[Li Dongyuan] Body heaviness is caused by dampness; the primary treatment is Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) with Guizhi (Cinnamon Twig) removed and Wuling San (Five-Ingredient Powder with Poria) added.
[Zhang Jiegu] Difficulty in rising and lying down cannot be defined as dampness; body heaviness is the manifestation of dampness, which can be treated with Xiaochaihu Tang (Minor Bupleurum Decoction) or Huangqin Shaoyao Tang (Scutellaria and Peony Decoction).
[Zhang Zhongjing] In cases of Wind-Dampness presenting with a floating pulse, body heaviness, sweating, and aversion to wind, Fangji Huangqi Tang (Stephania and Astragalus Decoction) is indicated. (See Shanghan Lun for the formula details on body heaviness.)
[Zhang Jiegu] In summer, contracting Wind-Dampness results in body heaviness as if sitting in a mountain, making it impossible to turn or shift position. Treatment should employ herbs that dispel wind, overcome dampness, and clear heat.
[Zhang Zhongjing] The disease known as "Kidney Fixation" (Shen Zhuo) presents with body heaviness, coldness in the lower back as if sitting in water, and an appearance resembling edema, yet without thirst, with normal urination and appetite. The pathology belongs to the Lower Jiao (Lower Burner). It arises from labor-induced sweating leading to chronic exposure of the exterior and interior to cold-dampness. Over time, this causes cold pain below the waist and abdominal heaviness as if bound by five thousand coins. Gan Jiang Ling Zhu Tang (Ginger, Poria, Atractylodes, and Licorice Decoction) is indicated.
Gancao (Licorice Root), Baizhu (White Atractylodes Rhizome) [each 2 liang]; Ganjiang (Dried Ginger), Fuling (Poria) [each 4 liang].
The above four ingredients are boiled in 5 sheng of water to yield 3 sheng, divided into two warm doses. The waist will naturally become warm.
[Su Wen] Deficiency of the Liver, Kidney, or Spleen all cause body heaviness and mental distress/oppression. (The Shicongrong Lun indicates that heaviness relates to Liver and Kidney deficiency: Liver deficiency leads to Spleen lacking restraint causing heaviness; Kidney deficiency leads to Water overacting on Spleen causing heaviness.)
[Li Dongyuan] Shen Shu Tang (Ginseng and Atractylodes Decoction): Treats Spleen-Stomach weakness where Yuan Qi fails to nourish the Heart and Lungs, resulting in heavy limbs and postprandial drowsiness and oppression.
Huangqi (Astragalus Root) [2 qian], Renshen (Ginseng Root) [0.5 qian], Shengma (Cimicifuga Rhizome) [3 fen], Zhi Gancao (Honey-fried Licorice Root) [4 fen], Chaihu (Bupleurum Root), Jiu Huangbai (Wine-processed Phellodendron Bark) [each 3 fen], Chenpi (Aged Tangerine Peel) [5 fen], Qingpi (Green Tangerine Peel) [5 fen], Shenqu (Medicated Leaven) [7 fen], Cangzhu (Black Atractylodes Rhizome) [1 qian], Danggui (Angelica Sinensis Root) [2 fen].
The above herbs are coarsely crushed, prepared as one dose, boiled in 2 zhan of water until 1.5 zhan remains, strained, and taken hot before meals.
Regarding Yunqi (Six Qi and Five Movements) theory, there are five types of body heaviness:
First is Dampness, where dampness suppresses Kidney deficiency leading to heaviness. The classics state: "When Taiyin arrives, it manifests as body heaviness." Also: "When Taiyin rebounds, the body is heavy and distended." Also: "When the Year's Earth is excessive, dampness spreads, and people suffer from heavy bodies and mental distress." Also: "When Earth stagnation erupts, people suffer from body heaviness."
Second is Damp-Heat. The classics state: "In years governed by Shaoyang Fire at Heaven, during the fourth Qi period, intense heat alternates with transformation, causing illness characterized by whole-body heaviness."
Third is Cold-Dampness. The classics state: "In years governed by Taiyin Earth at Heaven, during the third Qi period, contraction of cold-dampness causes people to suffer from body heaviness."
Fourth is Wind, where Wind controls Spleen deficiency leading to heaviness. The classics state: "When the Year's Wood is excessive, Wind spreads, and people suffer from heavy bodies and mental distress." Also: "When the Year's Earth is insufficient, Wind prevails greatly, and people suffer from heavy bodies and mental distress." Also: "When Jueyin Wind is at Earth, Wind excess wins, causing the whole body to feel heavy."
Fifth is Metal/Dryness, where Dryness controls Liver deficiency leading to heaviness. The classics state: "When the Year's Metal is excessive, Dryness spreads, and people suffer from heavy bodies and mental distress."
Acupuncture treatments for body heaviness involve two methods:
One involves selecting the Spleen. The classics state: "For Spleen diseases manifesting as body heaviness and muscle flaccidity, select the collaterals of the Foot-Taiyin, Foot-Yangming, and Foot-Shaoyin meridians." Also: "When the Foot-Taiyin Spleen meridian moves, it causes abdominal fullness and whole-body heaviness; treat based on excess/deficiency, heat/cold, and sinking/depression."
Two involves selecting the Kidney. The classics state: "For Kidney diseases manifesting as body heaviness, night sweats, and aversion to wind, select the collaterals of the Foot-Shaoyin and Foot-Taiyang meridians."
