Etiologies include Wind-Cold, Phlegm-Jue, Kidney Deficiency, and Qi-Blood Deficiency.
Since all Yang meridians converge on the head and face, pathogenic factors of Wind-Cold enter from the exterior and lodge in the collaterals, causing chills, headache, etc. There are also cases where emotional stagnation leads to phlegm retention in the chest and diaphragm, resulting in Jue-ni (rebellious qi) and headache; this is due to rebellious and obstructive qi rushing upward to the head. Headache caused by Kidney Deficiency occurs in individuals with weak constitutions whose innate endowment is deficient, leading to hyperactivity of Ministerial Fire and unrestrained desires, resulting in seminal emission and night sweats; this is a pattern of deficiency below and excess above. Furthermore, after childbirth, women suffer severe depletion of Qi and Blood, which also causes headache. As for True Headache (Zhen Toutong), it is characterized by pain throughout the entire brain, coldness of the hands and feet extending to the joints, a condition that is fatal. Additionally, there is a condition called Head Wind (Tou Feng), which is identical to headache but differs in terms of chronicity and depth. Generally, various pains differ, and treatment should be based on investigating the underlying cause.
Zhang Zhongjing stated: "Headache, stiffness and rigidity of the neck and occiput, aversion to cold, and heaviness of the body" constitute headache due to Cold Damage (Shanghan).
The *Neijing* states: "Headache, tinnitus, and dysfunction of the nine orifices" are generated by the intestines and stomach; this constitutes headache due to Qi Deficiency.
Zhu Danxi stated: "Headache is mostly governed by Phlegm," accompanied by dizziness, blurred vision, heaviness of the body, and a sensation of nausea; this constitutes Phlegm-Jue headache.
Dai Yuanli stated: The human head and face reside exclusively at the highest point. Only when pathogenic Wind enters due to deficiency and contends with healthy Qi, lodging and failing to disperse, does it manifest as lateral or frontal headache.
