[General Principle] The head is the common domain of the three Yang meridians (Taiyang, Yangming, Shaoyang). Medications should be administered according to the specific meridian involved. If dizziness requires differentiation between deficiency and excess, treatment should follow two paths: draining excess and nourishing deficiency. Headache located at the back, or accompanied by fever and aversion to cold, indicates Cold damage affecting the Taiyang meridian. Jiawei Baidu San (Modified Decoction to Overcome Toxins) is appropriate.
Ginseng Radix (Sha Shen, 2 qian), Notopterygii Rhizoma et Radix (Qiang Huo, 2 qian), Angelicae Pubescentis Radix (Du Huo, 2 qian), Bupleuri Radix (Chai Hu, 2 qian), Peucedani Radix (Qian Hu, 3 qian), Ligustici Chuanxiong Rhizoma (Chuan Xiong, 1 qian), Platycodonis Radix (Jie Geng, 3 qian), Poria (Yun Ling, 3 qian), Aurantii Fructus Immaturus (Zhi Ke, 1 qian), Glycyrrhizae Radix et Rhizoma (Gan Cao, 1 qian), Puerariae Lobatae Radix (Ge Gen, 2 qian), Jujubae Fructus (Da Zao, 2 pieces), Allii Fistulosi Bulbus (Cong Bai, 1 piece), Zingiberis Rhizoma Recens (Sheng Jiang, 3 slices).
The verse states: For headache at the back, use Baidu Tang; Bupleuri, Peucedani, Ligustici, Platycodon, Angelicae Pubescentis, and Ginseng Radix. Aurantii Fructus Immaturus, Poria, Allii Fistulosi, Puerariae, Glycyrrhizae, and Jujubae harmonize, with Zingiberis Rhizoma Recens as a guide to enter the Taiyang meridian.
(Note by Bo An: Applying fresh ginger slices to both Taiyang acupoints on the temples is also helpful in folk practice.)
Headache located at the side, or accompanied by alternating chills and fever, indicates Wind pathogen affecting the Shaoyang meridian. Jiawei Chaihu Tang (Modified Bupleuri Decoction) is appropriate.
Pinelliae Rhizoma Preparatum (Ban Xia, 3 qian), Bupleuri Radix (Chai Hu, 3 qian), Bamboo Shavings (Zhu Ru, 3 qian), Polygonati Odorati Rhizoma (Yu Zhu, 3 qian), Scutellariae Radix (Huang Qin, 3 qian), Paeoniae Radix Alba (Bai Shao, 3 qian), Uncariae Ramulus cum Uncis (Gou Teng, 3 qian), Glycyrrhizae Radix et Rhizoma (Gan Cao, 1 qian), Zingiberis Rhizoma Recens (Sheng Jiang, 3 slices), Jujubae Fructus (Da Zao, 2 pieces).
The verse states: Headache at the side is Shaoyang Wind; removing pathogens relies on Pinellia and Bupleuri. Bamboo shavings, Polygonati Odorati, Zingiberis, and Jujubae, combined with Glycyrrhizae, Uncariae, Scutellariae, and Paeoniae Alba.
(Note by Bo An: In the Shu edition, Uncariae Ramulus cum Uncis is written as Schizonepetae Spica/Jing Jie.)
Headache located at the front, or accompanied by fever and thirst, indicates Heat pathogen affecting the Yangming meridian. Jiawei Shengge Tang (Modified Pueraria and Cimicifuga Decoction) is appropriate.
Paeoniae Radix Alba (Bai Shao, 3 qian), Puerariae Lobatae Radix (Ge Gen, 3 qian), Scutellariae Radix (Huang Qin, 3 qian), Angelicae Dahuricae Radix (Bai Zhi, 3 qian), Trichosanthis Radix (Hua Fen, 4 qian), Cimicifugae Rhizoma (Sheng Ma, 1 qian), Glycyrrhizae Radix et Rhizoma (Gan Cao, 1 qian).
The verse states: For headache at the front with heat and thirst, use Paeoniae Alba, Scutellariae, Puerariae, and Cimicifugae. Glycyrrhizae combined with Trichosanthis Radix generates fluids; Angelicae Dahuricae has the strongest power to expel pathogens.
Leishouteng (Thunder-head wind pain), occurring either on the left or right, indicates Wind-Cold invasion due to Yang deficiency. Moxibustion and salt massage are appropriate.
Moxa wool and Moschus (She Xiang) are formed into small pellets for moxibustion on the painful area; Aconiti Lateralis Radix Preparata (Fu Zi) powder mixed with salt is used for massage. (Subsequently, Jiawei Baitong Tang [Modified Baitong Decoction] is taken; see next section for formula.)
The verse states: Leishouteng pain is treated with Fu Zi and salt massage; Moschus and Moxa burned together should not be excessive. Jiawei Baitong contains Ginger, Aconite, and Atractylodes, harmonized with Allium, Ginseng, and Glycyrrhizae.
(Note by Bo An: Qingzhen Tang can also be taken: Cimicifugae Rhizoma 8 fen, Atractylodis Macrocephalae Rhizoma 1 qian, Lotus Leaf 1 corner.)

The verse states: Pain persists every time it rains and clouds, because True Yang does not ascend to the head. Taking modified Baitong Tang will cure the disease completely with just one dose.
Headache feeling like breaking, accompanied by vomiting of saliva/phlegm, indicates Cold-fluid retention reversing in the Liver meridian. Jiawei Wuyu Tang (Modified Evodia Decoction) is appropriate.
Evodiae Fructus (Wu Yu, 2 qian), Codonopsis Radix (Dang Shen, 3 qian), Poria (Yun Ling, 3 qian), Cinnamomi Ramulus (Gui Jian, 3 qian), Pinelliae Rhizoma Preparatum (Ban Xia, 3 qian), Paeoniae Radix Alba (Bai Shao, 3 qian), Asari Radix et Rhizoma (Xi Xin, 5 fen), Glycyrrhizae Radix et Rhizoma (Gan Cao, 1 qian), Jujubae Fructus (Da Zao, 3 pieces), Zingiberis Rhizoma Recens (Sheng Jiang, 3 slices).
The verse states: Headache like breaking with phlegm vomiting; fluid reversal from Liver cold requires Ginger and Jujube. Codonopsis, Glycyrrhizae, Evodia, Asari, and Cinnamon Twig are used; Poria, Pinellia, and Paeoniae Alba are decocted in the soup.
Dizziness with mental cloudiness and oppression, accompanied by irritability, thirst, and chest fullness, indicates Fire carrying Phlegm turbidity upward. Jiawei Yinju Tang (Modified Honeysuckle and Chrysanthemum Decoction) is appropriate (add Rheum Radix et Rhizoma prepared with wine if head feels heavy).

The verse states: Dizziness and oppression are treated with Honeysuckle and Chrysanthemum; Bupleuri, Poria, Trichosanthis, Armeniacae, and Inula are applied. Bamboo shavings, Scutellariae, Aurantii Fructus Immaturus, and Glycyrrhizae harmonize; adding Mentha disperses the condition.
(Note by Bo An: The Shu edition lacks Chrysanthemi Flos, Lonicerae Japonicae Flos, Trichosanthis Radix, and Aurantii Fructus Immaturus.)
Swaying dizziness, accompanied by redness of the malar bones, indicates Deficient Fire stirring in the Kidney meridian. Jiawei Dihuang Tang (Modified Rehmannia Decoction) is appropriate.
