Speaking. "Sheyin" (tongue aphonia) refers to the inability of the tongue root to move for speech, while the voice from the throat remains normal. In stroke-induced sheyin, both the tongue and the throat are diseased, preventing the voice from being emitted by the epiglottis. However, the etiology can be either external contraction or internal injury. External contractions involve pathogenic factors of wind-cold, wind-heat, or fire. The Classics state: "When wind-cold invades the epiglottis, it fails to open and close, resulting in loss of voice." It also states: "All cases of sudden aphonia due to heat belong to fire." Internal injuries are caused by dysfunction of the Heart, Lung, and Kidney meridians, often due to phlegm-fire obstructing the upper orifices, with deficiency of both Qi and Blood failing to ascend and nourish, leading to immobility of the tongue mechanism. Some cases arise from Kidney deficiency where Qi fails to return to its source, with internal depletion causing severance of the Bao-Luo (uterine/pericardium collateral), preventing the ascent of Clear Yang Qi. Others result from insufficiency of Yuan Qi (Original Qi), leaving the Lung without support; still others stem from blood deficiency, causing the Heart to lose nourishment. Generally, the Heart is the master of voice, the Lung is the door of voice, and the Kidney is the root of voice. The Classics say: "The Qi of the Sanjiao (Triple Burner) connects to the larynx; when Qi is weak, it cannot ascend." If the practitioner exerts effort on the root cause, the Clear Yang Qi of the Dantian will naturally be promoted and revitalized. Therefore, ancient physicians often achieved success using Dushen Tang (Single Ginseng Decoction) and Dihuang Yinzi (Rehmannia Infusion). However, there are six types of aphonia in stroke: those with loss of voice and inability to speak, those with stiff tongue and inability to speak, those with clouded spirit and inability to speak, and those...
