[Cheng Wuji] stated: "Xuan" (dizziness) does not refer to the sensation of seeing dizziness, but rather to blacking out and blurred vision. "Yun" (rotation) refers to the rotation of turning, which is commonly known as vertigo or spinning sensation in the head.
The *Neijing* discusses dizziness, attributing it all to Liver Wood and upper deficiency. Zhu Danxi discussed dizziness, emphasizing tonifying deficiency, treating phlegm, and descending fire. Zhang Zhongjing treated dizziness, prioritizing phlegm-fluid retention.
All wind diseases causing tremors and dizziness belong to the Liver. (Full text see Yan Yi chapter.)
[He Jiansu] Yao means shaking; Xuan means confusion and rotation, because Wind is active by nature. What is called wind qi moving causing head and eye dizziness and rotation occurs because Liver Wood qi is excessive, necessarily due to Metal qi being weak and unable to control Wood, while Wood generates Fire. Both Wind and Fire belong to Yang, often manifesting as combined transformations. Yang governs movement; when two Yang forces collide, it causes head and eye dizziness and rotation. Therefore, Fire is inherently moving; flames rotate when encountering wind. When people ride boats or carriages, or perform circular dances and feel dizzy, it is because the motion is continuous and the path curves left and right. Thus, the classic states: Bending and straightening, shaking and moving, are the functions of Wind. Dizziness accompanied by vomiting is due to severe Wind-Heat.
Xun Meng Zhao You (blurred vision and swaying), closing eyes, deafness, lower excess and upper deficiency, the pathology lies in the Foot Shaoyang and Foot Jueyin meridians, and if severe, enters the Liver. (Full text see Zhi Xu Shi Fa chapter.)
[*Bencao*] For treating Liver Jue (reversal) causing dizziness, clearing the head and eyes, use Gouteng San (Uncaria Decoction).
Gouteng (Uncariae Ramulus cum Uncis), Chenpi (Citri Reticulatae Pericarpium), Banxia (Pinelliae Rhizoma), Maimendong (Ophiopogonis Radix), Fuling (Poria), each half liang; Shigao (Gypsum Fibrosum), Renshen (Ginseng Radix et Rhizoma), Ganju (Chrysanthemi Flos), Fangfeng (Saposhnikoviae Radix), each half liang; Gancao (Glycyrrhizae Radix et Rhizoma) one fen.
Grind into coarse powder. Take four qian per dose, add one zhong and a half of water, seven slices of Shengjiang (Zingiberis Rhizoma Recens), decoct until eight parts remain, take warm.
For treating Liver Jue presenting like epilepsy with unconsciousness and vomiting. After waking, for the formula addressing head emptiness, vertigo, and fever:
Mahuang (Ephedrae Herba), Gouteng (Uncariae Ramulus cum Uncis, bark only), Shigao (Gypsum Fibrosum), Gegen (Puerariae Lobatae Radix), Banxiaqu (Pinelliae Fermentatum), Chaihu (Bupleuri Radix), Gancao (Glycyrrhizae Radix et Rhizoma), Zhike (Aurantii Fructus Immaturus), Ganju (Chrysanthemi Flos).
Grind into coarse powder. Take four qian per dose, add one zhong and a half of water, three slices of Shengjiang (Zingiberis Rhizoma Recens), one Jzao (Jujubae Fructus), decoct together until eight parts remain, strain and take warm.
Women suffering from Toufeng (Wind Headache) account for half of cases. Each attack is accompanied by tremors and dizziness, feeling like being on a vehicle. This is due to Blood deficiency and Wind-evil heat in the Liver. The *Suwen* states: "Xun Meng Zhao Yao, blurred vision and deafness, lower excess and upper deficiency, pathology in Foot Shaoyang and Jueyin, severe cases return to the Liver." This refers to this condition. I often give this formula to others; it is faster and more effective than other medicines. Xiong San (Ligusticum Decoction).
Chuanxiong (Ligustici Chuanxiong Rhizoma), Danggui (Angelicae Sinensis Radix), Qianghuo (Notopterygii Rhizoma et Radix), Xuanjufhua (Inulae Flos), Manjingzi (Viticis Semen), Xixin (Asari Radix et Rhizoma), Shigao (Gypsum Fibrosum), Bencao (Ligustici Wallichii Rhizoma), Jingjiesui (Schizonepetae Spica), Banxiaqu (Pinelliae Fermentatum, roasted), each half liang; Fangfeng (Saposhnikoviae Radix), Shudi (Rehmanniae Radix Preparata), Gancao (Glycyrrhizae Radix et Rhizoma), each half liang.
Grind into powder. Take two qian per dose, add one large zhong of water, three slices of Shengjiang (Zingiberis Rhizoma Recens), decoct together until seven parts remain, strain and take warm, at any time.
The above uses sweet-cold herbs to drain Liver Wood fire.
[Luo Tianyi] Case of Wind-Phlegm Treatment: Vice Minister Yang, aged seventy-two, suffered from Wind-Phlegm. In the spring of Wuchen year of Zhiyuan, he suddenly fell ill with vertigo and blacking out, unable to see, with mental restlessness, a sense of impending vomit without actual vomiting, a feeling of internal distress akin to 'Ao Nu', unilateral headache with slight swelling, red face, red cheeks, and cold feet. He asked me to treat him. I deduced that in his youth he enjoyed drinking alcohol, accumulating damp-heat internally, leading to internal Wind-Phlegm, upper heat and lower cold, indicating a state where Yin and Yang cannot communicate, resembling Pi syndrome (stagnation). The classic states: Treat heat with cold. Even a skilled physician dare not abandon standards and change the method. However, disease has distance and severity, treatment has lightness and heaviness. The Vice Minister is old and weak this year; although upper heat is severe, how dare I use cold-cooling agents to damage his Spleen and Stomach? The classic states: Heat requires bloodletting (Bian). It also states: At the high summit, pierce and draw out. I used a Sanleng needle (SP15) to prick about twenty points, drawing dark purple blood, like dewdrops. Shortly after, his head and eyes felt clear and beneficial, and all symptoms decreased. Then I prescribed: Eye blacking and head spinning are caused by internal deficient wind; only Tianma can remove it. The seedling of Tianma is called Dingfeng Cao (Wind-Calming Grass); this grass alone is not shaken by wind, so it serves as the Sovereign herb. Unilateral headache belongs to Shaoyang; only Chaihu and Huangqin processed with wine can treat it. Huanglian is bitter and cold; wine-fried to treat upper heat, serving as an accessory to the symptom, thus acting as the Minister herb. Jupi is bitter, pungent, and warm; Zhi Gancao is sweet and warm, tonifying the Middle and benefiting Qi, serving as the Assistant. Shengjiang and Banxia are pungent and warm, able to treat Wind-Phlegm; Fuling and Gancao promote urination, guiding damp-heat downward, thus serving as the Envoy herb. After taking several doses, the evil qi was calmed, healthy Qi recovered, and he was cured.
Tianma Banxia Tang (Gastrodia and Pinellia Decoction) treats internal Wind-Phlegm causing chest and diaphragm discomfort, vertigo and blacking out, drowsy urge to vomit, upper heat and lower cold, inability to lie down peacefully.
Tianma (Gastrodiae Rhizoma), Banxia (Pinelliae Rhizoma), each one qian; Jupi (Citri Reticulatae Pericarpium, white part removed), Chaihu (Bupleuri Radix), Huangqin (Scutellariae Radix, wine-soaked, fried), Gancao (Glycyrrhizae Radix et Rhizoma, roasted), Baifuling (Poria, peeled), each five fen; Qianhu (Peucedani Radix), each five fen; Huanglian (Coptidis Rhizoma), three fen.
Chop into pieces, make one dose, add two zhong of water, three slices of Shengjiang (Zingiberis Rhizoma Recens), decoct until one zhong remains, take warm. Avoid alcohol, wheat products, and raw/cold foods after meals.
[Li Gao] Qianghuo Tang (Notopterygium Decoction) treats excessive Wind-Heat congestion attacking the head and eyes causing dizziness.
Qianghuo (Notopterygii Rhizoma et Radix), Fangfeng (Saposhnikoviae Radix), Huangqin (Scutellariae Radix), each one liang, washed with wine; Chaihu (Bupleuri Radix), seven qian; Huanglian (Coptidis Rhizoma), boiled in wine, one liang; Huangbai (Phellodendri Cortex), fried with wine, half liang; Gualou (Trichosanthis Fructus), washed with wine, half liang; Gancao (Glycyrrhizae Radix et Rhizoma, roasted), seven fen; Baifuling (Poria), five qian; Zexie (Alismatis Rhizoma), six qian.
Grind into coarse powder. Take five qian per dose, decoct in water to extract clear liquid, take hot through the mouth after meals or before lying down, twice daily.
The above uses bitter-cold herbs to drain Liver Wood fire.
[*Lingshu*] The essence of tendons, bones, blood, and qi combines with the vessels to form the Eye System (Muscular system connecting to the eye), connecting upward to the brain, and exiting posteriorly from the neck. Therefore, if evil qi strikes the neck, coinciding with bodily deficiency, if it enters deeply, it follows the Eye System into the brain. Entering the brain causes brain rotation; brain rotation pulls the Eye System tight; tight Eye System causes dizziness and rotation. (Da Huo Pian)
[*Bencao*] Treating all types of vertigo, originating from constitutional weakness, where Wind-evil attacks the Yangming Meridian, ascending to the head and face, then entering the brain. Also due to Phlegm-Fluid remaining in the chest and diaphragm, invading severe cold, preventing Yang Qi from circulating, causing Phlegm-Fluid accumulation, rushing upward to the head and eyes, causing head rotation and vertigo. Lingyangjiao San (Antelope Horn Decoction).
Lingyangjiao (Saigae Tataricae Cornu), Fushen (Poria cum Radice Pinus), each one liang; Chuanxiong (Ligustici Chuanxiong Rhizoma), Fangfeng (Saposhnikoviae Radix), Baizhi (Angelicae Dahuricae Radix), Banxia (Pinelliae Rhizoma, washed seven times in boiling water), each half liang; Zhike (Aurantii Fructus Immaturus), Fuzi (Aconiti Lateralis Radix Preparata), each one fen; Gancao (Glycyrrhizae Radix et Rhizoma), three qian.
Grind into coarse powder. Take four qian per dose, add one zhong and a half of water, five fen of Shengjiang (Zingiberis Rhizoma Recens), decoct slowly over low heat until seven parts remain, strain and take warm at any time.
[*Baiyi Xuanfang*] Duliang Wan (Duliang Pill) treats Wind blowing on the neck and back, causing head and eyes to feel dark, dizzy, and painful.
Xiang Baizhi (Angelicae Dahuricae Radix, large pieces, soaked and washed four to five times in boiling water, dried by baking).
Grind into powder, refine honey to make pills, size of bullet seeds. Take one pill per dose, chew finely, swallow with Jingjie tea infusion.
[Wu] Dadouzi Tang (Soja Seed Black Skin Decoction) treats Stroke with head dizziness, aversion to wind, spontaneous vomiting of cold water, and sweating. (Formula see [Missing] Chapter.)
[*Shengji Zonglu*] Treating Wind Head Vertigo, use two liang of Chanqiao (Cicadae Periostracum), lightly fried and ground into powder. Take at any time, with one zhong-bi of warm wine. The *Zhenjing · Weifeng Pian* states: Upper deficiency causes dizziness. The *Wuzang Shengcheng Pian* states: "Xun Meng Zhao You, closing eyes, deafness, lower excess and upper deficiency." Meng means dizziness/confusion. Zhaoyao means trembling/shaking. Ming means blacking out/dizziness. All refer to confusion, trembling, blacking out, and dizziness, belonging to lower excess and upper deficiency.
[Zhu Danxi] Patient Chen, a female guest, nearly fifty years old, suffered from headache, numbness, and vertigo, with a very deficient pulse. Should tonify Qi and nourish Blood, dispel Wind and dry Dampness.
Tianma (Gastrodiae Rhizoma, wine-soaked), one qian; Renshen (Ginseng Radix et Rhizoma), half qian; Baizhu (Atractylodis Macrocephalae Rhizoma), one qian; Huangqin (Scutellariae Radix), half qian; Danggui Shenwei (Angelicae Sinensis Radix, wine-soaked), half qian; Chuanxiong (Ligustici Chuanxiong Rhizoma), half qian; Chenpi (Citri Reticulatae Pericarpium), half qian; Gancao (Glycyrrhizae Radix et Rhizoma, roasted), three fen; Huangqi (Astragali Membranacei Radix), three fen; Banxia (Pinelliae Rhizoma), five fen; Shengjiang (Zingiberis Rhizoma Recens), three slices.

(Formula see Women's Major Laws section.)
[Zhu Danxi] Head dizziness belongs to Qi deficiency with Phlegm attachment. If dizziness is without Phlegm, do not focus primarily on treating Phlegm. Combine with Qi-tonifying herbs and Fire-descending herbs. For abundant Damp-Phlegm, use Erchen Tang. For Phlegm moved by Fire, add wine-fried Huangqin to Erchen Tang. For vertigo, Fire moves its Phlegm; add Huangqin, Cangzhu, and Qianghuo to Erchen Tang to disperse Wind and dry Dampness. Shuizhu Jinhua Wan treats head dizziness. (Formula see Phlegm-Fluid section.)
[Zhu Danxi] For unbearable vertigo, use Dahuang (Rhei Radix et Rhizoma), fried three times with wine until dry, ground into powder, mixed with tea and taken. One to two qian per dose.
A man, seventy-nine years old, had heavy and dizzy head and eyes, powerless hands and feet, continuous expectoration of phlegm. Left hand pulse was scattered, large, and slow; right hand pulse was slow and larger than the left but weaker upon deep pressure. Diet slightly reduced with slight thirst, bowel movement once every three to four days. Everyone gave Wind-dispersing herbs, predicting death by late spring. I said: This is a major deficiency syndrome; should take large doses of tonifying medicine. Everyone left angrily. I taught him to use Renshen (Ginseng Radix et Rhizoma), Huangqi (Astragali Membranacei Radix), Danggui Shen (Angelicae Sinensis Radix tip), Shaoyao (Paeoniae Radix), Baizhu (Atractylodis Macrocephalae Rhizoma), Chenpi (Citri Reticulatae Pericarpium), decoct thickly as soup, and take thirty granules of Lianbai Wan. Taken for a year and a half in this manner, his energy returned to that of his youth. Lianbai Wan: Add a little Ganjiang (Zingiberis Rhizoma) in winter; for the other three seasons, follow the same method. Lianbai are both fried with Jiangzhi (Ginger Juice) into fine powder, then made into pills using Ginger Juice paste.

This medicine regulates Yin and Yang, supplements and connects True Qi, not just treating head vertigo.
[Li Gao] Fan Tianxi's wife, long suffered from Spleen and Stomach disease, occasionally showing irritability, chest discomfort, and constipation. Early winter, she went out and returned late, constrained by Cold Qi, causing great oppression and chaos, Fire unable to extend. Doctors suspected Heat, treated with Shufeng Wan. Bowels moved but illness did not decrease. Suspecting insufficient drug strength, added another seventy to eighty pills, resulting in two bowel movements. Previous symptoms remained unchanged, adding vomiting, inability to keep food down, thick sticky saliva, continuous gushing, blacking out eyes and head vertigo, nausea and vexation, shortness of breath, weak upper panting, reluctance to speak, mental confusion, constant drowsiness, eyes unable to open, feeling like in wind and clouds, severe headache like splitting, body heavy as mountain, cold limbs, inability to lie down peacefully. I said the previous symptoms were due to damaged Stomach Qi; further purging twice severely damaged the Stomach, causing Phlegm-Jue Headache. Prepared Banxia Baizhu Tianma Tang to treat and cure.
Tianma (Gastrodiae Rhizoma), five fen; Banxia (Pinelliae Rhizoma, washed in boiling water), one qian half; Renshen (Ginseng Radix et Rhizoma), five fen; Baizhu (Atractylodis Macrocephalae Rhizoma), one qian; Cangzhu (Atractylodis Rhizoma), five fen; Jupi (Citri Reticulatae Pericarpium), Huangqi (Astragali Membranacei Radix), Zexie (Alismatis Rhizoma), Baifuling (Poria), each five fen; Shenqu (Massa Medicata Fermentata), one qian, fried; Da Mai Nie (Hordei Fructus Germinatus), one qian half; Ganjiang (Zingiberis Rhizoma), three fen; Huangbai (Phellodendri Cortex), two fen.
Chop the above ingredients. Take half liang per dose, two zhan of water, decoct until one zhan remains, strain, take hot before meals. This severe headache is called Foot Taiyin Phlegm-Jue Headache; only Banxia can treat it. Eye blacking and head spinning are caused by internal deficient wind; only Tianma can remove it. Its seedling is Dingfeng Cao, also a divine herb for treating internal Wind. Internal Wind is Deficient Wind.
Huangqi is sweet and warm, draining Fire and supplementing Original Qi. Renshen is sweet and warm, draining Fire and tonifying Middle and benefiting Qi. Both Baizhu and Cangzhu are sweet, bitter, and warm, drying Dampness, tonifying Middle, and benefiting Qi. Zexie and Fuling promote urination and guide Dampness. Jupi is bitter and warm, benefiting Qi, regulating Middle, and raising Yang. Shenqu digests food and clears stagnant Qi in the Stomach. Da Mai Nie broadens the Middle and assists Stomach Qi. Ganjiang is pungent and hot, washing away Middle Cold. Huangbai is bitter and greatly cold, wine-washed to address Winter Lesser Fire arising from the Interior causing irritability.
The above is Qi deficiency with Phlegm causing vertigo. I once treated a person who felt slightly better when lying down, but head spun and eyes blacked out when lifting feet. Used Tianma, Banxia, Fuling, Chenpi, Baifu, Jiangcan, Renshen, Caos (Gancao), Guis (Danggui), Shengjiang, Huangqin decocted in soup. Cured in five to six days, likely imitating and modifying this formula.
