Postpartum amenorrhea (absence of menstruation after childbirth) does not necessarily require medication. The Chong Mai (Penetrating Vessel) and Ren Mai (Conception Vessel) in women are the seas of the meridians, originating from within the Bao Gong (Uterus). The Hand-Taiyang and Hand-Taiyin meridians transform upwards into Ru Zhi (Breast Milk) and downwards into Yue Shui (Menstrual Blood). If excessive blood loss occurs postpartum, breast milk may often be obstructed; if menstruation does not occur within six months to one year of nursing, this is a common physiological state. If menstruation resumes around the time of weaning (approximately six months), it indicates a person who is young, vigorous, and has abundant blood. If menstruation remains absent for one to two years postpartum without other discomforts, there is no need to take medications to promote menstruation, as this may result from labor injury damaging Ying Wei (Nutritive and Defensive Qi), leading to deficiency of the Chong and Ren vessels and depletion of Qi and Blood. Simply taking herbs that strengthen the Spleen and Stomach and tonify Qi and Blood will naturally restore flow. Forcing menstruation would be akin to pulling up seedlings to help them grow.
If a postpartum woman has abundant blood in the Chong and Ren vessels, strong Qi in the Spleen and Stomach, and regular diet, her breast milk will be sufficient and thick because the source of biological transformation is robust. If Spleen and Stomach Qi is weak, food intake is low, and the Chong and Ren vessels are inherently deficient, the breast milk will be scanty and thin, causing the nursed infant to be frail and prone to illness. High-quality breast milk is characterized by being thick, white, lustrous, and clear like jade when poured into a cup; yellow, clear, and thin breast milk is inferior and should not be fed to infants. A lactating mother should ideally be chosen for having a moderate body build, absence of disease, regulated menstruation, and good appetite. Being too fat leads to excessive Phlegm-Dampness, while being too thin leads to internal Fire; if an infant drinks such milk, similar conditions may arise. For instance, an infant fell into a deep sleep and did not wake all day, causing family panic. Medical treatment was ineffective until a skilled physician diagnosed: "This child has alcohol intoxication; did the lactating mother drink heavily?" Upon inquiry, this was confirmed. The infant woke after stopping the medication, clearly demonstrating the significant impact of maternal habits on the infant. However, Li Shizhen stated: "Human breast milk has no fixed nature; it changes according to the properties of food and drink. Therefore, dietary regulation is crucial for lactating mothers."
Regarding the failure of breast milk secretion, there are two types: one is obstruction due to excess Qi and Blood stagnation, and the other is dryness and涩滞 (stagnation) due to Qi and Blood deficiency. Deficiency cases are treated with tonification, such as using Shi Quan Da Bu Tang (Ten Significant Great Tonifying Decoction) or Ba Zhen Tang (Eight Treasures Decoction). Excess cases are treated with dispersing methods, such as consuming Mai Dong (Ophiopogonis Radix), Gua Lou Ren (Trichosanthis Semen), Tian Hua Fen (Trichosanthis Radix), Ren Shen (Ginseng Radix), Kui Zi (Abelmoschi Semen), Zhu Yi (Porcine Pancreas), Mu Tong (Akebiae Caulis), Lou Lu (Cimicifugae Rhizoma), and Zhu Ti (Porcine Trotters) cooked as food.
For spontaneous leakage of breast milk: If caused by weak Stomach Qi failing to consolidate fluids, tonify Stomach Qi to consolidate. If caused by severe Qi and Blood deficiency, where Qi fails to protect the exterior and Blood fails to nourish the interior, leading to reckless discharge, harmonize and tonify Ying and Wei Qi to stop it. If breast milk leaks before delivery, it is called Ru Qi (Weeping Breast); children born under this condition often fail to survive. If a postpartum woman experiences gushing breast milk due to overexertion, this is a Jue syndrome (reversal condition) due to Yang Qi deficiency, treated with Du Shen Tang (Single Ginseng Decoction).
