Wandering pain throughout the body is named *Tongfeng* (Gout). It is caused by wind-dampness attacking due to blood deficiency and qi stagnation. Internal generation of damp-heat produces wind, which fails to control Earth (Spleen/Stomach), leading to phlegm obstructing the collaterals and difficult circulation. Wind pathogen affecting the extremities manifests as worsening during the day and easing at night, indicating issues with Qi and Blood circulation. Treatment primarily benefits from *Siwu Tang* (Four Substances Decoction) added with *Taoren* (Peach Kernel) and *Honghua* (Safflower) to nourish blood and invigorate blood stasis; for phlegm-heat, *Erchen Tang* (Two-Cured Decoction) added with *Gualou* (Trichosanthes Fruit) is similar. For upper wind, use *Qianghuo* (Notopterygium Rhizome), *Fangfeng* (Saposhnikovia Root), *Baizhi* (Angelica Dahurica), *Bohe* (Mint), and *Guizhi* (Cinnamon Twig); for lower dampness, use *Yiyiren* (Coix Seed) and *Haifengteng* (Lardizabala Vine) for unblocking, referencing Han Yong's method. For astringent urination, use *Siling San* (Four-Poria Powder) with stir-fried *Sangzhi* (Mulberry Twig) added to the decoction. Although this appears as blood stasis with tendons lacking attachment, the root cause is total blood deficiency failing to nourish internally. Cold qi congealing and stagnating, combined with wet-phlegm accumulation, moves with wind causing severe and ominous pain.
Chen Can stated: Among the Five Elements and Six Qi, none circulate faster than Wind and Fire. Pain that eases slightly upon deep pressure indicates blood stasis in the collaterals.
Blood Collateral Stasis Obstruction (Long-standing pain inevitably enters the collaterals; when Qi and Blood do not flow, Bi syndrome arises.)
*Jinfecao* (Boilweed Herb), *Taoren* (Peach Kernel), Raw Deer Antler, *Xinjiang* (Fresh Crimson Dye), *Guiwei* (Tail of Angelica Sinensis), Green Onion Tube.
Accumulated Injury Entering Collaterals Causing Pain
*Guixu* (Tip of Angelica Sinensis), *Xiangxiangmo* (Powder of Costus Root), *Xiaohuixiang* (Fennel), *Muxiang* (Aucklandia Root), *Baiziren* (Biota Seed), *Yeyujin* (Wild Turmeric).
Yin-Level Latent Heat Type Tongfeng
Numbness and stabbing pain from the vertex to the feet are treated with Dongyuan's *Zishen Wan* (Kidney-Nourishing Pill).
Liver and Kidney Deficiency Lower Jiao Pain
After illness, if essence and spirit have not recovered, excessive speech damages Qi, and walking strains the sinews. One should employ sweet, warm, and mild nourishment.
*Renshen* (Ginseng Root), *Dangguishen* (Body of Angelica Sinensis), *Baifushen* (Poria with Hostwood), *Gouqi* (Goji Berry), *Shayuanzi* (Astragalus Complanatus Seed), *Ganjuetan* (Charred Chrysanthemum).
Chen Can stated: Ministerial Fire resides in the Liver, Dragon-Lei Fire arises from the Kidney, both emerging from the Yin level. The root is initially deficient, and storage functions fail.
He Shutian stated: The Classics say, "All pain, itching, and sores belong to the Heart." Since the Heart governs Sovereign Fire, it naturally falls under heat patterns. However, this refers only to sores and cannot be generalized to other diseases. Ancient practitioners established the principle of "unblocking" for all pains, not meaning purgative or draining methods, but rather unblocking Qi and Blood so pain ceases. One must distinguish whether the pathology lies in the Qi level or Blood level. In the Qi level, one only moves Qi without disturbing Blood. In the Blood level, one treats both Qi and Blood, as moving Qi causes Blood to follow. In excess patterns, Qi stagnates and Blood coagulates; one unblocks Qi to dissipate Blood. In deficiency patterns, Qi is weak and cannot transport, Blood is deficient and cannot nourish; one must tonify Qi and Blood, incorporating unblocking within tonification.
Chen Can stated: Various pains are suitable for pungent, moistening, and unblocking methods, and unsuitable for sour, cold, astringent, and涩 (astringent) methods, lest evil pathogens remain trapped.
