Ziman (Excessive Fetal Fluid Retention) refers to the condition in pregnant women at five or six months of gestation, characterized by abnormal abdominal enlargement, distension and fullness of the chest and abdomen, edema of the hands, feet, face, and eyes, rebellious Qi causing mental restlessness, and difficulty in urination.
This is caused by water accumulation in the Baozhong (uterus), named Taishui (Fetal Water). If not treated from the upper body, the newborn may suffer from weak and short limbs with deformities, or the fetus may die in utero.
Qianjin Liyu Tang (Thousand Gold Carp Decoction) is used to treat this water retention.
If Spleen deficiency leads to impaired transportation and transformation, resulting in the failure to distinguish between clear and turbid Qi, it can be assisted with Sijunzi Tang (Four Gentlemen Decoction);
If there is flaccid edema of the face and eyes, and the limbs feel as if filled with water qi, Quansheng Baizhu San (Quansheng Atractylodes Macrocephala Powder) is used;
If there is Spleen dampness with deficient heat, causing swelling in the lower part, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) with added Fuling (Poria) is used;
If irregular diet causes vomiting and diarrhea, Liujunzi Tang (Six Gentlemen Decoction) is used;
If the legs and feet swell, accompanied by asthma, chest oppression, and restlessness, or fluid exuding from between the toes, Tianxianteng San (Akebia Vine Powder) is used;
If there is Qi stagnation in the Spleen and Lungs, Jiaweigui Pi Tang (Modified Gui Pi Decoction) is used, assisted with Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder).
However, severe generalized edema and distension are termed Ziman.
If only the dorsum of the foot swells, making walking difficult, or yellow fluid exudes from within the toe joints, it is called Zi Qi (Fetal Qi Edema). This condition persists until delivery.
Therefore, foot swelling during pregnancy, especially when extending to the calves and legs at eight or nine months, is not comparable to water qi disease and should not be treated as such, lest it damage the True Qi.
Those presenting with this condition will likely have an easier labor because there is abundant water and blood in the Baozang (uterus), preventing the fetus from drying out.
If swelling occurs immediately upon conception, it indicates excessive water qi while the fetus is not yet formed, leading to inevitable miscarriage.
Zizhong (Fetal Edema) is similar to Zi Qi, but Zi Qi occurs in the lower body, whereas Zizhong occurs on the head and face.
If Ziman appears after five or six months of gestation, it differs from Zi Qi and Zizhong. Generally, as the fetus grows large, the abdomen becomes full; severe fullness obstructs Qi, leading to generalized edema.
The *Jingui Yaolue* states: "When a woman is pregnant for six or seven months, if the pulse is wiry and she has fever, the fetal distension increases, accompanied by abdominal pain and aversion to cold, with the lower abdomen feeling as if fanned by wind, this is due to the opening of the Zizang (uterus). It should be treated with Fuzi Tang (Aconite Decoction) to warm the Zizang."
Xu Zhongke stated: "At six or seven months, the Stomach and Lung Qi nourish the fetus. If Qi is stagnant due to cold, the fetal distension increases; internal cold causes abdominal pain and aversion to cold. However, aversion to cold can be exterior-related; here, combined with abdominal pain, it indicates cold injuring the interior. The sensation of the lower abdomen being fanned, with periodic coldness resembling the action of a fan, is a distinct form of aversion to cold. Since it is isolated to the lower abdomen, it is because the Zizang has been injured by cold and cannot close, causing the lower abdomen to remain open and unconsolidated. Zizang refers to the uterus. Fuzi enters the Kidney to warm the Lower Jiao, hence Fuzi Tang is suitable to warm the meridians."
If a pregnant woman suffers accidental injury, causing restless fetus and unbearable pain, use Suasharen (Amomi Fructus Rotundus) with its shell, regardless of quantity, stir-fried with the peel until black, then ground into powder. Take two qian with hot wine. For those who do not drink alcohol, take with rice water. When warmth is felt in the abdomen, the fetus will naturally settle.
Abdominal and cardiac pain during pregnancy, whether pre-existing cold pain or newly contracted wind-cold, all arise from visceral deficiency.
Evil and righteous Qi clash and converge in the Qi mechanism, moving up and down with the Qi. Rising to attack the Heart causes heart pain; descending to attack the Abdomen causes abdominal pain.
If evil and righteous Qi continuously attack internally for a long time without resolution, the pain impacts the Bao Tuo (fetal membrane), inevitably disturbing the fetus.
Jiegu's Dihuang Danggui Tang (Rehmannia and Angelica Decoction) treats fetal pain in pregnant women.
Danxi attributed this to Blood deficiency, thus using Siwu Tang (Four Substances Decoction) removing Chuanxiong and doubling Shudi (Prepared Rehmannia Root). This is the core method.
Intermittent abdominal pain during pregnancy, or heaviness and bearing-down sensation in the lower abdomen, is termed Taotong (Fetal Pain).
Use three qian of Dihuang (Rehmannia Root) and one qian of Danggui (Angelica Sinensis), decocted in water as the primary treatment. If ineffective, add Renshen (Ginseng), Baizhu (Atractylodes Macrocephala Rhizome), and Chenpi (Tangerine Peel).
If the pain is caused by sinking due to Middle Qi deficiency, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction).
