If the pubic symphysis (Gu Jiao) fails to separate, and it is due to skeletal locking (Suo Gu), let the midwife apply talcum powder (Hua Shi) mixed with sesame oil into the vaginal opening (Chan Men), or slowly dilate it with two fingers. Concurrently administer Jiawei Guixiong Tang (Modified Angelica and Ligusticum Decoction). Once the medicinal effect takes hold, delivery will proceed smoothly and safely. If the vaginal opening remains open after childbirth, it indicates a deficiency of Qi and Blood; treat with Bazhen Tang (Eight-Treasure Decoction) to tonify. If ineffective, use Shiquan Dabu Tang (Ten-Ingredient Great Tonifying Decoction).
Dang Gui (Angelicae Sinensis Radix, 5 qian), Zi Bai Gui Ban (Testudinis Carapax et Plastrum from naturally deceased turtle, roasted in child's urine until brittle, 3 qian), Chuan Xiong (Ligustici Chuanxiong Rhizoma, 3 qian), Fu Ren Tou Fa (Human Hair, one handful, burnt to charcoal retaining its nature). Decoct in water and take orally. Within the time it takes a person to walk five li, the fetus will be delivered; even if it is a stillbirth, it will be expelled. Calcined Gui Ban may also be used.

Failure of the placenta to descend may be due to exhaustion of Qi and strength, preventing effective pushing. It is advisable to secure the umbilical cord during cutting, and then administer one dose of Guixiong Tang (Angelica and Ligusticum Decoction), after which the placenta will descend. Alternatively, if blood enters the placental sac causing it to swell and preventing descent, leading to abdominal distension, pain, and panting, immediately administer Shixiao Wan (Pain-Stopping Pill) three qian with clear wine. This will disperse the blood stasis and reduce swelling, allowing the placenta to fall naturally. If ineffective, supplement with Huarui Shi San (Huarui Stone Powder) or Niuxi San (Achyranthes Powder).
Guixiong Tang is mentioned previously.
This formula treats blood stasis causing fullness in the uterus and "Er Zhen Tong" (Postpartum Abdominal Pain resembling a pillow), with miraculous efficacy.
Wu Ling Zhi (Trogopteri Excrement, cleaned of dirt and stir-fried) and Pu Huang (Typhae Pollen, stir-fried), in equal parts. Grind into fine powder, make pills with vinegar paste, sized like paulownia seeds. Take 2–3 qian per dose, washed down with light vinegar water.
This treats residual foul blood postpartum, leading to blood confusion and fainting (Xue Mi Xue Yun), failure of the placenta to descend, severe abdominal distension, unconsciousness, provided the heart region remains warm. Immediately administer this decoction by force-feeding. The blood stasis will transform into fluid and exit, and the patient will regain consciousness. The efficacy is divine; this prescription is indispensable for physicians.
Hua Rui Shi (Ochra, 1 jin), Shang Se Liu Huang (High-quality Sulfur, 4 liang). Grind both into powder and mix evenly. Seal a clay jar with paper mud, place the two medicines inside, seal again with paper mud, and dry in the sun. Calcine using charcoal for the duration of two incense sticks. On the next day, remove and grind finely. Take 1 qian per dose, mixed with child's urine and hot wine. For severe cases, take 2–3 qian.
