Translation:
Treatment Principle: When all symptoms are present and digestive function has recovered with normal bowel movements, if the rash has not yet erupted, do not forcibly induce eruption; if the rash has already erupted and is sparse, do not allow it to increase further.
Prescription Composition: Fangfeng (Saposhnikoviae Radix), Qianghuo (Notopterygii Rhizoma et Radix), Mahuanggen (Ephedrae Radix et Rhizoma), Shengma (Cimicifugae Rhizoma), Shengdihuang (Rehmanniae Radix), Lianqiao (Forsythiae Fructus) (reduce dosage for newly erupting ones, increase for fully erupted ones), Jiu Huangbai (Phellodendri Cortex prepared with wine) (each 5 fen), Dangguishen (Angelicae Sinensis Radix), Huanglian (Coptidis Rhizoma) (each 3 fen), Chuanxiong (Ligusticum Chuanxiong Rhizoma), Bencao (Ligustici Wallichii Rhizoma), Chaihu (Bupleuri Radix), Gegen (Puerariae Lobatae Radix), Jiu Huangqin (Scutellariae Radix prepared with wine), Sheng Huangqin (Scutellariae Radix raw), Cangzhu (Atractylodis Rhizoma) (each 2 fen), Xixin (Asari Radix et Rhizoma), Sheng Gancao (Glycyrrhizae Radix et Rhizoma raw), Baizhu (Atractylodis Macrocephalae Rhizoma), Chenpi (Citri Reticulatae Pericarpium), Sumu (Sappan Lignum), Honghua (Carthami Flos) (each 1 fen), Wuzhuyu (Evodiae Fructus) (0.5 fen).
Decoction and Administration: Crush the above herbs to the size of hemp seeds. Take 5 qian per dose, add two large zhen of water, decoct until one zhen remains, remove dregs, and take slightly warm on an empty stomach.
Pathogenesis Discussion: The eruption of rashes is always due to internal injury and must erupt; this is caused by the rebellion of Ying Qi (Nutritive Qi). It is strictly forbidden to use purgative foods or drugs such as Qianniuzi (Fructus Phytolaccae) and Badou (Crotonis Fructus). It is appropriate to use substances such as Banxia (Pinelliae Rhizoma), Zhizhu Wan (Bitter Orange and Atractylodes Macrocephala Pill), Dahuang (Rhei Radix et Rhizoma), and Yizhiren (Alpinia Oxyphylla Fructus) to treat diarrhea and stop vomiting. If the ear tips are cold, there are yawning, sneezing during sleep, startle, and dry eyes, it indicates that the rash will inevitably erupt. Various large pustules, small vesicles, and urticarial rashes are all caused by the rebellion of Ying Qi and the re-accumulation of cold in the exterior. It is appropriate to add Dangguishen and Lianqiao to Siwei Shengma Tang (Four-Ingredient Cimicifuga Decoction); this is a fixed method.
Modification for Complications:
If the Lung forms pus, characterized initially by cough and wheezing, or rapid breathing and panting, add Renshen (Ginseng Radix et Rhizoma) and a small amount of Huangqin (Scutellariae Radix) to drain latent fire and tonify Yuan Qi (Primary Qi).
If the Heart manifests as small red rashes, it must first present with dry throat, palpitations, fever, muscle swelling, and a wiry and surging pulse; add a small amount of Huanglian (Coptidis Rhizoma).
If the Mingmen (Gate of Life) manifests as urticarial rashes, it must first present with bone pain, fever, and pain preventing movement; add a small amount of Shengdihuang (Rehmanniae Radix) and also add Huangbai (Phellodendri Cortex).
Summary: All rashes belong to Yin-type sores and are invariably caused by internal injury from diet, insufficiency of the Spleen and Stomach, and rebellion of Ying Qi. Although fire-heat blazes internally, Yin-cold covers the exterior. The treatment method is as described above.
Differentiating Pediatric Rash Symptoms:
Symptoms include: yawning, sneezing, startle during sleep; or cold earlobes, dry eyes; or differentiating whether food stagnation is involved: hot mouth, sour breath odor, undigested milk curds; or abdominal pain.
Treatment Strategy:
If only few symptoms are present and the rash has not yet erupted, administer Shengma Tang (Cimicifuga Decoction) 3–5 qian, taken warm. Wait until the body surface feels mild and the rash becomes visible before stopping medication.
If the body is cool and the rash has not erupted, after confirming the diagnosis of rash, continue administration regardless of the number of doses, waiting only until the body surface feels mild and the sores/rash become visible before stopping. Only occasionally administer Jiegeng Tang (Platycodon Decoction) to widen the chest and diaphragm and benefit the throat.
