For the treatment of pediatric throat obstruction (Hou Bi), one should refer to and apply the methods used for adult throat obstruction.
[Tang] There is no other method for treating throat obstruction; emesis (inducing vomiting) is indispensable. If this condition arises with extreme danger and previous methods fail, urgently use Bixia Dan (Emerald Pill) from the Official Bureau, grind it into fine powder, mix with natural juice of Bohe (Menthae Haplocalycis Herba) and vinegar, dip a chicken feather in the medicine, insert it into the throat, and slowly guide it. The best outcome is the expulsion of phlegm and saliva. The *Ben Shi Fang* argues that Bixia Dan harms people; this is incorrect, as it likely encountered patients destined to die at that time.
Bixia Dan
Shi Lv (Malachite), Fuzi Jian (Tip of Aconiti Lateralis Radix Praeparata), Wutou Jian (Tip of Aconiti Radix), Xie Shao (Tail of Scorpio).
Grind the above into powder and make pills. Dissolve with Menthae Haplocalycis Herba juice to induce expectoration of phlegm.
[Ming] Hoarse sounds in the throat, difficulty swallowing milk: Xuanti (Ren 22). Acute throat obstruction: Tiantu (Ren 22) (Moxibustion of one moxa cone is not suitable; caution against moxibustion here).
[Qian] Liver heat, manifested by grasping the collar and randomly twisting objects, treated primarily with Xie Qing Wan (Drain the Green Pill). High fever, drinking water, wheezing and chest oppression, treated primarily with Xie Bai San (Drain the White Powder). ("High fever" refers to continuous fever without cessation.)
Lung heat, manifested by pinching eyebrows and eyes, worsening heat in the evening, cough with chills and fever, high fever with thirst, treated primarily with Gan Jie Tang (Licorice and Platycodon Decoction). For lung deficiency-heat, with deep red lips, mild cases use Xie Bai San, severe cases use Liang Ge San (Cool the Diaphragm Powder).
Heart heat, observed by sleep patterns: warm breath from the mouth, sleeping face down, or upward staring, head shaking, and teeth grinding indicate heart heat, treated primarily with Dao Chi San (Direct Red Out Powder). (Heat in heart qi causes heat in the chest and heart; wanting to speak but unable, seeking cold, hence sleeping face down.)
Spleen heat, manifested by yellow eyes, distended abdomen, lethargy and desire to lie down, body heat, thirst, and flaccidity of limbs, treated primarily with Xie Huang San (Drain the Yellow Powder).
Kidney deficiency, manifested by crouching downwards and fear of light, treated primarily with Dihuang Wan (Rehmannia Pill). Red cheeks are due to pathogenic heat from the heart attacking upwards, suitable for Dao Chi San.
[Qian] Facial signs: Left cheek corresponds to the Liver, right cheek to the Lung, forehead to the Heart, nose to the Spleen, and jaw/cheek area to the Kidney. Redness indicates heat; treat according to the syndrome. Redness inside the eyes indicates heart heat, treated with Dao Chi San; pale red indicates heart deficiency-heat, treated with Sheng Xi San (Fresh Rhinoceros Horn Powder). Cyan color indicates liver heat, treated with Xie Qing Wan; pale cyan indicates deficiency, treat with tonification. Yellow indicates spleen heat, treated with Xie Huang San. Lack of luster in the eyes indicates kidney deficiency, treated with Dihuang Wan. (Jiegu said: Prominent five colors in the eyes indicate draining the corresponding zang organ; faint colors indicate tonifying. Also: White indicates cold, clear urine, use Sini Tang (Frigid Extremities Decoction). Yellow-red indicates heat, use Chengqi Tang (Order the Qi Decoction), drain the zang organ. Cyan indicates abdominal pain, treated with Niantou San (Pinch Head Powder), Guizhi Shaoyao Tang (Cinnamon Twig and Peony Decoction) also applies.)
[Mai] Pediatric pulse that is deep and rapid indicates heat in the bones; the child desires to press their abdomen against cool surfaces.
[Qian] Body heat without thirst indicates heat on the exterior; body heat with thirst indicates heat on the interior.
[Hai] Sishun Yinzi (Four Smooth Infusion) treats heat in the interior without cold extremities. Lianqiao San (Forsythia Powder) treats heat on the exterior without cold extremities.
[Dan] For pediatric fever, latent heat in hands and feet, and phlegm-heat, emesis is mandatory. Mild cases use Ku Shen (Sophorae Flavescentis Radix) and Chi Xiao Dou (Adzuki Bean) powder; severe cases use Gouti San (Melon Peduncle Powder) mixed with weak vinegar juice. If vomiting does not occur, stimulate the throat with a goose feather.
[Qian] The son of Supervisor Zhu, aged five, suddenly developed fever. The physician said: "This is heart heat." With red cheeks and lips, irritability, and craving for drinks, three doses of Niuhuang Wan (Calculus Bovis Pill) were administered, followed by a single-herb Xiexin Tang (Drain the Heart Decoction). The next day, the condition did not improve; instead, weakness and anorexia worsened. Another purgative was given, resulting in yellow frothy stools. Qian Yi stated: "Deficiency of the Heart channel with retained heat internally, caused by cooling purgatives, leads to this consumptive disease." Qian first used Baizhu San (Atractylodes Macrocephala Powder) to generate stomach fluids, then treated with Sheng Xi San. Zhu asked: "What about the yellow frothy stool?" Answer: "When stomach qi recovers, diarrhea stops naturally; this is deficiency-heat." Zhu asked: "How was the use of Xiexin Tang?" Qian replied: "Xiexin Tang consists only of Huanglian (Coptidis Rhizoma). Huanglian is cold in nature; excessive use causes diarrhea and damages spleen and stomach qi." After sitting for a while, other physicians arrived, all claiming it was excess heat. Qian insisted: "It is deficiency-heat." If it were excess heat, why would Xiexin Tang cause discomfort, along with sallow complexion, red cheeks, five-center vexation, anorexia, and thirst? Physician asked: "If deficiency-heat, why yellow frothy stool?" Qian laughed: "The yellow frothy stool is due to excessive consumption of Xiexin Tang." Qian cured him with Huhuanglian Wan (Coptis Chinensis Pill). Qi Langzhong of Zheng, who distributed herbal powders, had a son with sudden internal organ heat. Qi gave Qingjin Gao (Blue Gold Ointment) three times. By midnight, the child had five bowel movements, became drowsy. Qi noted frequent startles during sleep and gave another dose of Qingjin Gao. Three more bowel movements occurred, accompanied by dry mouth and fever. Qi claimed residual mild heat remained and gave more Qingjin Gao. His wife questioned if further medication would cause new diseases. Qian Yi was summoned and diagnosed "consumptive emaciation." He prescribed continuous administration of Baizhu San, followed by Xianggua Wan (Muskmelon Pill), curing him in thirteen days. (Xianggua Wan is the same formula as Xianggua San, see spontaneous sweating section.)
[Hai] Wind-heat and pathogenic heat: Si Junzi Tang (Four Gentlemen Decoction) with Shengjiang (Zingiberis Rhizoma Recens) and Jingjie (Schizonepetae Herba) decocted.
[Yun] Pediatric external heat trapped internally, loss of appetite for milk: First use Dao Chi San, then Yihuang San (Strengthen the Yellow Powder). Excess heat internally: Use Sishun Yin etc.; heat in the upper jiao requires emesis. (Formulas see Exterior-Interior section.)
[Dan] For pediatric heat diseases, Liu Yi San (Six-to-One Powder) is a miraculous drug.
Dietary injury causing stomach heat steaming.
Baizhu (1 liang), Banxia (Pinelliae Rhizoma), Huanglian (Coptidis Rhizoma) (each half liang).
Add Pingwei San (Peace the Stomach Powder), taken with plain water after meals.
[Ben] Treats pediatric fright-heat.
Quanxie (Scorpio), Tiannanxing (Arisaematis Rhizoma - take center core, powder, 1 qian), Renshen (Ginseng Radix et Rhizoma, 3 qian), Shetui (Ophiopogonis Skin/Dermatolepis, 3 qian).
Grind into powder, mix with Menthae Haplocalycis Herba and honey water for administration.
[Qian] Wind-warmth fever resembles high fever. Tidal fever: fever occurs at specific times, ceases afterwards, recurs the next day at the same time; this indicates impending wind-stroke/fright.
High fever: Continuous fever without cessation; severe cases develop fright-seizures.
Wind-warmth: Body not hot, but breath is hot; some wind-warmth syndromes are merely warm, not hot.
Shanghan (Cold Damage) heat: Hot mouth, yawning, stuffiness, stiff neck. (Treatment see Shanghan section.)
Smallpox heat: Sneezing, palpitations, cold ear tips. (Treatment see Smallpox section.)
Bianzheng (Developmental Fever): White blisters on lips, cold ears. (No treatment needed.)
Gan (Wasting-Thirst) heat: Sallow complexion, eating charcoal/earth, emaciation, red erosion under nose. (Treatment see Gan section.)
Fright-wind heat: Convulsions, palpitations, seizures, rapid pulse, vexation, screaming, confusion. (Treatment see Acute Fright-Wind section.)
[Su] Emperor asked: When an infant has heat disease and the pulse is suspended and small, what happens? Qibo replied: Survival if hands and feet are warm; death if cold. (From *Tong Ping Xu Shi Lun*, *Jia Yi Jing* refers to women nursing infants.)
[Qian] For heat syndromes requiring purgation or resolution, if no deficiency signs remain after clearing heat, do not use warming tonics; otherwise, heat will inevitably regenerate.
