Jiaogu bu kai (failure of the pubic symphysis to open), and chanmen bu bi (failure of the vaginal opening to close), are both caused by inherent weakness of Yuan Qi (Original Qi) and improper regulation during pregnancy, resulting in the inability of Blood and Qi to circulate and reach their destinations. Jiaogu bu kai indicates deficiency of Yin Qi. (Suitable formulas include Jia Wei Xiong Gui Tang [Modified Ligusticum and Angelica Decoction], Bu Zhong Yi Qi Tang [Tonify the Middle and Augment the Qi Decoction], Gu Ke San [Turtle Shell Powder], etc.) Chanmen bu bi indicates deficiency of Qi and Blood. (Suitable formula is Shi Quan Da Bu Tang [Great Tonifying Decoction for Ten Deficiencies].)
Fetus death in the abdomen often results from premature fright or violation of taboos, leading to difficult labor. After the amniotic sac has ruptured, lack of blood nourishment causes the fetus to wither and die. One must examine if the mother's tongue is cyanotic and black; if so, the fetus is dead, and purgation should be employed. (Suitable formulas include Ping Wei San [Calm the Stomach Powder] with Po Xiao [Mirabilite], Qian Jin Shen Zao Tang [Thousand Gold Divine Creation Decoction], Ban Xia Tang [Pinellia Decoction], etc.) The general principle is: for cold conditions, use hot agents to promote circulation; for heat conditions, use cool agents to promote circulation; for dryness, use slippery agents to moisten; for critical emergencies, use toxic agents to purge. Purging should involve agents like Po Xiao.
During late pregnancy, binding the fetus (Shu Tai) to make it slender and contracted facilitates easier delivery. (Suitable formulas include Shu Tai Wan [Fetus-Binding Pill], Da Sheng San [Generate Life Powder], Zhang Shi Fang [Master Zhang's Formula], Bao Sheng Wu You San [Protect Life Without Worry Powder], Shen Qin Wan [Divine Sleep Pill], etc.)
The general method for inducing labor involves: using slippery substances to unblock stagnation, bitter substances to drive away obstruction, and aromatic substances to open orifices and expel stasis. For Qi stagnation, promote Qi movement. If the amniotic sac ruptures prematurely and the condition is urgent, consolidate the Blood. (Suitable formulas include Fo Shou San [Buddha's Hand Powder], San He Ji Sheng Tang [Three Harmonies Generating Life Decoction], Hei Shen San [Black Divine Powder], E Jiao Chi Xiao Dou Tang [Donkey-Hide Gelatin and Red Adzuki Bean Decoction], Tu Nao Wan [Rabbit Brain Pill], Ru Shen San [Like God Powder], Huang Jin San [Golden Powder], etc.)
After delivery, drink one cup of hot Tong Bian (Child's Urine). Do not lie down immediately; instead, sit quietly with eyes closed. After a short while, go to bed in a supine position, not lateral. Keep knees upright and do not extend legs fully. Lean high against the headboard, with thick bedding. Seal all walls to prevent gaps, avoiding invasion by Zei Feng (Robber Wind). Apply vinegar to the nose, or use vinegar charcoal or burnt lacquerware. Additionally, press with hands from the heart area down to below the navel to prevent stagnation of E Lu (Lochia). Maintain this for three days to prevent Xue Yun (Blood Fainting) and Xue Ni (Blood Reversal). Regardless of abdominal pain or health status, take half a cup of Tong Bian mixed with wine, warm and taken five to seven times. Although wine promotes Blood circulation, do not consume too much, as it can induce fainting. Frequently consume small amounts of white rice porridge. After one month, consume small amounts of mutton and pig trotters. Continue to be cautious regarding speech, Seven Emotions (Qi, Joy, Worry, Thought, Sadness, Fear, Fright), temperature changes, hair combing, and foot washing, for a period of one hundred days. If Qi and Blood are inherently weak, regardless of time elapsed, otherwise symptoms such as limb, waist, and leg pain may occur, known as Ru Lao (Postpartum Asthenia), which is difficult to treat. At the time of birth, do not ask about the gender, lest speech leak Qi or love/hatred stir Qi, both causing disease. Do not sleep alone, lest虛惊 (Empty Fright) occur. Do not retract the tongue, lest Heart Qi be damaged. Do not rinse teeth, lest Blood Reversal occur. Only when Qi and Blood have stabilized should one resume daily activities. Violations at first are slight as autumn down, but resulting diseases are heavy as mountains. Can one not be vigilant? If the puerpera rests according to proper methods and internal organs harmonize, there will be no illness. Initially, take Hei Shen San and Si Wu Tang [Four Substances Decoction]. If there is high fever and headache, this indicates the Milk Meridian is about to flow. If the head and eyes feel unclear, it is Xue Yun (Blood Fainting); use Qing Hun San [Clear Soul Powder]. If food tastes bland, it indicates deficiency of Stomach Qi. (Suitable formulas are Liu Jun Zi Tang [Six Gentlemen Decoction] and Dang Gui Jian Zhong Tang [Angelica Build the Middle Decoction]. Chen Cangqi stated: Soak Ramie juice for the puerpera to drink; place Ramie under the pillow for women to stop blood fainting. For postpartum abdominal pain, place Ramie on the abdomen to stop pain.)
