Attribute: For conditions resulting from trauma presenting with wheezing and coughing:
[1] If caused by excessive blood loss, manifesting as a dark complexion, chest distension, pain in the chest and diaphragm, accompanied by wheezing, this is due to Qi deficiency allowing Blood to invade the Lung. Emergency administration of Erwei Rensu Yin (Two-Ingredient Ginseng and Perilla Decoction) is required; delay makes rescue difficult.
[2] If hemoptysis (coughing up blood) or epistaxis (nosebleed) occurs with wheezing, this is due to rebellious Qi causing Blood to stagnate in the Lung. Only methods promoting Blood circulation and moving Qi are suitable; purgative methods must be avoided. Shiyi Rensu Yin (Ten-Ingredient Ginseng and Perilla Decoction) should be used for treatment.
Note on Formulas:
[1] Erwei Rensu Yin (Two-Ingredient Ginseng and Perilla Decoction)
Renshen (Ginseng Radix et Rhizoma) 1 liang, Sumu (Sappan Lignum) 2 liang. Decoct in water and take orally.
[2] Shiyi Rensu Yin (Ten-Ingredient Ginseng and Perilla Decoction)
Renshen (Ginseng Radix et Rhizoma), Zisu (Perillae Folium), Banxia (Pinelliae Rhizoma Preparatum), Fuling (Poria), Chenpi (Citri Reticulatae Pericarpium), Jiegeng (Platycodonis Radix), Qianhu (Peucedani Radix), Gegen (Puerariae Lobatae Radix), Zhike (Aurantii Fructus Immaturus) each 1 qian, Gancao (Glycyrrhizae Radix et Rhizoma) 0.5 qian. Add 2 slices of fresh ginger as an adjuvant. Decoct in water and take orally.
Note:
Attribute:
[1] Trauma-induced Hunkui (coma and confusion) indicates severe injury leading to unconsciousness and loss of awareness. Emergency administration of Dushen Tang (Single-Ginseng Decoction) is indicated.
[2] Even if intra-abdominal stasis of Blood is present, purgative methods must absolutely not be used. Emergency administration of Huaruishi San (Huaruishi Powder) is required to resolve the stasis. This is because purgatives may cause diarrhea, leading to Wangyin (collapse of Yin fluids).
[3) In cases of extreme deficiency of Yuanqi (Original Qi), purgatives are even more contraindicated. The aforementioned Huaruishi San should also be used to resolve the stasis.
[4) For any case where intra-abdominal stasis of Blood causes constipation, if Da Huang (Rhei Radix et Rhizoma) and Puxiao (Natrii Sulfas) alone fail to move the coagulated Blood, Muxiang (Aucklandiae Radix) and Rougui (Cinnamomi Cortex) 2–3 qian should be added. Administer orally mixed with hot wine. Survival depends on the passage of the stasis.
[5) For individuals with weak constitutions, when using Puxiao (Natrii Sulfas) and Da Huang (Rhei Radix et Rhizoma), Muxiang (Aucklandiae Radix) and Rougui (Cinnamomi Cortex) must be added for simultaneous decoction. This utilizes their heat properties to facilitate the action of the cold-natured drugs (borrowing heat to promote the movement of cold).
Note on Formula Preparation:
[1] Huaruishi San (Huaruishi Powder)
Shiliuhuang (Sulfur) 4 liang, Huaruishi (Flower-bearing Ochre) 2 liang.
Mix the above two ingredients evenly. Place them in one earthenware jar, seal the mouth, and reinforce the exterior with paper fibers and salt mud around the jar. Wait for the mud to dry, place the jar on four square bricks, inscribe Bagua (Eight Trigrams) and Wuxing (Five Phases) characters on it. Surround the jar completely with 10 jin of charcoal. Ignite the fire from the bottom starting at Wu hour (11:00 AM–1:00 PM), heating gradually upwards until the charcoal is consumed after one night and has cooled. Leave it for another night. Once the jar is cool, remove it, grind into fine powder, sieve through silk gauze, and store in a porcelain box. Take 3 qian per dose, mixed with Tongbian (Child’s Urine).
