The syndrome of macules and rashes (Ban Zhen) is mostly caused by improper regulation and treatment during the fever of various diseases, leading to stagnant heat in the blood level that cannot be vented, boiling on the muscle surface to form spots. Therefore, if it is specifically due to fever from measles (Ma Zhen), it should naturally be treated with exterior-releasing methods. However, if one encounters fever without identifying the disease, and arbitrarily labels it as "measles," then recklessly uses purgative or pushing-down therapies, it results in excessive blood-heat stagnation, forcing blood to flow reversely and boil between muscles, resembling the image of water splashing over the forehead. This is truly not the original syndrome of gan-rash (Gan Zhen), but rather a consequence of harsh medicinal attacks, similar to macules appearing when cold damage (Shang Han) heat reaches its extreme. Although spots appear, the blood level has already been severely scorched and distressed. Furthermore, there are cases where spots finally appear due to blood stasis or blood depletion; doctors, eager to validate their previous assertions, use harsh dispersing medicines until they see results. Macules and rashes are originally transformed from blood-heat; without this pathology, they would not be formed by burning. The harm lies in the fact that more severe cases result in death due to medication errors rather than the macules and rashes themselves. Thus, for various fevers, if treated appropriately according to their causes, recovery is certain! Conversely, claiming that rash toxins are not yet cleared and mistakenly adding cooling and detoxifying herbs to treat a condition where excess heat-toxin drugs attack an illness with injured primary qi makes survival impossible. Zhang Jingyue, after treating patients with visible macules and rashes presenting with high fever, coma, panting, irritability, thirst, inability to eat, diarrhea, vomiting of roundworms, or head and face submerged in heat while the forehead burns and feet are cold, all used harsh tonification of true yin and true yang. After one or two doses, fever subsided, spirit strengthened, eating resumed, and bowels regulated. Another case record is detailed in Volume 20 of *Feng Shi Jin Nang Mi Lu* under "Case Treatments and Formulas"; I hope experts will review it carefully. Generally, when blood is forced by fire to manifest externally, the consumption of yin in the meridians can be imagined; when yang qi floats to the exterior, the decline of the "less fire" organ storing qi in the dantian can also be imagined. With both water and fire depleted, how can the spleen's yuan energy function? The severe injury of middle qi needs no further explanation. Without harshly tonifying true yin and true yang, how can one preserve essence, qi, and spirit to fulfill their life-sustaining functions? Wu Hegao said: "Using Ginseng Radix et Rhizoma (Renshen), Cinnamomi Cortex (Guipi), and Aconiti Lateralis Radix Praeparata (Fuzi) to treat macules and rashes is a variation in method." If physicians do not understand flexibility, how can they discuss this? However, Cinnamomi Cortex (Guipi) purely moves through the yang level, which is not entirely suitable. Without combining it with yin-nourishing herbs, how can latent heat be controlled, especially when rash toxins have severely damaged the yin level? The *Classic* states: "When yang is strong and cannot secure, yin qi perishes; when yin is peaceful and secret, and yang is firm, spirit thrives; when yin and yang separate and rupture, essence and qi perish." This is indeed the supreme truth for seeking survival in all diseases.
