Annotation:
Original Text: The eyelids belong to Spleen diseases, the inner and outer canthi are governed by the Heart, the white sclera belongs to the Lung, the black cornea belongs to the Liver, and the pupil belongs to the Kidney. Diseases of the Five Wheels correspond to the Five Zang organs; their cold, heat, deficiency, and excess patterns should be differentiated according to the specific symptoms.
Formula: None
Annotation [1]: The eyelids constitute the Flesh Wheel (Rou Lun), governing Spleen diseases. The inner and outer canthi constitute the Blood Wheel (Xue Lun), governing Heart diseases. The white sclera constitutes the Qi Wheel (Qi Lun), governing Lung diseases. The black cornea constitutes the Wind Wheel (Feng Lun), governing Liver diseases. The pupil constitutes the Water Wheel (Shui Lun), governing Kidney diseases.
[2]: The pathologies of the Five Wheels are dominated by the Five Zang organs. Their nature of cold, heat, deficiency, or excess should be distinguished based on the manifested symptoms.
Original Text: The Five Wheels are the Flesh Wheel, Blood Wheel, Qi Wheel, Wind Wheel, and Water Wheel. The Flesh Wheel corresponds to the upper and lower eyelids, the Blood Wheel to the inner and outer canthi, the Qi Wheel to the white sclera, the Wind Wheel to the black cornea, and the Water Wheel to the pupil. These must be clearly understood.
Formula: None
Annotation [1]: The Five Wheels refer to the Flesh Wheel, Blood Wheel, Qi Wheel, Wind Wheel, and Water Wheel.
[2]: They are called "Wheels" because the movement of the eyeball resembles a rotating wheel.
[3]: The upper and lower eyelids are the Flesh Wheel; the inner and outer canthi are the Blood Wheel; the white sclera is the Qi Wheel; the black cornea is the Wind Wheel; and the pupil is the Water Wheel. This clarifies the locations of the Five Wheels and their attribution to the Five Zang organs (Figure 1).
Original Text: Internal cataracts are all caused by injury from the Seven Emotions (Qing): excessive joy, anger, worry, thought, grief, fear, and shock. These damage the internal Zang and Fu organs, preventing Jing-Qi (Essence-Qi) from ascending to nourish the eyes. Initially, this manifests as internal cataracts; over time, it evolves into the affliction of the Five Winds.
Formula: None
Annotation [1]: This explains the etiology of internal cataracts.
[2]: "Zhang" means obstruction or covering; "internal cataract" refers to obstruction originating from within.
[3]: The disease of internal cataracts is caused by excessive injury from the Seven Emotions: excessive joy injures the Heart, excessive anger injures the Liver, excessive worry injures the Lung, excessive thought injures the Spleen, excessive grief injures the Heart, excessive fear injures the Kidney, and excessive shock injures the Gallbladder. The internal Zang and Fu organs are damaged, and Jing-Qi fails to ascend to the eyes.
[4]: Therefore, the initial stage presents as internal cataracts, which, if prolonged, develop into the afflictions of the Five Winds (Wu Feng: Black Wind, Green Wind, Dark Wind, Yellow Wind, Cyan Wind).
[5]: The clinical manifestations include no redness or swelling, changes in the color of the pupil, and an initial brightening of vision (or light perception).
