Treatment for hemorrhoidal diseases and hemorrhoidal fistulas presenting with pus and blood, dry and hard stools, and unbearable pain.
Qinjiao (Gentianae Radix et Rhizoma, remove rhizome) 1 liang, Taoren (Persicae Semen, soak in hot water, remove skin and tips) 1 liang, Zaojiaoren (Gleditsiae Fructus Spina, burn until carbonized but retain efficacy) 1 liang, Danggui Shao (Angelicae Sinensis Radix tip, soak in wine), Zexie (Alismatis Rhizoma), Zhishi (Aurantii Fructus Immaturus, stir-fried with bran to yellow), Baizhu (Atractylodis Macrocephalae Rhizoma, each 5 qian), Diyü (Sanguisorbae Radix) 3 qian.
Grind the above herbs into a fine powder, mix evenly with Persicae Semen paste, use hot water to blend wheat starch paste to form pills the size of a corn kernel, ensuring the surface is smooth, then bake dry. Take 50–70 pills per dose, administered with warm water. Take on an empty stomach. After taking the medicine, wait a short while and follow with a nutritious meal. Avoid raw, cold, hard foods, cold water, cold vegetables, damp flour, alcohol, and pungent, spicy, or hot spices. Violation of these dietary restrictions will render the medicine ineffective.
Regarding the treatment of hemorrhoids, if they ulcerate, it is called hemorrhoidal fistula. Constipation and涩滞 (obstruction/stagnation) inevitably cause severe pain. This arises from wind-heat pathogens invading due to overeating leading to qi stagnation, which forces the large intestine to malfunction. The patient's constitution is characterized by excess dryness; the pathogenic factor involves dampness in the stomach. Stomach dampness penalizes the large intestine, transforming into dry fire to attack the existing dry-heat excess. Wind attaches to heat, resulting in the combination of four pathogens: dampness, heat, wind, and dryness. Therefore, the formation of masses at the head of the large intestine is due to dampness; severe pain is due to wind; if stools are dry and knotted, it indicates the primary disease combined with fire-heat accumulation and obstruction. To eliminate these four pathogens, one must consider that the Lung (Metal/West) governs all qi and has a contracting and descending nature, which may also aid the pathogen as an evil factor. Thus, qi-breaking herbs must be added, completing the treatment method. Administer the decoction, and the effect will be miraculous.
Qinjiao (Gentianae Radix et Rhizoma, remove rhizome) 1 qian, Taoren (Persicae Semen, soak in hot water, remove skin, grind separately) 1 qian, Zaojiaoren (Gleditsiae Fructus Spina, burn until carbonized, grind separately) 1 qian, Cangzhu (Atractylodis Rhizoma, processed), Fangfeng (Saposhnikoviae Radix, each 7 fen), Huangbai (Phellodendri Cortex, remove bark, wash with wine) 5 fen, Danggui Shao (Angelicae Sinensis Radix tip, wash with wine), Zexie (Alismatis Rhizoma, each 3 fen), Suoshen Binglang (Arecae Semen, 1 fen, grind separately), Dahuang (Rhei Radix et Rhizoma, a small amount; even if constipation is severe, do not use excessively).
Except for Arecae Semen, Persicae Semen, and Gleditsiae Fructus Spina, chop the remaining herbs into pieces the size of hemp seeds. Usually, this constitutes one dose. Use three cups of water, decoct to one cup and two fen, strain, add the powders of Arecae Semen and the other two herbs, return to heat, and decoct to one cup. Take hot on an empty stomach. After taking the medicine, wait a short while and follow with a nutritious meal to prevent damage to stomach qi. Daily during medication, avoid raw, cold, hard foods, alcohol, damp flour, spices, and dried ginger. Violation of these restrictions will render the medicine ineffective.
If white pus is present, add five heads of Bai Kuihua (Abutilonis Indici Flos, remove calyx), Qingpi (Citri Reticulatae Pericarpium Viride, 5 qian, keep the white pith), and decoct together with the main herbs. Muxiang (Aucklandiae Radix, 3 fen) should be ground into a fine powder and taken according to the previous decoction method along with Arecae Semen and the other two powders. Ancient physicians treated this disease over years, but this single dose can cure it.
