The throat is the vital passage of the whole body and the pathway for food and drink. If heat-toxin accumulates between the chest and diaphragm, generating wind-phlegm that obstructs and fails to disperse, it manifests as diseases of the throat, or internally generates sores resembling rotting flesh, causing blockage where neither vomiting nor swallowing is possible. In cases of Dan Rou'e (Single Tonsillar Hypertrophy) or Shuang Rou'e (Double Tonsillar Hypertrophy), and swelling of the cheeks, severe cases involve swelling both inside and out, attacking upwards to the head and face. Treatment should first induce vomiting to expel wind-phlegm, thereby unblocking the throat and diaphragm, followed by resolving heat-toxin and clearing the Lung and Stomach. Delay will render treatment ineffective. For Dan Rou'e or Shuang Rou'e, acupuncture should be used if applicable; if not suitable, use blowing or dropping agents (Jie Yao) to expel wind-phlegm for rapid effect, followed by decoctions, as acute conditions cannot wait. For cheek swelling, severe cases require pricking with porcelain edges to release bad blood; mild cases may be treated with topical applications or drops, followed by decoctions to disperse wind, clear heat, resolve toxicity, and eliminate phlegm, leading to spontaneous recovery.
Houbi (Throat Bi) is similar to Chanhou Feng (Coiling Throat Wind). Its manifestations include red face and coarse breathing, with swollen and closed throat, caused by accumulated heat generating wind and accumulating toxic phlegm. Severe cases involve an internal fleshy tumor blocking the airway completely. If the nose and face turn blue-black, accompanied by chills, dysphagia, drooping head, and rattling phlegm sounds like sawing, the condition is incurable. Furthermore, Zang Cold (Visceral Cold) can also cause throat closure and difficulty in swallowing or speaking, due to extreme cold below causing excessive heat above, not limited to this condition alone. Its symptoms are similar to the previously discussed Rou'e conditions, but the treatment methods differ. Generally, invisible swelling and closure are termed Bi, while visible swelling and pain are termed Rou'e. Expelling wind-phlegm first addresses the branch in acute situations; resolving heat-toxin later addresses the root in chronic situations. For conditions with upper heat and lower cold, hot medicines should be taken cold before meals; cool medicines must not be mistakenly administered.
