Pi Man (Distension and Fullness) refers to the stagnation and obstruction of Qi in the chest. Although Pi Man syndromes are mostly excess patterns, there are also cases caused by deficiency. When Pi Man is accompanied by pathogenic factors and stagnation, it is an excess pattern; when it occurs without pathogenic factors or stagnation, it is a deficiency pattern. When fullness is accompanied by distension and pain, it is an excess pattern; when fullness occurs without distension or pain, it is a deficiency pattern. If Pi Man belongs to an excess pattern, Erchen Tang (Two-Chen Decoction) should be used as the primary formula. If Pi Man belongs to a deficiency pattern, Lizhong Tang (Regulate the Middle Decoction) should be used. If Pi Man presents as half-excess and half-deficiency, complicated by mixed cold and heat, Banxia Xiexin Tang (Pinellia Heart-Draining Decoction) should be used.
Shang Shi (Food Injury) necessarily presents with symptoms such as chest tightness, belching with a putrid odor, and abdominal distension, indicating food retention and failure to digest. Pingwei San (Level the Stomach Powder) should be used as the primary formula. If food is retained in the upper epigastrium and constitutes an excess pattern suitable for emesis, Gua Di San (Melon Seed Powder) should be used. If food is retained in the lower epigastrium and constitutes an excess pattern suitable for purgation, Da Chengqi Tang (Major Order the Qi Decoction) should be used.
Shang Jiu (Alcohol Injury) manifests as vomiting and regurgitation, irritability, chest fullness, loss of appetite, and difficulty in urination. Ge Hua Jie Cheng Tang (Kudzu Flower Sobering Decoction) should be used as the primary formula. If the condition is suitable for resolution through bladder Qi transformation, Wuling San (Five-Ingredient Powder with Poria) modified should be used. In cases of severe drunkenness where intestinal rotting is feared, the soaking method from Qianjin Fang (Essential Prescriptions Worth a Thousand Gold) can be applied.
Modified Wuling San: Atractylodis Macrocephalae Rhizoma (2 qian), Poria (3 qian), Polyporus (3 qian), Alismatis Rhizoma (3 qian), Phellodendri Cortex (1.5 qian), Coptidis Rhizoma (1 qian), Puerariae Flos (3 qian). Decoct in water until approximately half a cup remains, and take warm.
Qianjin Fang Soaking Method: Prepare a decoction, store it in a large vessel, and soak the affected area. Replace with fresh solution when cooled; this will naturally dissipate the alcohol toxicity and is effective. Alternatively, drink juice extracted from Imperata Rhizome, or drink juice extracted from fresh Pueraria root, or drink rice gruel.
