Leukorrhea is often classified in medical texts into five colors—green, yellow, red, white, and black—each associated with the Zang organs, with specific formulas prescribed. However, one should not be rigidly bound by this classification. Generally, this condition stems from Spleen deficiency accompanied by dampness. When Spleen Qi is robust, the essence derived from food and drink transforms into Qi and Blood rather than leukorrhea; when Spleen Qi is deficient, the refined essence of food transforms into leukorrhea instead of generating Qi and Blood. In southern regions where the terrain is low and damp, people generally have weaker constitutions; thus, nine out of ten suffer from turbid leukorrhea. I treat this with Wuwei Yigong San (Five-Ingredient Powder for Extraordinary Efficacy), adding herbs such as Faba Seed (Viciae Fabae Semen), Coix Seed (Coicis Semen), and Chinese Yam (Dioscoreae Rhizoma), which yields prompt results. If the discharge exhibits mixed colors, simply add one or two herbs corresponding to the affected Zang organ. Although leukorrhea seems trivial, if it progresses to metrorrhagia that does not stop, it can lead to exhaustion of marrow and emaciation of bones, resulting in consumptive disease. How can one neglect its treatment!
Wuwei Yigong San (see Consumption chapter) strengthens the Spleen and stops turbid leukorrhea. If the discharge is purely white, it belongs to the Lung; increase the dosage of Coix Seed. If mixed with red, it belongs to the Heart; add Salvia Root (Salviae Miltiorrhizae Radix) and Angelica Sinensis Root (Angelicae Sinensis Radix). If mixed with green, it belongs to the Liver; add Bupleurum Root (Bupleuri Radix) and Gardenia Fruit (Gardeniae Fructus). If mixed with yellow, it belongs to the Spleen; add Dendrobium Stem (Dendrobii Caulis), Lotus Leaf (Nelumbinis Folium), and aged rice. If mixed with black, it belongs to the Kidney; add Eucommia Bark (Eucommiae Cortex) and Teasel Root (Dipsaci Radix). If the pulse is rapid with heat signs, add stir-fried Phellodendron Bark (Phellodendri Cortex) and Lotus Plumule (Nelumbinis Plumula). If the pulse is slow with cold extremities, add processed Aconite Root (Aconiti Lateralis Radix Praeparata).
Conceiving offspring is a common occurrence, yet infertility can be extremely difficult to resolve. This is usually due to improper regulation during sexual intercourse. For men, the primary goal is preserving Jing (Essence); for women, it is regulating menstruation. The way to preserve Jing is through reducing desires: avoiding excessive sexual activity, refraining from overindulgence in alcohol, and minimizing mental exertion ensures sufficient Jing and Qi. If there is congenital insufficiency, medicinal tonification is required. Generally, a weak pulse at the left Chi position or a rapid pulse with heat indicates deficiency of True Yin (True Water); treat with Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) combined with Wuzi Yanzong Wan (Five-Seed Progeny-Deriving Pill) to nourish the Water of Tianyi. A weak pulse at the right Chi position or a slow pulse with cold extremities indicates deficiency of True Yang (True Fire); treat with Bawei Dihuang Wan (Eight-Ingredient Rehmannia Pill) combined with Wuzi Yanzong Wan to nourish the Fire of Erdi. If both Chi pulses are weak, or if Middle Qi is deficient, indicating dual deficiency of Water and Fire along with Qi and Blood depletion, use Shibu Wan (Ten-Ingredient Pill) combined with Wuzi Yanzong Wan for strong tonification. If semen is thin and uncoagulated, add Fish Swim Bladder Gelatin (Ichthyoswetonis Colla) and Deer Antler Glue (Cervi Cornus Colla). If ejaculation lacks force, decoct approximately one jin (500g) of Astragalus Root (Astragali Radix) into a paste to make pills, thereby benefiting Qi. This constitutes the treatment method for men.
The principle of regulating menstruation lies first in cultivating temperament. The Book of Songs states: "If a woman is harmonious and peaceful, she will joyfully conceive." Harmony ensures Qi and Blood do not conflict; peace ensures Yin and Yang do not contend. Medical texts state: "With a harmonious constitution, menstruation occurs once every thirty days, which is normal." If menstruation is delayed, medication should regulate it. Generally, early onset with a rapid pulse and heat indicates Blood Heat; treat with Yimu Shengjin Dan (Motherwort Golden Pill for Victory) plus Rehmannia Root (Rehmanniae Radix) and Moutan Bark (Moutan Cortex). Late onset with a slow pulse and cold extremities indicates Blood Cold; treat with Yimu Shengjin Dan plus Cinnamon Bark (Cinnamomi Cortex). Abdominal pain preceding menstruation indicates Qi Stagnation; add Qi-regulating herbs. Poor appetite, Qi deficiency, pale complexion, and limb weakness indicate dual deficiency of Qi and Blood; modify the previous formula by halving the amount of Cyperus Rhizome (Cyperī Rhizoma) and adding Ginseng Root (Ginseng Radix), Astragalus Root (Astragali Radix), Placenta Hominis (Placentae Hominis), Poria (Poria), Jujube Seed (Ziziphi Spinosae Semen), and Polygala Root (Polygalae Radix). This enriches Qi and Blood, allowing the meridians to regulate naturally. Just as prolonged drought prevents river flow, and blocked rivers cannot nourish all things, how can a woman with amenorrhea or scanty menstruation conceive? This outlines the treatment methods for women. Thus, preserving Jing is the responsibility of men; regulating menstruation is the key for women. Each has specific pathologies requiring careful identification; one must not act carelessly and lose the principle of generation. Those seeking offspring, take heed!
