[Zhang Zhongjing] Asked: There are three diseases in women who have just given birth. One is Jing disease (a condition characterized by stiffness of the neck and back, and opisthotonos), one is Yumao (a condition characterized by dizziness, visual disturbances, and mental depression), and one is difficulty in defecation. What does this mean? The Master said: After childbirth, blood deficiency occurs, accompanied by excessive sweating, making the body susceptible to wind pathogens, thus causing Jing disease. Loss of blood followed by sweating leads to an excess of cold, thus causing Yumao. Depletion of body fluids results in dryness of the stomach, thus causing difficulty in defecation. (Yumao in postpartum women is what is currently known as Xueyun or blood syncope.)
[Da Ren Quan Sheng Fang] Postpartum Xueyun (blood syncope) occurs due to a sudden deficiency of Qi and Blood. When the body has not yet stabilized, blood follows the ascending Qi, confusing the Shen (Spirit), causing visual hallucinations such as seeing flowers before the eyes. In extreme cases, it causes severe suffocation, unconsciousness, clenched jaws, coma, and cold breath. Administering Qinghun San (Clear Soul Powder) will restore consciousness.
Zelan Ye (Lycopus Herba), Renshen (Ginseng Radix et Rhizoma) (each 2.5 qian), Jingjie (Schizonepetae Spica) (1 liang), Chuanxiong (Ligustici Chuanxiong Rhizoma) (0.5 liang), Gancao (Glycyrrhizae Radix et Rhizoma) (1 liang, absent in the original formula).
Grind the above into fine powder. Mix with half a cup of warm wine and half a cup of hot water, take 1 qian, and administer urgently. Upon swallowing, the eyes open, breathing stabilizes, and consciousness returns.
[Bao Ming Ji] To treat postpartum wind-deficiency Xueyun (blood syncope) with mental confusion, use Jingjie San (Schizonepeta Powder).
Jingjie (Schizonepetae Spica) (1 liang 3 qian), Taoren (Persicae Semen) (5 qian, stir-fried).
Grind into fine powder. Take 3 qian with warm water. If there is slight panting, add Xingren (Armeniacae Semen Amarum) and stir-fried Gancao (Glycyrrhizae Radix et Rhizoma), each 3 qian.
[Yan Yi] To treat postpartum Xueyun (blood syncope). Use Jingjie Sui (Schizonepetae Spica) ground into powder, mixed with Tongbian (Child's Urine) for oral administration of 2-3 qian. This is extremely effective and verified. Symptoms of postpartum Xueyun include rigid straightening of the body, upward deviation of the eyes (Daiyan), upward traction and spasm of the mouth corners and outer canthi, and unconsciousness. Take one chicken egg, remove the shell, take the egg white, mix with 2 qian of Jingjie powder, and administer. Then treat according to subsequent conditions. If there are no other illnesses, no further treatment is needed; it acts very swiftly.
Note: The above methods using Jingjie should not be used for patients with Qi deficiency.
[Da Ren Quan Sheng Fang] For fainting due to heavy bleeding, there is only drowsiness, suffocation, and restlessness; tonify Blood. For fainting due to light bleeding, it is caused by retained lochia (Elu) failing to discharge, rushing up to the Heart, causing fullness and urgency in the epigastrium, coma, clenched jaws, and complete unconsciousness; break stasis and promote Blood circulation.
[Zhu Danxi] Postpartum Xueyun (blood syncope) in women is caused by deficient fire carrying Blood upwards gradually. Burn Lu Jiao (Cervi Cornu) to ash, eliminate fire toxicity, grind extremely fine, mix with good wine and Tongbian (Child's Urine), and administer. One sip restores consciousness. This substance is extremely effective in moving Blood.
[Yun Qizi] Duxing San (Single Herb Powder) treats postpartum Xueyun (blood syncope), coma, unconsciousness, and suffocation due to Blood rushing to the Heart.
Wulingzhi (Trogopteri Dung) (half raw, half stir-fried), 2 qian each, grind into fine powder. Mix with warm wine and take 2 qian. For clenched jaws, pry open the mouth and administer; it cures upon entering the throat. Another method: add Jingjie (Schizonepetae Spica), divide equally into powder, mix with Tongbian (Child's Urine) and take.
[Shi Yi De Xiao Fang] To treat death-like state due to placenta expulsion or postpartum Blood rushing to the Heart. Burn Yu Jin (Curcumae Radix) until carbonized (preserving its nature), grind into 2 qian of powder. Mix with 1 he of strong vinegar and administer; life is immediately restored.
[Jian Yi Fang Lun] Postpartum Xueyun (blood syncope) with chest oppression and cessation of Qi. Honghua (Carthami Flos) 1 liang, grind into powder. Divide into two doses. Decoct with 2 zhong of wine to obtain 1 zhong. Take together. If jaws are clenched, pry open and administer; effect is rapid. Another method: use 3 liang of fresh Honghua (Carthami Flos), Wu Hui Jiu (Rice Wine without Ash) and Tongbian (Child's Urine) each 0.5 sheng. Boil to obtain 1 zhen and take.
[Zhou Hou Bei Ji Fang] To treat Xueyun (blood syncope). Su Mu (Sappan Lignum) 3 liang, finely chopped. Decoct in 5 sheng of water to obtain 2 sheng. Take in two doses; cure. If Su Mu is unavailable, boil Fei Yi (Red Cloth/Garment) and drink the juice; this also works.
[Sheng Ji Zong Lu] Postpartum Xueyun (blood syncope) with unconsciousness and delirious speech. Qilin Jie (Dragon's Blood Resin) 1 liang, grind into fine powder. At any time, mix with warm wine and take 2 qian bi.
[Bao Ming Ji] Honghua San (Safflower Powder) treats postpartum blood dimness, Xueyun (blood syncope), metrorrhagia, irregular menstruation, and old dried Blood stagnation.
Gan He Ye (Dried Nelumbinis Folium), Mudan Pi (Moutan Cortex), Chuan Gui (Angelicae Sinensis Radix), Honghua (Carthami Flos), Puhuang (Typhae Pollen) (stir-fried, equal parts).
Grind into fine powder. Take 0.5 liang each time, decoct with wine, and take warm including the dregs. If the placenta fails to descend, mix with Yu Bai Pi (Ulmus Radix Cortex) decoction, 0.5 liang; immediate effect.
This is a mild formula for breaking Blood stasis.
[Anonymous] Mudan San (Peony Powder) treats postpartum Xueyun (blood syncope) with severe suffocation. For clenched jaws, pry open and administer.
Mudan Pi (Moutan Cortex), Da Huang (Rhei Radix et Rhizoma) (baked), Mangxiao (Natrii Sulfas) (each 1 liang), Donggua Zi (Benincasae Semen) (0.5 he), Taoren (Persicae Semen) (30 grains, skin and tip removed).
Chop coarsely. Take 5 qian each dose, decoct in 3 zhong of water until 1.5 zhong remains. Strain, add Mangxiao (Natrii Sulfas), decoct again. Divide into two doses.
[Chan Shu] To treat postpartum vexation, heat in hands and feet, exhaustion of strength, Xueyun (blood syncope) connecting to the Heart, hardness of the head, and uncontrollable chills and fever. Break Jiejiaomu (Araliae Chinensis Caulis) into pieces about the size of a handful of counting rods. Decoct in 1 sheng of water to obtain 0.5 sheng. Take warm in two doses. Or if urination is frequent and lochia does not stop, taking this will cure. Decoction of this wood three times retains its potency; this is a life-saving formula.
This is a heavy formula, suitable for cases where no blood drops out.
[Da Ren Quan Sheng Fang] Huarishi San (Orpiment Powder) treats postpartum Qi nearly exhausted, or death caused by malignant Blood rushing to the Heart. Mix 1 qian with Tongbian (Child's Urine); expelling malignant matter is extremely effective. (Formula see Trauma section.)
Heishen San (Black Divine Powder) treats postpartum Xueyun (blood syncope); best taken mixed with fine wine. (Formula see General Postpartum section.)
The above two formulas are for cases with much Cold.
Xiong Gui Jia Shaoyao Tang (Cyathula and Angelica Decoction plus Peony) treats fainting due to excessive blood loss postpartum. (Formula see Postpartum Hemorrhage section.)
[Bao Ming Ji] Another Formula to treat critical postpartum Xueyun (blood syncope).
Sheng Di Zhi (Rehmanniae Radix Juice) (1 large zhen), Dang Gui (Angelicae Sinensis Radix) (2.5 qian, stir-fried), Chi Shao Yao (Paeoniae Radix Rubra) (2.5 qian, sliced).
Decoct in water for 3-5 boils. Take warm. If feeling vexatious heat, remove Dang Gui (Angelicae Sinensis Radix), add 0.5 zhen of Tongbian (Child's Urine) and take.
[Mei Biao] To treat postpartum residual Blood attacking the Heart, or continuous bleeding, chest oppression, blue face, cold body, and near-death Qi. Drink 1 zhen of fresh Sheep Blood (Ovis Aries Blood), three times daily; excellent.
[Da Ren Quan Sheng Fang] Sudden postpartum fainting, sweating, and inability to recognize people is due to sudden deficiency. Break 3 chicken eggs and swallow them; consciousness returns. If not awake, give 1 sheng of Tongbian (Child's Urine); very effective. If postpartum blood loss was heavy and this ailment increases, and eggs do not restore consciousness, urgently prepare Zhu Li (Bamboo Sap). Take 5 he per dose. If still unsettled shortly after, give another 5 he. Administer frequently for 3-5 doses; cure.
[Zhu Danxi] A woman in her thirties, pale-faced and tall, often harbored unresolved grievances. She gave birth suddenly at midnight. Early next morning, she fainted and lost consciousness. Acupuncture moxa was urgently applied to Qihai (CV6) for 15 moxa cones, and she revived. Later, she took herbs like Renshen (Ginseng Radix et Rhizoma) and Baizhu (Atractylodis Macrocephalae Rhizoma), and stabilized after two months.
[Zhang Zhongjing] A postpartum patient with Yumao (dizziness/coma) has a weak pulse, cannot eat, stool is hard, but only the head sweats. The reason is Blood deficiency leading to Jue (reversal/cold extremities), which causes Yumao. For Yumao to resolve, there must be profuse whole-body sweating. Because Blood deficiency causes lower Jue, solitary Yang floats upward, hence only head sweating. Therefore, postpartum women tend to sweat because Yin Blood is depleted and Yang Qi is dominant; sweating allows Yin and Yang to restore balance. With hard stool, vomiting, and inability to eat, Xiaochaihu Tang (Minor Bupleurum Decoction) is the primary treatment. (Yumao is synonymous with syncope.)
[Da Ren Quan Sheng Fang] To treat postpartum Xueyun (blood syncope) with complete unconsciousness, extremely critical. Cut Chive (Allium tuberosum) into segments, place in a bottle with a spout. Pour hot Vinegar (Acetum) over it, seal the bottle mouth tightly. Hold the spout towards the patient's nostrils so the vinegar vapor enters. First, help the patient sit up; she will wake. Another method: if feeling dizzy, apply vinegar to the face. Once awake, sip vinegar slowly. Apply vinegar to mouth and nose, and keep vinegar nearby for constant inhalation. Another method for treating Xueyun (blood syncope) collapse. Quickly burn Ganqi (Dry Resin of Toxicodendron) and inhale the smoke; wake immediately. If Ganqi is unavailable, old lacquerware works too. Another method for postpartum Xueyun (blood syncope): blow a small amount of raw Banxia (Pinelliae Rhizoma) powder into the nose.
[Ben Cao Gang Mu] To treat Blood rushing to the Heart after delivery. Slice 5 pieces of Shengjiang (Zingiberis Rhizoma Recens). Decoct in 8 sheng of water to obtain 3 sheng. Take in three doses.
[Zhu Danxi] Vinegar-Ink prevents intrapartum Xueyun (blood syncope). Take half a stick of good Inkstone Ink. Burn red-hot, plunge into vinegar. Repeat three times. Eliminate fire toxicity, grind fine. Take 5 fen each time, mixed with dilute vinegar soup.
[Zhai Xuan Fang] Postpartum Xueyun (blood syncope), unconsciousness: Zusanli (ST36), Zhigou (SJ6), Sanyinjiao (SP6). (Xin Shu does not list this acupoint.)
[Biao You Fu] Another method: Yinjiao (RN7), Yangbie (extra point).
[Shi Yi De Xiao Fang] Another method: Shenmen (HT7), Neiguan (PC6). (If no response, take posterior points:) Guanyuan (CV4) (moxibustion).
