Zhu, male, over fifty years old, from Linghu. A smoker with constitutive deficiency of the Spleen and Lung, latent phlegm-fluid retention in the Middle Jiao (the area of the spleen and stomach). Compounded by Liver Qi disturbance, phlegm obstructs Lung Qi, resulting in coughing with counterflow Qi, thick sputum, tidal fever, abdominal distension, and a lingering course. The pulse is slippery on the right, and fine-rapid-wiry on the left. Treatment should focus on regulation.
Prescription: Dongyang Ginseng (Codonopsis Pilosula), Bran-fried Bitter Orange Peel (Fructus Aurantii Preparatus), Red Poria (Poria Cocos), Ge Banxia (Pinellia Ternata, 5 fen), Raw Atractylodes Macrocephala Rhizome (Atractylodis Macrocephalae Rhizoma), True Purple Agarwood (Aquilariae Lignum Resinatum, 3 fen, sliced and decocted separately, taken with the main decoction), Xinhui Tangerine Peel (Citri Reticulatae Pericarpium), Inula Flower (Inulae Flos), Stir-fried White Tribulus Fruit (Tribuli Fructus), Rose Flower (Rosae Rugosae Flos), Purple Quartz (Quartzum Rubrum), Dong White Peony Root (Paeoniae Radix Alba), Ginger-juice stir-fried Bamboo Shavings (Bambusae Caulis in Taenia).
Liu, from Taihe Fang, seen in January. *Chao Shi Bing Yuan* states: Hypochondriac pain on the left belongs to blood stasis; on the right, to phlegm-fluid. Symptoms include pain radiating from the right hypochondrium, counterflow Qi, and thick sputum. This clearly indicates phlegm obstructing the Qi collaterals, failing to allow free diffusion. The pulse is wiry on the right, and small-wiry-rapid on the left. Treatment should aim to drain Wood and harmonize the Center.
Prescription: Inula Flower (Inulae Flos), Whole Trichosanthes Fruit (Trichosanthis Fructus), Song Banxia (Pinellia Ternata), Red Poria (Poria Cocos), Xinjiang (Safflower-infused silk), Sichuan Curcuma Tuber (Curcumae Longae Rhizoma), Stir-fried White Tribulus Fruit (Tribuli Fructus), Rose Flower (Rosae Rugosae Flos), Green Onion Tube (Allii Fistulosi Caulis), Xinhui Tangerine Network (Citri Reticulatae Pericarpium Retinervium), Loquat Stem Network (Erythrinae Ramulus), Ginger-juice gauze Bamboo Shavings (Bambusae Caulis in Taenia).
Certain patient: Suffering from hanging fluid (Xuan Yin) located between the ribs, producing gurgling sounds upon palpation. Sanzi Yangqin Decoction (Three Seeds Decoction for Nourishing the Stomach) is appropriate.
Shen, female, twenty-five years old, seen in February. Contracted Wind-Warmth pathogen, disturbing the Yangming channel. Symptoms include head distension and pain, body heat, epigastric stuffiness, expectoration of phlegm, and bleeding from the nose. This is what *Shanghan Lun* refers to as "red sweat" leading to resolution. The pulse is wiry-rapid and floating. Treatment should employ clearing and resolving methods.
Prescription: Scrophularia Root (Scrophulariae Radix), Black Gardenia Fruit (Gardeniae Fructus), Moutan Bark (Moutan Cortex), Fresh Bamboo Shavings (Bambusae Caulis in Taenia), Honeysuckle Dew (Lonicerae Japonicae Inflorescens), Mint Tip (Menthae Haplocalycis Herba), Tangerine Peel (Citri Reticulatae Pericarpium), Fritillaria Bulb (Fritillariae Thunbergii Bulbus), Smooth Passiflora Stem (Passiflorae Caulis), Forsythia Fruit (Forsythiae Fructus), Sichuan Curcuma Tuber (Curcumae Longae Rhizoma), Huai Dioscorea Tuber (Dioscoreae Hypoglaucae Rhizoma), Imperata Rhizome (Imperatae Rhizoma).
For nasal bleeding due to summer-heat wind, this formula is suitable. It is advisable to crush and extract juices from lotus leaf, Rehmannia Root, and imperata rhizome to mix with it.
Shen, seen in February. *Neijing* states that in spring, bi-nasal obstruction and epistaxis are common, largely due to excessive Qi-fire, external invasion of wind-heat, wind and fire fanning each other, and injury to Yang collaterals. Treatment should follow the method of Heishen Xijiao Tang (Black Ginseng and Rhinoceros Horn Decoction).
Prescription: Scrophularia Root (Scrophulariae Radix), Dong White Peony Root (Paeoniae Radix Alba), Charred Gardenia Fruit (Gardeniae Fructus), Charred Schizonepeta Spike (Schizonepetae Spica), Rhinoceros Horn Plate (Rhinocerotis Cornu), Moutan Bark (Moutan Cortex), Forsythia Fruit (Forsythiae Fructus), Imperata Rhizome (Imperatae Rhizoma), Rehmannia Root (Rehmanniae Radix), Huai Dioscorea Tuber (Dioscoreae Hypoglaucae Rhizoma), Clean Honeysuckle Flower (Lonicerae Japonicae Flos), Mint Stem (Menthae Haplocalycis Herba), Fresh Bamboo Leaf (Bambusae Folium).
