When the smallpox rash has not yet appeared and convulsions are present, Qingjie San (Clearing and Resolving Decoction) should be used to enhance clearing and resolving power; if the rash has emerged but convulsions persist, Daochi San (Guide Out Redness Decoction) is indicated; for deficiency-type convulsions occurring after the scabs have fallen off, Ningshen Tang (Calming the Spirit Decoction) should be administered; for convulsions triggered by food stagnation and fever in the post-smallpox stage, Dadiankang Wan (Great Peaceful Health Pill), also known as Muxiang Da'an Wan, is appropriate.
[Note]
"Jing Dou" (Startled Smallpox) is mostly caused by the fire of smallpox toxin disturbing the Heart-Mind, with heat transferring to the Liver. The Liver Wind and Heart Fire struggle against each other, resulting in this syndrome.
The treatment method must distinguish between the initial and later stages of the disease.
For instance, if convulsions occur before the smallpox rash emerges, one must not solely use cold-natured herbs, as this may obstruct and close up the toxin, preventing its release.
One should only use Qingjie San (Clearing and Resolving Decoction) to disperse exterior pathogens; once the rash emerges, the convulsions will naturally cease.
If the rash has already taken shape but convulsions continue unabated, this indicates that toxic fire is internally lodged in the Heart Meridian. It is suitable to treat this with Daochi San (Guide Out Redness Decoction) added with Huanglian (Coptis Rhizome).
If spasms occur after the scabs have fallen off (Yie Hou), it signifies a deficiency of True Qi with internal attack by pathogenic fire; Ningshen Tang (Calming the Spirit Decoction) should be the primary prescription.
If convulsions are triggered by food stagnation and fever after the recovery from smallpox, this is due to Spleen and Stomach weakness, inevitably accompanied by sallow complexion, tidal fever, and sour-smelling stool.
It is suitable to treat this with Muxiang Da'an Wan (Aucklandia Great Peace Pill).
(Qingjie San) See the section on "Treatment of Fever Syndromes" for the formula.
