The above twenty formulas generally treat acute and chronic convulsive disorders, because deficiency and excess symptoms often appear together, and acute and chronic conditions interrelate, hence there is a general treatment method. In summary, although acute and chronic convulsions differ, they both originate from Phlegm; therefore, Mengshi (Pyrite), Xing (Arisaema), and Banxia (Pinellia) types can treat them generally. Specifically, Mengshi types drain phlegm, Qingdai types drain Liver Wood, and Zhusha types drain Fire; these are all agents for treating Qi Excess. Renshen and Gancao types tonify Spleen Earth, Tianma and Quanxie types tonify Liver Wood, and Wu (Aconite) and Fu (Prepared Aconite) types tonify Kidney Fire; these are all agents for treating Qi Deficiency. Thus, this is a method of combining tonification and purgation, treating both deficiency and excess.
[Shanting] Infantile convulsive disorders. Monkey feces burned until carbonized but retaining nature, cover with a bowl to remove fire toxicity, mix with raw honey and灌 (administer) a small amount.
[Hai Zang] Restless mind and spirit. Si Junzi Tang (Four Gentlemen Decoction) plus 0.5 fen of Chen Sha (Cinnabar), mixed with Jujube Decoction. There is another syndrome: before erupting sores and rashes, the body first becomes hot with startles or convulsions. This is not convulsive disorder; use dispersing medicines.
[Yan Xiaozhong] Convulsive disorders or diarrhea accompanied by irritability and thirst indicate internal consumption of Body Fluids (Jin Ye). Regardless of Yin or Yang, decoct Qian Yi's Baizhu San (Atractylodes Powder), allowing the patient to drink to satisfaction; the more, the better.
[Mingtang] Acute and Chronic Convulsions: Yintang (Ex-HN14) moxa (Acute convulsions: draining method; Chronic convulsions: reinforcing method).
Acute Convulsions: Zhengzheng (SI7), Xialian (LI8). Infantile Chronic Convulsions: Moxa Chize (LU5) acupoints (seven moxa cones each, located in the center of the cubital crease, cone size like wheat). Startling during sleep: Lidui (ST45) (one moxa cone). Startling during sleep with eyes open: Moxa three cones above the elbow flexion crease.
[Qian Yi] Severe teeth grinding indicates impending convulsions. Eyes staring straight, face blue, body opisthotonos indicates emerging convulsions. Yawning with blue face indicates convulsive disorder. Yawning with yellow face indicates Spleen deficiency with fright. Red eyes with blue tint indicate onset of convulsions.
[Qian Yi] In convulsive epilepsy, for boys, convulsions with eyes looking left are silent, looking right produce sound; for girls, convulsions with eyes looking right are silent, looking left produce sound, due to mutual overcoming. (Jiegu said: Boys belong to Wood, so looking left to Wood position is silent, looking right to Metal position produces sound from striking. Girls belong to Metal, so looking right to Metal position is silent, looking left to Wood position produces sound from striking.)
Li Sicheng's son, three years old, had convulsions from Mao hour to Si hour, eyes looking right, crying loudly. Qian saw and said: This is a reverse pattern. Boys are Yang; originally looking left silently is smooth/normal, looking right with sound is reverse. The reason is that the Left is Liver Wood, Right is Lung Metal. Reverse means the two organs fight, Metal and Wood strike producing sound. Treatment should drain the strong and tonify the weak. Suppose a girl has convulsions, eyes looking left, this is Lung attacking Liver, Liver cannot bear it, hence crying. Should drain Lung, then treat Heart, then treat Liver. If illness is in Autumn (same as evening time), Lung is in a prosperous phase, should heavily drain Lung. If illness is in Spring (same as morning time), Liver is prosperous but still cannot overcome Lung, meaning Lung is strong and Liver Wood is weak, should tonify Liver and Kidney, heavily drain Lung. If a boy has convulsions, eyes looking right, this is Liver overcoming Lung causing crying, should drain Liver and Heart. If illness is in Spring and Summer (same as morning, midday time), Liver and Heart are prosperous, should heavily drain Liver. If illness is in Autumn and Winter (same as dusk time), Lung is prosperous but still cannot overcome Liver, meaning Liver is strong and Lung is extremely deficient, should tonify Lung, heavily drain Liver. The reason for eyes looking backward is that Liver governs the eyes. All convulsions are due to Wind-Heat fighting internally. Wind belongs to Liver, hence manifests externally in the eyes. Now this case shows male presenting female symptoms, so slightly easier to treat than female cases. First drain Lung, using Xiefei Tang (Drain Lung Decoction). After two days without confusion, know the disease is receding. Then use Dihuang Wan (Rehmannia Pill) to tonify Kidney. After three doses, use Xieqing Wan (Drain Green Pill) twice each to drain Heart and Liver, cured in five days. What if Lung deficiency does not drain?
