Heat-Dampness Injury, Red and White Dysentery: Wang Sun'an's treatise on dysentery specifically emphasizes heat-dampness as the etiology. The symptoms include tenesmus (urgent desire to defecate with a feeling of incomplete evacuation), abdominal pain, the urge to defecate but inability to do so, and the discharge of purulent, bloody, and turbid matter, which may be white, red, or a mixture of both.
Excess Heat, Red Dysentery Staining: The stomach is the sea of qi and blood. Heat is a yang pathogen. When heat exceeds dampness, it injures the blood aspect of the stomach, resulting in red dysentery.
Excess Dampness, White Dysentery Dropping: Dampness is a yin pathogen. When dampness exceeds heat, it injures the qi aspect of the stomach, resulting in white dysentery. If red and white are mixed equally, it indicates injury to both qi and blood.
Guidelines for Regulating and Moving, Must Be Remembered: Regulating the blood leads to the natural healing of pus and blood; regulating the qi eliminates tenesmus. These four sentences are maxims for treating initial dysentery and must be firmly remembered.
Shaoyao Decoction, Taken in Excess Heat: Shaoyao Decoction regulates qi and moves blood. Although it is a general formula for initial dysentery, it is ultimately suitable for heat syndromes.
Pingwei San Added, Test for Cold-Dampness: For initial cases of diarrhea and dysentery caused by cold-dampness, Pingwei San (a formula to dry dampness, strengthen the spleen, move qi, and harmonize the stomach) is used, added with Ganjiang (Dried Ginger), Zexie (Alisma), Zhuling (Polyporus), and Muxiang (Aucklandia). If it persists without cure, Xianglian Wan is administered.
Unremitting Fever, Incurable: Medical texts state: If a patient with dysentery has fever that does not subside, it is incurable.
Formulas for Dysentery Are All Taboo: Whenever dysentery begins with fever, if it is not due to exterior pathogens or disharmony of the meridians, one should use warm dispersing methods to harmonize Ying and Wei (nutritive and defensive qi). Once the exterior pathogen is resolved, the dysentery will also ease. If one generally assumes it is heat and immediately uses standard formulas for dysentery, the condition often deteriorates severely.
Guizhi and Gegen, Expel Pathogens: Contemporary physicians have a taboo against sweating, fearing it when there are no exterior signs. However, if headache, fever, and aversion to cold are present, sweating with sweat suggests using the method of Guizhi Tang (Cinnamon Twig Decoction); without sweat, the method of Gegen Tang (Kudzu Root Decoction) is appropriate. This drives pathogens out from the exterior before treating the dysentery.
Exterior Unblocked, Interior Smooth: These two sentences explain the reason for inducing sweating. Zhang Feichou stated: Danggui Sini Tang (Angelica Sinensis Decoction for Replenishing Blood and Unblocking Collaterals) is extremely effective for treating dysentery. If there is fever accompanied by vomiting, Xiaochaihu Tang (Minor Bupleurum Decoction) or Gegen Huanglian Huangqin Gancao Tang (Kudzu Root, Coptis, Scutellaria, and Licorice Decoction) should be used. If there is thirst and severe tenesmus, Baitouweng Tang (Pulsatilla Decoction) works miraculously.
Jiayan's Writings, Unique Secrets: Yu Jiayan's discussions in *Yimen Falü* (Medical Laws and Standards) are particularly thorough.
Meaning Preserved, Supplementing Jingui: In Yu Jiayan's *Yuyi Cao*, cases such as those involving Mahuang Fuzi Xixin Tang (Ephedra, Aconite, and Asarum Decoction) and Renshen Baidu San (Ginseng and Toxin-Expelling Powder) supplement what was omitted in *Jingui Yaolue* (Essential Prescriptions from the Golden Cabinet).
