Wan Tou Chuang (Bowl-Head Sore) may also occur due to residual toxins not being fully cleared after sweating or purging therapies, thus manifesting as Wan Tou Chuang after recovery. It can be treated by decocting and administering herbs such as Huanglian (Coptidis Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma), Guiwei (Angelicae Sinensis Radix), Honghua (Carthami Flos), Fangfeng (Saposhnikoviae Radix), Kushen (Sophorae Flavescentis Radix), Jingjie (Schizonepetae Spica), Lianqiao (Forsythiae Fructus), Qianghuo (Notopterygii Rhizoma et Radix), and Baizhi (Angelicae Dahuricae Radix). Externally, Mangxiao (Natrii Sulfas), Chixiaodou (Adzuki Bean), and Qingdai (Indigo Naturalis) are ground into powder, mixed with egg white (Jizi Qing) and pig bile (Zhudan Zhi), and applied to the sore for best effect. Do not disturb the scabs; wait for them to fall off naturally. If urinary difficulty with blood occurs after recovery, this is also due to residual toxic heat, known as Neiwai Chuang (Internal and External Sores), which also presents with scabs. If pus does not discharge, death ensues. One should use Huanglian Jiedu Tang (Coptis Decoction for Resolving Toxins) supplemented with Shengdi (Rehmanniae Radix), Guiwei (Angelicae Sinensis Radix), Lianqiao (Forsythiae Fructus), Mutong (Akebiae Caulis), Huashi (Talcum), Niuxi (Achyranthis Bidentatae Radix), Xianhecao (Herba Siegesbeckiae), Hupo (Amber), and Gancao Shao (tip of Glycyrrhizae Radix et Rhizoma).
In cases where the Yangming channel paradoxically lacks sweat, and the skin feels like insects crawling, it is because the Stomach governs muscles, indicating chronic deficiency of the Yangming channel. Shuzhu Fuzi Tang (Atractylodis Macrocephalae and Aconite Decoction) or Huangqi Jianzhong Tang (Astragalus Decoction for Building the Middle) should be used. If wind-heat is excessive, causing body itching, fever, absence of sweat, dry mouth and tongue, and constipation with difficult urination and defecation, Fangfeng Tongsheng San (Windproof Powder for Unblocking the Intestines) added with Qianghuo (Notopterygii Rhizoma et Radix) is appropriate. For body itching caused by wind patterns, Xiaoxuming Tang (Minor Continuation of Life Decoction) without Fuzi (Aconiti Lateralis Radix Praeparata) but added with Baifuzi (Acori Tatarinowii Rhizoma) should be used. For body itching due to blood deficiency, Siwu Tang (Four Substances Decoction) added with Fuping (Ulmus Micocarpus Cortex), Jili (Tribuli Fructus), and Fangfeng (Saposhnikoviae Radix) is suitable.
Swelling occurring on the nose and forehead, red and swollen, extending to severe swelling of both eyes preventing them from opening, with the entire forehead and face red and swollen, belongs to the Yangming channel. If accompanied by high fever, panting, dry mouth and tongue, painful and obstructed throat, and a rapid, large pulse, Puji Xiaodu Yin (Universal Benefit Decoction for Resolving Toxins) should be used. If internal excess heat is severe, Tongsheng Xiaodu Yin (Unblocking Benefit Decoction for Resolving Toxins) is indicated. If swelling occurs around the ears (above, below, front, back) and the temporal regions, red and swollen, this belongs to the Shaoyang channel. If accompanied by muscle fever, afternoon fever, alternating chills and fever, bitter taste in the mouth, dry throat, eye pain, and fullness under the ribs, Xiao Chaihu Tang (Minor Bupleurum Decoction) added with Huafen (Trichosanthis Radix), Qianghuo (Notopterygii Rhizoma et Radix), Jingjie (Schizonepetae Spica), Lianqiao (Forsythiae Fructus), Huangqin (Scutellariae Radix), and Huanglian (Coptidis Rhizoma) is appropriate. If swelling occurs on the head, including the back of the head, nape, and behind the eyes, red and swollen, this belongs to the Taiyang channel, and Jingjie Baidu San (Schizonepeta Antidote Powder) should be used. If all three yang channels are affected simultaneously, concentrating on the head, face, ears, eyes, and nose, Puji Xiaodu San (Universal Benefit Powder for Resolving Toxins) should be used, with Qingliang Jiuku San (Cooling and Saving from Suffering Powder) applied externally. The treatment method should proceed from mild to urgent measures so that pathogens become latent. The mild approach involves reducing fever and resolving toxins; for deficient individuals, supporting Yuan Qi (Primal Qi) is necessary. For those with weak stomach qi and poor appetite, strengthening stomach qi is required. Only when fecal impaction with severe internal heat occurs should Dahuang (Rhei Radix et Rhizoma) be used to purge and extract the root of the toxin. This is the method of proceeding from mild to urgent. Generally, this toxin first swells on the nose and forehead, then on the eyes, then on the ears, spreading from the ears to the head and connecting to the back of the brain. Swelling stops when it forms a mass; if it does not dissipate, it will suppurate. Folk wisdom calls this "Da Tou Tian Xing" (Great Head Epidemic), advising relatives not to visit, as infected persons often do not survive. During the Taihe era, many suffered from this disease. Physicians used Chengqi Tang (Chengqi Decoction) added with Lan Gen (Isatidis Radix) to purge, resulting in slight relief, but symptoms returned the next day. Repeated purging provided only temporary relief, and the condition eventually became critical. Li Dongyuan examined the patient and stated: The upper half of the body is governed by Heaven Qi, and the lower half by Earth Qi. This pathogenic heat lodges between the Heart and Lung, attacking upward to cause severe head swelling. Using Chengqi Tang to purge excess heat from the Stomach would be punishing the innocent. Therefore, he prescribed Puji Xiaodu Yin Zi (Universal Benefit Decoction for Resolving Toxins), saving many lives.
