), Dahuang (Radix et Rhizoma Rhei, wrapped in wet paper and baked), Qianghuo (Rhizoma Notopterygii), Zaojiaoren (Fructus Gleditsiae Sinensis, burned to retain its nature, peeled) (1 liang 5 qian; its nature becomes slippery when moist, and moisture eliminates dryness and constipation), Taoren (Semen Persicae) (2 liang, ground into paste), Haren (Semen Cannabis) (2 liang 5 qian, ground into paste).
Except for Haren and Taoren which are ground into paste separately, the remaining herbs are pulverized into an extremely fine powder. Mix with refined honey to make pills the size of paulownia seeds. Take 50 pills per dose with plain boiled water. After taking the medicine for three days, one must consume Su-Ma-zi porridge (perilla and hemp seed porridge) every morning and evening, because long-standing constipation cannot be resolved solely by medication. Store the pills in porcelain vessels, seal with paper, and prevent exposure to wind.
To treat large intestine dryness and obstruction.
Shengdihuang (Radix Rehmanniae Recens), Shenggancao (Radix Glycyrrhizae Preparata, fresh) (each 1 qian), Dahuang (Radix et Rhizoma Rhei, baked), Shudihuang (Radix Rehmanniae Preparata), Dangguishaoo (Radix Angelicae Sinensis, root tip), Shengma (Rhizoma Cimicifugae), Taoren (Semen Persicae), Haren (Semen Cannabis) (each 1 qian), Honghua (Flos Carthami) (3 fen).
The above herbs are chopped, decocted in two zhan of water until reduced to one zhan, strained, and taken warm well after meals.
The *Nan Jing* (Classic of Difficult Issues) states: Diseases involve Guan (closure) and Ge (regurgitation/obstruction). Guan means urine cannot be passed. It also says "Guan" implies no exit. These are all diseases caused by pathogenic heat. Treatment is differentiated based on whether the pathogen is in the Qi level or Blood level, using thirst or lack thereof as a diagnostic criterion.
If there is thirst and difficulty urinating, this indicates heat in the Upper Jiao (Upper Burner), specifically in the Lung division. Thus, there is thirst and urinary difficulty. Urination is governed by the Foot-Taiyang Bladder Meridian. The Bladder meridian is born at Shen hour, which corresponds to West Metal. Lung combines with Water generation (Metal generates Water). If there is heat in the Lung that prevents water generation, it cuts off the source of water. The Classics state: "Deficiency should be tonified via its mother." It is appropriate to clear the Lung to nourish the source of transformation. Therefore, treatment should focus on the Lung division to assist the Autumn energy, allowing water to generate naturally. Just as rain, dew, and frost descend from heaven, so does the Yin of Yangming (referring to Autumn air), descending from heaven. Herbs with thin aroma and taste belong to Yin within Yang, generated by sensing the clear and肃 (solemn/killing) Qi of Autumn. They can supplement Lung deficiency. Bland-tasted, draining, and leaching herbs such as Fuling (Poria), Zexie (Rhizoma Alismatis), Hupo (Subcorallium Rubrum), Dengxincao (Medulla Junci), Tongcao (Tuber Akebiae), Cheqianzi (Semen Plantaginis), Mutong (Caulis Akebiae), Qumai (Herba Dianthi), Pianxu (Herba Polygoni Avicularis) etc., are used to clear Lung Qi, drain fire, and aid the upper source of water.
If there is no thirst but urinary obstruction, this indicates heat in the Lower Jiao (Lower Burner) Blood level. Hence, no thirst but extreme dryness and urinary obstruction. Heat closing the Lower Jiao involves the Kidney and Bladder, which is Yin within Yin. When Yin is blocked by pathogenic heat, the flow tends to obstruct upwards. Old physicians said: "Cold in the chest blocks entry; heat in the Lower Jiao fills and obstructs passage." One must use herbs that sense the transformation of Northern Cold Water, with both aroma and taste being Yin, to eliminate heat and unblock obstruction. The *Nei Jing* (Inner Canon) states: "Without Yang, Yin cannot generate; without Yin, Yang cannot transform." If bland-draining herbs are used, their nature is Yin within Yang, not pure Yang agents. Without Yang, there is no transformation, so how can they supplement severe Yin deficiency? One must use herbs that sense Earth's water movement and have bitter tastes, and herbs generated by Heaven's cold with great cold qi. These herbs have both aroma and taste as Yin, representing Yin within Yin. Great cold qi allows humans to possess the operation of Bladder cold water; sensing it generates Kidney function. These herbs tonify the Kidney and Bladder, protecting them from Yang-within-Yang fiery heat pathogens that close the Lower Jiao and cause urinary obstruction. Therefore, when using great bitter-cold herbs, the treatment method should be "treating heat with cold" (Han Yin Re Yong). It is also said: "Must subdue what dominates, and first address the cause." Initially, the Qi is similar; finally, the Qi is different.
