Liaoning Ma XX, aged fifty, suffered from wind-water edema accompanied by phlegm syndrome.
Etiology The condition arose from traveling far in late autumn, where fatigue and exposure to wind led to this illness.
Symptoms Abdominal distension, generalized swelling throughout the body, rapid and labored breathing, occasional obstruction of saliva and phlegm in the throat, chest, and sternum area, pale white tongue coating, scanty dark yellow and painful urination, bowel movements every other day, pulse is without heat signs but slightly floating. It is diagnosed as wind-water; treatment should follow the formulas for wind-water in the *Jin Gui Yao Lue*.
Prescription Raw Gypsum (30g, crushed fine), Ephedrae Herba (9g), Glycyrrhizae Radix et Rhizoma (6g), Zingiberis Rhizoma Recens (6g), Jujubae Fructus (4 pieces, split open), Western medicine Aspirin (1g).
The prescription consists of six ingredients. Decoct the first five ingredients in a large bowl of water, dissolve the Aspirin in the decoction, and take it warm while covering with blankets to induce sweating.
Formula Analysis This formula is the original Yuebi Tang (Yuebi Decoction) plus Western medicine Aspirin. At that time, early winter, the weather in the north was cold and sweat was difficult to induce. Fearing that taking only Yuebi Tang would not induce sweating, Aspirin, which is excellent at inducing sweating and clearing heat among Western medicines, was added as an adjuvant.
Follow-up Visit After taking the medicine, sweat appeared all over the body, and the rapid breathing ceased immediately. Other symptoms remained unchanged, but there was new onset of heat and thirst in the heart region, loss of appetite. Palpation of the pulse still showed no heat signs. This is likely due to excessive phlegm-fluid retention and prevailing dampness. In this case, one should disregard the pulse and treat according to the symptoms, using a heavy dose of heat-clearing agents.
Prescription Raw Gypsum (120g, crushed fine), Trichosanthis Radix (24g), Menthae Haplocalycis Folium (5g).
Decoct together in a large bowl of water, and drink slowly in multiple divided doses while warm.
Third Visit After taking the medicine, the heat, thirst, and phlegm-saliva improved by more than half. Urination increased, and appetite returned, but the generalized swelling and abdominal distension did not significantly alleviate. Therefore, emphasis should be placed on promoting diuresis to reduce the swelling, with a small addition of qi-regulating herbs to relieve abdominal distension.
Prescription Raw Gypsum (30g, crushed fine), Talcum (30g), Kochiae Fructus (9g), Helianthi Annui Semen (9g, crushed), Chrysosplenii Herba (9g), Arecae Semen (9g), Imperatae Rhizoma Recens (9g).
Decoct together in one and a half large bowls of water, and take warm in two divided doses.
"Zhangju" is commonly known as sunflower. Upon investigation, the name "sunflower" should belong to *Helianthus annuus* (Wei Zu Hua), and cannot be applied to Zhangju. Zhangju Zi (Sunflower Seed) is not recorded in the *Ben Cao Gang Mu*, but because it is effective in treating strangury pain and promoting diuresis, it was used in this prescription.
Outcome After taking two doses of the decocted medicine, urination became profuse, and both swelling and distension subsided. Consequently, the dosages of Gypsum, Talcum, and Arecae Semen in the formula were halved. After taking three consecutive doses, the disease was completely cured.
