Apoplexy refers to the sudden loss of consciousness, where the body appears completely like a corpse, yet breath remains and the pulse beats as usual, or the pulse is chaotic, fluctuating between large and small, or weak and fine but continuous, accompanied by warmth in the chest and heart region.
[Zhang Zhongjing] Corpse Jue (Shi Jue) is characterized by a beating pulse but absence of breath, with Qi blockage preventing circulation, resulting in a still appearance resembling death. One may insert powder of Rhizoma Acori Tatarinowii (Changpu, 石菖蒲) into the nostrils and blow it, while placing powder of Ramulus Cinnamomi (Guizhi, 桂枝) under the tongue. Another method involves cutting one square cun of hair from the left corner of the forehead, burning it to ash, mixing with wine, and administering orally for immediate revival. (This is the method from *Huangdi Neijing*. See later.)
[*Suwen*] When pathogenic factors invade the collaterals of the Hand-Shaoyin, Foot-Shaoyin, Hand-Taiyin, Foot-Taiyin, and Foot-Yangming meridians, these five collaterals converge in the ears and ascend to connect with the left frontal region. If the Qi and Blood of these five collaterals are exhausted, the vessels of the body may beat while the form remains insensible, appearing like a corpse, also called Corpse Jue. Acupuncture at the medial side of the big toe of the foot, one grain of rice width away from the nail edge, followed by the center of the sole, then the middle toe of the foot on the nail edge, then the medial side of the thumb of the hand, one grain of rice width away from the nail edge, and finally the tip of the radial styloid process of the Hand-Jueyin (Pericardium) and Hand-Shaoyin (Heart) meridians, will cure it immediately. If not cured, blow air into both ears using a bamboo tube, burn one square cun of hair from the left frontal region, drink a cup of fine wine, or force-feed if unable to drink, leading to immediate recovery. (Full text in *Miao Ci Lun*, pronounced "Yi".)
[*Yuji Zhenzang Lun*] Dizziness and fainting due to Qi: Duoming point (located above Quxue, insert needle 3 fen, tonify first, sedate after Qi returns, do not leave hand, moxibustion contraindicated; if no revival, take Jizhong point). Umbilicus (moxa 7 zhuang, acupuncture contraindicated. These two points can revive the dead.)
[*Zhenjiu Zhai Xuan*] Corpse Jue: Yuquan point (depth 8 fen, tonify 10呼, sedate upon Deqi). Yinbai point (depth 3 fen, tonify first then sedate; tonification generates Qi, sedation completes the action; frequently apply heat compress to both hypochondriac regions). Ghost Strike: Zhigou point (depth 2 fen, tonify first then sedate; tonification generates Qi, sedation completes the action). Sanli point (tonify 10呼, sedate 5呼. For these two points, if Qi does not arrive after needling, continue regardless of number; if Qi arrives, stop and do not needle again). Evil Invasion: Renzhong point, Zhongwan point, Qihai point.
[Zhang Zhongjing] If the Shaoyin pulse beats incessantly, it indicates weak Kidney Qi and scant Essence-Blood. The rushing Qi forces upward into the chest and diaphragm, causing Zong Qi to gather inversely, Blood to knot below the heart, Yang Qi to retreat downward, and Heat to return to the inner thighs, interacting with Yin, causing numbness and insensibility. This is Corpse Jue. One should needle Qimen point and Juque point. For symptoms of trance, Corpse Jue, irritability, and pain, Zhongji point and Pucan point are indicated.
[*Jia Yi Jing*] Corpse Jue, where the person is dead and unaware, but the pulse beats as usual, is treated by Yinbai point and Dadun point. Sudden death due to Corpse Jue is treated by Jinmen point.
[Sun Simiao] Emperor Renzong of Song favored his Imperial Concubine most. One day while eating, she suddenly collapsed, her whole body becoming cold instantly. He urgently reported to the Emperor, who summoned Sun Simiao and Du. Upon arrival, they reported: "No danger, this is merely Qi Jue. After a short while, vomiting will lead to revival." After some time, she indeed vomited and revived. The Emperor asked the cause. They replied: "The Concubine was eating when忧怨 Qi (Qi of sorrow and resentment) surged upward, stagnating with the food. Vomiting clears the Qi, hence revival." The Emperor asked the Concubine what caused this. She answered: "Your Majesty has no heir, and my family cannot produce an imperial heir for you, so I resent myself. My Qi suddenly reversed, startling Your Majesty." The Emperor sighed: "I blame myself too, causing you illness through labor." He praised Sun and Du's medical skill, saying long after: "Is this not the way of medicine? Are they not good physicians?"
The Emperor once suffered sudden Jue and lost consciousness, recovering without treatment. The Classic states: The Yellow Emperor asked, Jue sometimes causes abdominal fullness, sometimes sudden unconsciousness, lasting half a day to a day before recovery. Why? Qibo answered: Excess Yin Qi above leads to deficiency below; deficiency below causes abdominal fullness. Excess Yang Qi above causes lower Qi to rise repeatedly and pathogenic Qi to reverse; reversal leads to chaotic Yang Qi, and chaotic Yang Qi leads to unconsciousness. It also states: When Blood and Qi rush upward together, it is Great Jue. Jue leads to sudden death; if Qi returns, life is preserved; if not, death occurs.
[Zhu Danxi] Once treated a case where Yin was deficient first and Yang suddenly severed. Brother Zheng of Pujiang, nearly sixty years old, a man of leisure. In mid-summer, suffering from chronic dysentery, he engaged in sexual activity. Suddenly one night while using the toilet, his hands became flaccid, eyes open but devoid of spirit, urine incontinence, sweating profusely, rattling throat sounds, very faint breathing. His pulse was large and chaotic, without distinct position, extremely alarming. I happened to be there, ordered Ginseng Paste to be boiled, and first moxa Qihai point with moxa cones the size of a little finger, up to 80 zhuang, until the right hand moved. Three more zhuang, lips moved slightly. Ginseng paste was ready, gave one bowl. By midnight, after three bowls, eyes could move. After two jin, speech returned, requesting porridge. After five jin, dysentery stopped. After dozens of jin, full recovery.
[Zhang Zhongjing] Formula for saving sudden death with limbs unretractable and incontinence.
One sheng of horse dung, three dou of water, boil to two dou, use for washing. Also take one sheng of cow dung (thin feces), mix with warm wine, pour into mouth. Moxa one cun below the heart, three cun above umbilicus, four cun below umbilicus, one zhuang each, until cured.
Formula for saving sudden death with open mouth and opisthotonos.
Moxa fourteen zhuang behind the nails of hands and feet. Drink Wudu various ointment powders. (Formula unverified.)
Formula for saving sudden death in children with vomiting and diarrhea, unknown disease.
One pill of dog feces, squeeze juice and administer. If dry, boil in water and take juice.
For saving sudden death and Ke Wu death, Huanhun Tang (Return Soul Decoction) is indicated.
Ephedrae Herba (Mahuang, 麻黄) (remove nodes, 3 liang), Armeniacae Semen Amarum (Xingren, 杏仁) (remove skin and tips, 70 seeds), Glycyrrhizae Radix et Rhizoma Preparata (Zhigancao, 炙甘草) (1 qian). *Qianjin Fang* adds Cinnamomi Ramulus (Guizhi, 桂枝) (2 liang).
These three ingredients, boil in eight sheng of water to obtain three sheng, remove dregs, let patient swallow in divided doses. Treats all kinds of Ke Wu. *Qianjin Fang* says: Treats sudden Ke Wu, Ghost Strike, Flying Corpse, sudden Qi absence and loss of consciousness, or pulselessness, locked jaw unable to open. Remove teeth before administering decoction. If decoction cannot go down, part hair left and right, pull shoulders to extend neck, add one sheng more dose, revive shortly.
[*Ju Fang*] Suhexiang Wan (Styrax Pill) treats sudden stroke from Evil Ke Wu Zhu.
Suhexiang You (mixed into Anshixiang paste), Boswelliae Resina (Xunluxiang, 薰陆香) (separately ground), Borax (Longnao, 龙脑) (ground, 1 liang each), Aquilariae Lignum Resinatum (Qingmuxiang, 青木香), Atractylodis Macrocephalae Rhizoma (Baizhu, 白术), Santali Albi Lignum (Baitanxiang, 白檀香), Eugeniae Flos (Dingxiang, 丁香), Cinnabaris (Zhusha, 朱砂) (ground, water levigated), Aquilariae Lignum (Chenxiang, 沉香), Cyperi Rhizoma (Xiangfu, 香附) (stir-fried, remove hairs), Cornus Fructus (Xijiao, 犀角) (sawdust), Piperis Longi Fructus (Bibo, 荜茇), Anshixiang (Anshixiang, 安息香) (separately powdered, mixed with 1 sheng of non-gray wine, boiled to paste), Moschus (Shexiang, 麝香) (ground), Terminalia Chebulae Fructus (Helile, 诃梨勒) (roasted, use peel, 2 liang each).
Grind to fine powder, mix with ground drugs evenly, use Anshixiang paste and refined white honey to make pills, size of Sophora japonica seeds. Take four pills dissolved in fresh well water, hot or cold, on an empty stomach in the morning. Elderly and children take one pill, dissolved in warm wine. Wrap one pill in wax paper, size of a bullet, carry in a sewn bag. Chen Wuyue said: For diseases arising internally with unknown names, taking this is effective. Most effective for Qi Jue and Qi disharmony, vomiting, diarrhea, Guan Ge syndrome, with miraculous effects.
[*Zhou Hou Bei Ji Fang*] Bian Que: Zhong E (Evil Invasion), sudden death, and Ghost Strike are similar in treatment, refer to each other. For those already dead, crush fresh Rhizoma Acori Tatarinowii (Changpu, 石菖蒲), squeeze juice and administer, curing immediately.
[Zhang Zhongjing] To save sudden death with closed eyes, crush Allii Tuberosi Root (Jiu, 韭根) juice and pour into ears, blow Podis Fructus (Zaojiao, 皂角) powder into nose, effective.
[Chen Wuce] To treat Nightmare Sudden Death and various sudden collapse syndromes, use Pinelliae Rhizoma (Banxia, 半夏) unlimited amount, soak in boiling water seven times, powder. Use a small amount, blow into nose. If heart area is warm, treatable. In emergencies without formula, urgently moxa Renzhong point and both big toes one leek-leaf width from nail, 3-5 zhuang each, revive.
[He Jian/Zihehe] A patient had Phlegm Jue, unconscious, tight jaw, medicines could not enter, waiting for death. Dai Ren saw him, asked attendants: Was there saliva in the mouth? Answered: Yes. Dai Ren first decocted Saposhnikoviae Radix (Fangfeng, 防风) and Veratrum Nigrum (Lilu, 藜芦), mixed with Thladianthae Fructus (Goudi, 瓜蒂) powder to administer. Could not go down mouth, so took long clam shell, removed sharp edge, wrapped tip in paper, inserted into right nostril,
then swallowed with sound, repeated in left nostril. Dai Ren said: Can be treated. After a while, no saliva came out, so used Realgar (Pishi, 砒石) 1 qian, put into nose again. Suddenly lay supine, seemed to feel pain, shortly vomited foul matter and sticky saliva several sheng, instantly revived. Arsenic is usually avoided, but because the disease was severe, only this could move it. However, without Goudi, it cannot be used; judge carefully.
Generally, Apoplexy, Corpse Jue, Yu Mao (Depression and Vertigo), Stroke, Summerheat, etc., should all be treated by releasing the exterior. Zhang Zhongjing said: For Yu Mao to resolve, there must be heavy sweating.
Huanhun Tang uses Ephedrae Herba (Mahuang, 麻黄) and Cinnamomi Ramulus (Guizhi, 桂枝); Qinghun Tang uses Schizonepetae Herba (Jingjie, 荆芥); and for various apoplexies, use Podis Fructus (Zaojiao, 皂角) and Pinelliae Rhizoma (Banxia, 半夏) to induce sneezing by insufflating nose, use Veratrum Nigrum (Lilu, 藜芦) and Arsenic (Pishuang, 砒霜) to break teeth and extract phlegm. All these open the Upper Jiao to allow exterior pathogens to circulate. Summerheat contraindicates cold water closing the exterior, for the same reason.
[*Bencao*] Jiedu Xionghuang Wan (Detoxifying Realgar Pill) treats sudden collapse, unconsciousness. (Formula see Sore Throat.)
[Zhang Zhongjing] Sanwu Beiji Wan (Three-Ingredient Emergency Pill)
Rhei Radix et Rhizoma (Dahuang, 大黄), Zingiberis Rhizoma (Ganjiang, 干姜), Croci Fructus (Badou, 巴豆) (each 1 liang, remove skin, tip, heart, roast and grind to fat-like consistency).
Ingredients must be fresh. First grind Dahuang and Ganjiang to powder, mix in Badou, pound thousand times, mix with honey to make pills, store in vessel. Treats sudden acute diseases of Heart and Abdomen. If Zhong E/Ke Wu, abdominal distension, sudden pain like needle刺, urgent breathing, locked jaw, Stopping Corpse sudden death, take 3-4 pills the size of soybean with hot bitter wine. If not going down, lift head and pour down, should cure shortly. If not cured, give three more pills, abdomen will rumble, vomiting and purging will cure.
If locked jaw, must break teeth to administer.
[Chen Wuce] For summerheat fainting, urgently move to cool shade, absolutely do not give cold water. Use cloth or clothes dipped in hot water to compress Umbilicus and Qihai, continuously pour hot water over cloth to keep navel and abdomen warm, gradually wake. If no hot water available, scoop hot road soil onto navel, more is better, change when cold.
Ancient method: No hot water on road, scoop hot soil on navel, make a depression, have someone urinate into it to replace hot water. Then give Baihu Tang (White Tiger Decoction) or Zhuye Shigao Tang (Bamboo Leaf and Gypsum Decoction). Whenever feeling summerheat, urgently chew a large piece of fresh ginger, swallow with cold water. If confused and fainting, chew a large clove of garlic, swallow with cold water. If cannot chew, grind with water and pour to wake. On road without water, thirsty, urgently chew 2 cun raw scallion, swallow with saliva, equivalent to drinking two sheng of water. (Summerheat disease is Mao Yu (dizziness and depression) when Heat arrives, as per *Neijing*.)

[Zhang Zhongjing] For Yu Mao to resolve, there must be heavy sweating.
[*Bencao*] Person normally healthy, suddenly like dead person, body motionless, silently unaware, eyes closed unable to open, mute unable to speak, or slightly aware, dislikes hearing voices, only feels dizzy, wakes after some time. This is due to excessive sweating, scant Blood, Qi merging with Blood, Yang alone ascending not descending, Qi blocking and not circulating, thus body like dead. When Qi passes and Blood returns, Yin and Yang reconnect, thus waking after some time. Named Yu Mao, also named Blood Jue, common in women. Should use Baiwei Tang (Anemarrhenae Decoction) or Cang Gong San (Cang Gong Powder).

[Li Dongyuan] Discussion on harm of mistakenly using Cold-Cooling medicines for women with spontaneous abortion. Women during delivery or spontaneous abortion with leakage, fainting and unconsciousness, eyes closed without perception, is due to sudden Blood loss. After有形 Blood (tangible Blood) leaves, Heart Spirit lacks nourishment. Heart and Pericardium are Ministerial Fire and Sovereign Fire; nourished by Blood are safe, losing Blood is dangerous. Fire blazing upward causes fainting. Fire attacking Lung, eyes closed unconscious, is sudden Yin Blood loss unable to calm. Thus, when Blood is deficient, often using Talcum (Huashi, 滑石), Glycyrrhizae Radix et Rhizoma (Gancao, 甘草), Gypsum Fibrosum (Shigao, 石膏) etc., which are pungent-sweet great cold medicines, clearing Heat in Qi, is draining Qi due to Blood deficiency, draining Yang due to Yin deficiency, injuring both, making them insufficient. Deficiency consumption disease, coma and unconsciousness, is Heat in Upper Jiao Heart and Lung. This formless Heat, using cold-cooling medicines to drive downward. Does one not know Upper Jiao diseases belong to Exterior, are Yin syndromes, sweating cures them? Now contrary purging, luckily avoiding death, violently depletes Qi and Blood, how can life last long? Also does not know *Neijing* says: Disease Qi insufficient, should tonify not purge. Eyes closed diseases all belong to Yin, should sweat not purge. Also does not know Cold Damage Yu Mao resolves with sweat, thus cold-cooling medicines are contraindicated. Delivery and spontaneous abortion, original Qi not diseased, is sudden loss of tangible Blood. Losing Blood, tonify Blood, what doubt? Tonifying Blood makes Spirit prosperous. Normally Blood descends and leaks, now should tonify and raise. Heart receiving Blood nourishes Spirit, not fainting. Blood suddenly descending is Autumn-Winter command vigorous, now raise and ascend to assist Yang, eyes open, Spirit not fainting. Now establish a formula, tonify Blood, nourish Blood, generate Blood, benefit Yang. To supplement deficiency of Hand-Jueyin and Foot-Jueyin, Quan Sheng Huoxue Tang (Life-Preserving Blood-Activating Decoction) is indicated.
Quan Sheng Huoxue Tang Since all Yang have sunk, how to know? Due to Blood loss downward.
