Treatise
The *Huangdi Neijing* (Yellow Emperor's Inner Canon) states: "All cases of twisting, convulsions, and turbidity of body fluids belong to Heat." The syndrome of turbid urine arises from damp-heat in the Spleen and Stomach flowing downward and infiltrating the Bladder, causing urination to be either white or red and turbid. The *Yuanbing Shi* (Book on the Original Causes of Diseases) states: "Just as in summer when the weather is very hot, water becomes turbid and trees exude sap." When Blood deficiency is accompanied by severe Heat, it results in red turbidity; this is because the Heart and Small Intestine govern diseases related to Fire. When Qi deficiency is accompanied by mild Heat, it results in white turbidity; this is because the Lung and Large Intestine govern diseases related to Metal. Zhu Danxi stated: "Generally, it is mostly due to the flow and accumulation of Damp-Phlegm, so one should dry the Dampness in the Middle Jiao (Spleen/Stomach)." He further stated: "Treatment should focus on drying Dampness and descending Fire, combined with lifting methods." These are all crucial statements. Additionally, there are symptoms of seminal emission and sliding discharge that resemble turbidity but cannot be treated uniformly. Seminal emission often occurs with dreams or hallucinations of sexual intercourse, termed "Nocturnal Emission" (Meng Yi). Or it may occur during urination, termed "Sliding Essence" (Jing Hua). Some contract this disease due to endless thoughts and unfulfilled desires; treatment should calm the Spirit and descend Heart Fire. Others contract it due to excessive lust and sexual fatigue leading to Kidney deficiency; treatment should nourish Kidney Water to restore True Yin. These are immutable principles for all time; students should study them carefully.
Pulse Diagnosis
If both Chi pulses are surging and rapid, there will definitely be turbid urine and loss of essence. If a woman's Chi pulses are choppy and weak, or surging and rapid with interruptions, these indicate turbid urine and leukorrhea. If the Heart pulse is short and small, caused by Heart deficiency, there will inevitably be nocturnal emission and turbid urine.
Methods
Zhu Danxi said: Turbid urine belongs to Warmth-Heat, involving Phlegm and Deficiency. Red turbidity relates to Blood, caused by the Small Intestine's association with Fire. White turbidity relates to Qi, caused by the Large Intestine's association with Metal. Generally, it is all due to the flow and accumulation of Damp-Phlegm, suitable for drying the Dampness of the Middle Jiao. Red turbidity is Dampness injuring Blood, where turbid Qi in the Stomach flows downward and infiltrates the Bladder.
Obese individuals with pale complexion tend to have Phlegm; treatment should dry Dampness and descend Fire, combined with lifting methods. The general principle is to use Er Chen Tang (Two-Cured Decoction) added with Cang Zhu (Atractylodis Rhizoma), Bai Zhu (Atractylodis Macrocephalae Rhizoma), Sheng Ma (Cimicifugae Rhizoma), and Chai Hu (Bupleuri Radix). For red turbidity, add Bai Shao Yao (Paeoniae Radix Alba), decoct and take.
One patient suffered from turbid urine for half a year, occasionally experiencing nocturnal emission, with emaciation. Treated as Heart deficiency, taking Zhenzhu Fen Wan (Pearl Powder Pill) combined with Dingzhi Wan (Stabilize Spirit Pill) was effective. (Formula see under Palpitations section.)
Nocturnal emission primarily involves Heat; sliding essence primarily involves Damp-Heat, because Heat causes circulation.
Injury from internal causes, with Qi and Blood deficiency failing to secure and hold, requires tonification. Use Ba Wu Tang (Eight Substance Decoction) modified, taken with Chu Mu Gen Wan (Toona Root Pill).
General principles use Qing Dai (Indigo Naturalis), Hai Shi (Lapillus), and Huang Bo (Phellodendri Cortex).
For sliding essence, use Zhi Mu (Anemarrhenae Rhizoma) and Huang Bo to descend Fire; Mu Li (Ostreae Concha) and Ge Fen (Concha Meretricis et Cyclinae) to dry Dampness. White turbidity is treated similarly.
Prescription 1: Use Liang Jiang (Alpiniae Officinarum Rhizoma) 3 qian, Shao Yao (Paeoniae Radix) and Huang Bo (Phellodendri Cortex, scorched) each 2 qian, Chu Gen Bai Pi (Toonae Cortex) 1 liang 5 qian. Grind into powder, make pills with paste, take 30 pills per dose.
Disease caused by excessive thinking originates in the Heart; treatment should calm the Spirit and supplement. One method uses Wendan Tang (Warm Gallbladder Decoction) removing Zhu Ru (Bambusae Caulis in Taenia), adding Ren Shen (Ginseng Radix et Rhizoma), Yuan Zhi (Polygalae Radix), Lian Rou (Nelumbinis Semen), Suan Zao Ren (Ziziphi Spinosae Semen), and Fu Shen (Poria cum Risus) to decoct and take.
Prescription 2: Pills for treating turbid urine.
Chu Bai Pi (Toonae Cortex), Huang Bo (Phellodendri Cortex, fried), Qing Dai (Indigo Naturalis), Gan Jiang (Zingiberis Rhizoma, fried), Hua Shi (Talcum), Ge Fen (Concha Meretricis et Cyclinae, fried, equal parts).
Make pills with Shen Qu (Massa Medicata Fermentata) paste and take.
Dai Shi said: Huang Bo treats Warmth-Heat; Qing Dai clears Heat; Ge Fen is salty and cold, entering the Kidney; Hua Shi opens orifices; fried Gan Jiang is bitter, astringes Lung Qi and causes it to descend, generating Yin Blood, and acts as a controller.
Er Chen Tang treats turbidity; adding lifting drugs makes stool moist and urine long.
Zhenzhu Fen Wan: Treats sliding essence and white turbidity.
Huang Bo (Phellodendri Cortex), Zhen Ge Fen (Concha Meretricis et Cyclinae, each 1 jin), Zhen Zhu (Margarita, 3 liang; another version has no Pearl but adds Qing Dai).
Grind into fine powder, mix with water to make pills the size of Sophora seeds. Take 100 pills per dose, down with warm wine on an empty stomach. Can also add Chu Gen Bai Pi, Hua Shi, Qing Dai, etc.
Ban Ling Wan: Treats white turbidity.
Shen Qu (Massa Medicata Fermentata), Ban Xia (Pinelliae Rhizoma, dries dampness), Zhu Ling (Polyporus, separates fluids, equal parts).
Make pills with Qu paste and take.
For Xu Lao (Consumptive Disease), use Yin-tonifying medicines. For weak Stomach, add Ren Shen, along with Sheng Ma and Chai Hu to lift clear Qi from the Stomach.
Zhang Ziyuan had dual deficiency of Qi and Blood, Phlegm, turbid urine, and Wind-Pain from Yin Fire.
Ren Shen (Ginseng Radix et Rhizoma, 1 liang), Bai Zhu (Atractylodis Macrocephalae Rhizoma, 5 qian), Shu Di Huang (Rehmanniae Radix Preparata), Huang Bo (Phellodendri Cortex, scorched black, each 2 liang), Shan Yao (Dioscoreae Rhizoma), Hai Shi (Lapillus), Suo Yang (Cynomorii Herba, each 5 qian), Gan Jiang (Zingiberis Rhizoma, 5 qian, burnt to ash retaining nature), Nan Xing (Arisaematis Rhizoma, 1 liang, roasted until cracked), Gui Ban (Carapax et Plastrum Testudinis, prepared with butter, 2 liang).
Grind into fine powder, make pills with wine and wheat paste, size of Sophora seeds. Take 100 pills per dose, down with Ginger-Salt Soup.
For drying Damp-Phlegm and treating white turbidity, if the Liver pulse is wiry, must use Qing Dai to drain Liver. Generally, do not use purely cooling medicines. (Lu Shi said: The cause is Damp-Heat, so the medicine is correct, but one must not exclusively use cold-cooling medicines. Thus, fried Huang Bo is used, assisted by the warmth of Gan Jiang.)
(Above are sixteen methods from Danxi.)
Miao Xiang San (from Ju Fang): Treats Heart deficiency causing nocturnal emission and white turbidity.
She Xiang (Moschus, 1 qian, separately ground), Ren Shen (Ginseng Radix et Rhizoma, 5 fen), Mu Xiang (Aucklandiae Radix, 2 qian 5 fen, roasted), Fu Ling (Poria), Fu Shen (Poria with Hostwood), Huang Qi (Astragali Radix), Yuan Zhi (Polygalae Radix, heart removed and fried, each 1 liang), Jie Geng (Platycodonis Radix), Gan Cao (Glycyrrhizae Radix et Rhizoma, each 5 qian), Chen Sha (Cinnabaris, 2 qian, separately ground), Shan Yao (Dioscoreae Rhizoma, 2 liang, fried with Ginger Juice).
Grind into fine powder. Take 2 qian per dose, mixed with warm wine, at any time.
Mi Zhen Wan (from He Jian): Treats endless thoughts, unfulfilled desires, external lust, excessive coitus leading to flaccidity of the sinews (Jin Wei), Bai Yin (white discharge), white substances discharging with urine, or dreaming of sexual intercourse with women.
Bai Long Gu (Fossilia Ossis Mastodi, 1 liang, separately ground), He Zi Pi (Terminalia Chebulae Pericarpium, 5 large fruits), Suo Sha Ren (Amomi Fructus, 5 qian, shell removed), Zhu Sha (Cinnabaris, 1 liang, separately ground, coat 1 fen with it).
Grind into fine powder, make pills with wheat paste, size of mung beans. Take 1 pill per dose, down with warm wine on an empty stomach; cold water is also acceptable. Do not take too much to avoid constipation. Alternatively, take down with Scallion-White Tea Soup.
Bi Xie Fen Qing Yin (Yang Family Heritage): Treats insufficiency of True Yuan, Cold-Dampness in Lower Jiao, white turbid urine, frequent and unlimited urination, urine surface like oil, unstable luster, sediment at bottom condensing like paste.

Grind into powder, make pills with wine and wheat paste, size of Sophora seeds. Take 50 pills per dose, down with wine or Salt Soup. For sliding essence and turbid urine, after 2-3 doses, urine becomes clear as water, appetite doubles, and walking becomes light and vigorous. For women with difficult labor, 2-3 doses stop pregnancy. If childbirth is still desired, take promoting and purgative medicines.
Shui Lu Er Xian Dan (Recorded Verified Prescription): Treats nocturnal emission, white turbidity, dream emission, essence depletion.

Grind into fine powder, make pills with wheat paste, coat with Qing Dai, size of Sophora seeds. Take 50 pills per dose, down with Ginger-Salt Soup on an empty stomach.
(Danxi Flexible Methods) says: Red and white turbidity is caused by Phlegm accumulation in the Stomach flowing downward and infiltrating the Bladder. Use Er Chen Tang added with Sheng Ma, Chai Hu, Fang Feng, etc., to lift. Obese pale people belong to Damp-Heat; add Cang Zhu, Bai Zhu, fried Huang Bo, Huang Jing Zi, etc. If accompanied by Cold, add fried Gan Jiang, Rou Gui to the base formula; severe cases add Fu Zi. For Heart deficiency unable to secure, or those with chronic Cold-Deficiency, add Bi Xie, Shi Chang Pu, Yi Zhi, fried Gan Jiang, Mu Li, Long Gu, etc. For Qi deficiency, add Huang Qi, Bai Zhu, Ren Shen to the base formula, or add Fu Zi. Red turbidity is mostly contracted by Blood-deficient and emaciated people; use Si Wu Tang added with Wine-Zhi Mu, Wine-fried Huang Bo, decoct to send down Zhenzhu Fen Wan. For unbearable lower abdominal pain from red/white turbidity, treat as Cold; Li Dongyuan's Jiu Zhu Dang Gui Wan is best. (Formula see Women's Section.)

Medical Case
Zhu Xianzhong from Liantang suffered from nocturnal emission and tidal fever, unable to get out of bed for three months. I was summoned to treat him. Pulse diagnosis showed both left and right Cun and Guan pulses floating, deficient, and powerless; both Chi pulses were surging, large, and soft. Prescribed Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) added with Shu Di Huang, Zhi Mu, Huang Bo, and Di Gu Pi to decoct, taken with swallowed Zhenzhu Fen Wan. Externally made a small bamboo cage to cover the penis, preventing skin contact. After more than 30 doses, he was safe and sound for one month.
