Dushen Decoction (Single Ginseng Decoction) Huisheng Dan (Revival Pill)
22. In cases of abortion with sudden, profuse bleeding that does not stop, and where the complexion is sallow and lips are pale, use Dushen Decoction (Single Ginseng Decoction). If lochia (postpartum discharge containing blood and tissue) is stagnant and fails to exit, causing abdominal distension and pain, select Huisheng Dan (Revival Pill) or Yimu Wan (Motherwort Pill), choosing appropriately based on the deficiency/excess nature and urgency of the condition.
[Note] After an abortion during pregnancy, if blood flows out suddenly and profusely without stopping, accompanied by a sallow complexion and pale lips, this is termed "Tuo Rong" (Collapse of Essence/Qi following blood loss). It is appropriate to use Dushen Decoction (Single Ginseng Decoction) to vigorously tonify Qi, thereby generating Blood. This illustrates the principle that "formless Qi can generate formed Blood." If lochia is stagnant and obstructed, causing distension and pain in the abdomen and hypochondrium, one should choose between Huisheng Dan (Revival Pill) and Yimu Wan (Motherwort Pill), selecting the suitable formula according to the patient's deficiency/excess status and the urgency of the disease.
Huisheng Dan (Revival Pill) See Postpartum Formulas Collection.
23. When the fetus dies in the uterus, urgent purgation is required; a cyan tongue and cold, ice-like abdominal pain indicate severity. Prolonged retention leads to foul breath from the mouth; the nature of Cold/Heat must be carefully weighed for treatment.
[Note] In any case where trauma causes the fetus to die in the uterus, urgent purgation must be employed to expel the dead fetus, preventing its pathogenic influence from rushing upward to the heart and chest. However, one must verify signs such as a cyan tongue, red face, abdominal distension, coldness of the abdomen like ice, and the emission of foul odor from the mouth after some time, before discussing the use of purgation. Furthermore, it is essential to carefully examine the patient's deficiency/excess and Cold/Heat patterns. Treatment may involve Cold-purgation, Heat-purgation, vigorous purgation, or gentle purgation, applied according to what is most suitable.
Foshou San (Buddha's Hand Powder) Pingwei San (Stomach-Pacifying Powder) plus Mangxiao (Mirabilite)
24. For gentle expulsion of the fetus, use Foshou San (Buddha's Hand Powder); for vigorous expulsion, use Pingwei San (Stomach-Pacifying Powder) added with Mangxiao (Mirabilite). Whether Cold-purgation or Heat-purgation is indicated requires careful examination; do not confuse the deficiency/excess states of the parturient woman.
[Note] If a pregnant woman has a dead fetus in the uterus requiring expulsion, use Foshou San (Buddha's Hand Powder) for gentle purgation, and Pingwei San (Stomach-Pacifying Powder) added with Mangxiao (Mirabilite) for vigorous purgation. Whether Cold-purgation or Heat-purgation is appropriate must be meticulously examined before administering medication. One must consider the deficiency/excess state of the mother to decide between gentle or vigorous purgation, avoiding confusion or rashness to prevent misdiagnosis and error.
Foshou San (Buddha's Hand Powder) See Volume 1.
25. In all critical conditions during pregnancy, the survival of mother and child can be predicted in advance: Red face and cyan tongue indicate fetal death; cyan face and red tongue indicate maternal danger; both face and tongue cyan with frothy saliva indicate the death of both mother and child.
[Note] In all critical and dangerous manifestations during pregnancy, to determine the survival of the mother and child, one should judge based on the color of the pregnant woman's face and tongue. If the face is red and the tongue is cyan, the fetus is certainly dead. If the face is cyan and the tongue is red, the mother is certainly in mortal danger. If both the face and tongue appear cyan, and there is salivation/froth at the corners of the mouth, then neither the mother nor the child can be saved.
