The upper teeth belong to Kun Earth, connected by the meridians of the Foot-Yangming Stomach Channel. The lower teeth belong to Qian Metal, connected by the meridians of the Hand-Yangming Large Intestine Channel. In the initial stage of fever from sores and rashes (Chuang Zhen), if the patient opens their mouth and the anterior plate teeth are dry, it indicates intense heat in the interior; it is appropriate to use cooling agents to gently resolve it. If teeth grinding occurs during fever and lasts for a long time, it signifies Liver Heat; if the teeth appear to move upward, it signifies Heart Heat, which is a sign that convulsions (Chu) are about to occur. However, since the Kidney governs bones and teeth are the surplus of bones, chills with teeth grinding indicate that the pathogenic toxin has returned to the Kidney, all of which are ominous prognoses. One must carefully examine other symptoms to determine the auspicious or inauspicious outcome. As for gum ulceration when the scabs (Yie) fall off after the rash subsides, this indicates that the flesh sores have not yet healed. Failure to treat properly will lead to ulceration and bleeding of the tooth roots, blackening of the flesh, and foul odor, resulting in Zou Ma Gan (Gangrenous Stomatitis). Furthermore, dry teeth with black coating are an ominous sign. Tooth loss after the scabs fall off indicates the exhaustion of Kidney Qi and is incurable.
The tongue is the manifestation of the Heart. It is the sprout of the Heart, yet the Spleen meridian also connects to the tongue. Thus, its role in accepting food and drink and controlling voice is significant. In cases of fever from smallpox (Dou), a red and moist tongue is auspicious. A tongue dry as thorns indicates extreme internal heat, requiring urgent resolution. If lesions appear on the tongue, dense like piled millet and broken like a honeycomb, the condition is critical. If accompanied by choking when drinking water, hiccups when eating, hoarseness preventing speech, excessive rash eruption with tongue protrusion, or a black tongue due to heat disease and dry mouth, these are all untreatable. Additionally, continuous tongue protrusion indicates insufficient fluids in the Spleen system with slight heat, causing the tongue meridians to be slightly tight. Do not administer cold medicines to avoid further damaging Stomach Qi. Even if the patient craves water, do not mistakenly use purgatives, as this will exhaust fluids further.
A visible blue vein behind the ear indicates frequent future convulsions. Yellow color on both the front and back of the ears indicates fright entering the Kidney, manifesting as true teeth grinding. If blue-black veins appear at the side of the ear, crossing the hairline directly inward, do not mistake this for other diseases; know that it is caused by colic pain below the umbilicus. Ear heat and dark/black ear cartilage from smallpox indicate deep-seated pathogenic toxins due to Kidney heat. If blackness spreads from the Taiyang area around the ear, death (Huang Quan) is near; do not delay. Lack of brightness in the spirit of the eyes, greenish hue turning red in the black pupil, all indicate critical conditions; careful nursing may allow survival. Red and lively lips without dryness or whiteness predict an auspicious outcome for smallpox. Dry cracks on the lips with white tongue coating indicate accumulated heat in the Heart channel; do not be confused by other symptoms. Profuse diarrhea like water flowing through a bamboo tube, where medicine passes straight through, leads to certain death. Sudden dryness and blueness of the Philtrum (Ren Zhong) indicate undigested milk and difficulty in defecation. Blueness above the Yintang point indicates fetal fright, leading to night crying and cold sensitivity. Black qi on the nose is a cause of death; nasal blackness also indicates urinary retention. Dry nostrils with coarse breathing indicate impending epistaxis. Redness on the tip of the nose extending up to the Yintang indicates night crying, appearance of measles, or generation of carbuncles due to Heart heat and toxicity.
