A maiden of the Fan family, whose parents had both passed away, suffered from summer-heat. I treated her. One day, upon taking her pulse, I found it surging and large in both hands, particularly at the Chi (Kidney) positions, leading me to suspect the onset of menstruation. I asked the female servant if she had experienced any changes, and she replied that there were none. Believing the summer-heat was excessive, I prescribed Baihu Tang (White Tiger Decoction). By evening, the patient developed spasms and syncope, at which point it became clear that her menses had indeed arrived. Because the maiden was shy, I instructed her to keep it secret, which ultimately led to an untreatable condition and death.
In the Renxu year of the Tongzhi era, while seeking refuge in the countryside, I was called to treat a woman suffering from an epidemic disease. Her menses began eight days after the onset of illness. Four days later, I was invited to treat her again. I promptly applied the previous method suitable for menstrual illness. However, due to the long distance from the village to the market, the medicine had not been ingested before her menses stopped. This resulted in coma, muteness, spasms, and syncope, leading to her death.
A married woman of the Mao family, who was eight months pregnant, contracted latent summer-heat after the Frost Descent solar term. At dusk, she experienced alternating chills and fever resembling malaria but not quite so. Persistent vomiting indicated a disorder of the Upper and Middle Jiao; the obstruction of Yang Qi was caused by the prior physician's prohibition of Shi Gao (Gypsum Fibrosum). Although Houpo (Magnolia Bark) and Huoxiang (Patchouli) were used frequently, the vomiting persisted. Later, I employed Yu Jia Yan’s (Yu Chang) method of advancing and retreating. After one dose, the vomiting ceased, and I took my leave. Since the pathology was extreme at the upper level, it inevitably rebounded to the lower level. Childbirth itself is difficult; the family repeatedly requested treatment using methods to calm the fetus and clear summer-heat. She delivered within a month post-treatment, and both mother and child were well, except for scant lochia. I reasoned that after a month of illness, attempting to promote blood circulation now would be like taxing starving people; one must add capital to gain profit (i.e., tonify to facilitate recovery). Thus, I prescribed Danshen (Salvia Miltiorrhiza) 8 qian, Danggui (Angelica Sinensis) 3 qian, and Chuanxiong (Ligusticum) 2 qian, adding Shayuanzi (Astragalus Complanatus) 1 liang to replace Dihuang (Rehmannia). The menstrual blood flowed heavily. After ten doses, the lochia cleared. Dusk saw the return of chills and fever. I stated that extremity at the lower level implies a rebound to the upper level. I used Bohe (Mint) and Huashi (Talcum) to release the Lung with acrid-cool properties, resulting in a cure.
My family friend, Brother Zhang Lufeng, possessed high medical skill. Whenever relatives or friends faced difficult cases, his prescriptions brought immediate recovery. His unmarried daughter suffered from warm-pathogen invasion for many days without improvement. Upon examination, her tongue was black and dry, her spirit was dull, and her pulse was stagnant and large. I diagnosed pathogen invasion into the Pericardium and recommended Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction). Mr. Zhang said they had already taken it, perhaps the dosage was too light, and proposed increasing the dose, but he refused. Upon further examination and questioning the servant about whether the patient had menstruated during the illness, I was told it occurred over ten days ago, before taking the owner's medicine. I realized he would certainly not use methods to regulate menstruation. I interpreted the black tongue as depletion of body fluids and drying of Kidney Water; deafness as failure of water energy to ascend; confusion as essence failing to connect with the Heart; inability to move legs and pain upon clothing touching muscles as deficiency of Blood aspect. I modified Fumai Tang (Restore the Pulse Decoction) from the Lower Jiao chapter of Wu Ju Tong’s *Wenbing Tiaobian* (Systematic Differentiation of Warm Diseases), using up to 8 qian of Shudi (Prepared Rehmannia) and 6 qian of Zhigancao (Honey-fried Licorice). Mr. Zhang disliked the heavy prescription. I cited the book stating Licorice should not exceed 1 liang and had cured many with this approach. He eventually administered this decoction daily. Within ten days, she fully recovered. I gifted him a copy of *Wenbing Tiaobian*, printed in Beijing. Wu Jutong was close friends with my senior relative Mr. Hu Shuiyun. Now, Hu Xin Heng, magistrate of Xi Zao, is Mr. Hu Shuiyun’s son. Thanks to his repeated gifts, Ningbo has reprinted it. Later, Mr. Zhang mentioned using Da Ding Feng Zhu (Great Calming Wind Pearl) from the book to successfully treat metrorrhagia. This daughter married into the Yangjia Long Sun Bao shop. In July, she suffered a miscarriage due to summer-dampness, with no vaginal discharge. Mr. Zhang treated her initially and then invited me. His prescription was substantial, using 7 qian of Danggui. I argued that since the nature is warm, it should be replaced with 1 liang of Danshen. As the miscarriage was forced down by summer-heat, cool herbs are necessary. Without cooling, heat would enter the Blood Chamber. I added Danpi (Moutan Cortex), Zhizi (Gardenia Fruit), Liuyisan (Six-one Powder), and Mutong (Akebia Caulis). She recovered suddenly but developed wind-warmth rashes. In Shaoxing, this is called "Ma"; in Suzhou, "Sha"; Ye Tianshi called it Lung Pathogen. In winter/spring, use warm-pathogen methods; in summer/autumn, use summer-dampness methods, all aiming to cool the Lung and dispel wind. The main concern is bloating and constipation. If diarrhea occurs, no medicine is needed; even remedies like cherry pits or willow branches are useless. In March of the Bingyin year of Tongzhi, my whole family contracted "Ma," but it was mild, and we took no medicine. Two daughters had mild illness with menstruation during the sickness, which I ignored. One remained healthy; the other’s menses ceased. Two months later, she developed asthma and facial swelling, sitting day and night. I questioned the cause. After administering 1 qian of Heixi Dan (Black Lead Pill), her breathing stabilized, swelling subsided, but she had watery diarrhea. Consulting *Zhangshi Yitong* (Medical Secrets of Master Zhang), this is Liver-Kidney Asthma; watery diarrhea indicates incurability. Various treatments failed. These two daughters were exceptionally intelligent, filial, and kind. One named Yimingzhi, the other Quan Zhi. Quan Zhi married the next year and cut her arm flesh to save me when I was critically ill. The following year, she fell ill and died, revealing the scar only at her deathbed. There were three sisters; the eldest, Yuan Zhi, also cut her arm to save me at age fifteen. She died giving birth at twenty-one. I reflect on Cao E’s filial piety at fourteen, lasting eternally. Now, a fifteen-year-old dies quickly after showing filial piety. Questioning the Qicheng Magistrate’s lot, it showed that every sip and bite is predestined, as is lifespan. Observing the Tang dynasty civil service exams linking Buddhism to the Ghost Mansion star, I realized the three teachings—Confucianism for humans, Buddhism for spirits, Daoism for monsters—are indispensable. Fate is fixed; even Cang Gong and Bian Que could not alter it.
Chen Yongmao from West Gate recognized good and bad medicine. When I treated women in his family, illness resolved when menses arrived. Now old and sick, I stopped treating them. Last autumn, someone had summer-heat with menses. Remembering his advice to guide physicians to use blood-draining methods for heat, I was confused and still used Houpo, Zhishi, and Gualou, plus fried Shaoyao, leading to non-recovery. ###Dysentery###
I treated Shen Xia Chunqing from Qingliang Bridge with pure bloody dysentery. Tongue yellow, pulse string-large. Prescribed Danggui, Chishao, Huangqin, Gancao, Muxiang, Chuanlian, Lianzi, fried Xiao Hui, Jiuhanyu, Baiziren. After 2-3 doses, roundworms appeared in the stool. Removed Gancao and Xiao Hui, added Chuanjiaotan (Charred Sichuan Pepper), Wumei (Smoked Plum), Tai Wuyao. After 2 doses, feces appeared in blood. Finally cured using Liver methods.
Mao’s wife from Jinlin Bridge had dysentery: yellow tongue, thirst, pain around the navel. Used Spleen Dysentery method: Xingren, Houpo, Zhike, Yinhua Tan, Xianglian Wan, Chenpi. On day 7, pulse deep-solid. Used Zhijun (Da Huang) and Zhishi to purge. The family slept at Shen hour, taking medicine the next day. On day 8, after purgatives, severe lower abdominal pain. I said this transferred to the Liver channel due to delayed medication. Prescribed Danggui Huangqin Tang combined with Lingliao San, adding Baiziren and fried Xiao Huixiang. Cured after another 7 days. First Spleen, then Liver.
Qiu from Bao You Bridge had dysentery for 5 days. Pulse string-thin, alternating chills/fever, hypochondriac/abdominal pain. Previous prescriptions were standard dysentery formulas, hence ineffective. Used Xiao Chaihu Tang combined with Danggui Huangqin Tang and Xianglian Wan. Cured in 7 days.
Liang from Dongchang Fang likely had Liver Dysentery. Standard formulas ignoring Liver/Spleen differentiation led to hiccups and snowflake-like tongue coating. Pulse string-small like a thread indicates severe Liver Yin damage. Used Shudi, Danggui, Shaoyao, Ejiao, Huaishanyao to tonify Liver; Suosha, Muli to descend rebellious Qi; Xuanfuhua to stop hiccups. Adjusted tonification over 10+ days until cured.
Chen Buyun, a clerk from Shanyin, had prolonged dysentery treated with draining/purging drugs. Snowflake tongue, hiccups, anorexia, Chi pulse large, others string-small. Prolonged dysentery damaged Liver/Kidney. Used Shudi, Guiban, Danggui, Shaoyao, Ejiao to tonify Liver/Kidney; Jiang, Wuweizi, Zhigancao, Chenpi to harmonize Stomach (Stomach as gate of Kidney, imitating Weiguan Jian). Added Dou Dou to stop hiccups. Cured in 10+ doses.
Hotel owner from Bao You Bridge, male, 20s. Dysentery for 1 month. Breath faint, pulse deep-empty-small, lying on side afraid to move. Movement causes anal leakage of thin water. Tongue bright red, clean. Severe Kidney Yin damage due to excessive attacking. Used Shudi, Rougui, Wuweizi, Guiban, plus Paojiang, Jingmi, Chishizhi (Taohua Tang). Multiple doses until cured.
My ancestor moved from Jiangnan to Shaoxing 200 years ago, living near the Liu family. Uncle Huai Chuan, a multi-generational friend, suffered from intermittent dysentery for 4 years, 4-5 times daily, difficult passage, passing gas followed by sauce-colored purple stool. During remission, stool was tube-like. Consulted me. I advised treating around Spring Equinox. At that time, pulse string-stagnant-large. Damp-heat remained, damaging Qi. Used Hanfangji, Jiaomaoshu, Chuanlian, Fuling, Zexie to drain damp-heat; Guangmuxiang, Suosha, Chenpi to move Qi; Wendangshen 1 liang, soaked in Shengma decoction to raise Qi. 7 doses, disease vanished. Since then, many intermittent dysentery cases in town/country cured around Spring Equinox. Zhang from Songlin, 40s, 2 years intermittent dysentery, Liver involvement. Used Danggui Huangqin Tang + Xianglian Wan + Zhixiangfu, Suosha to soothe Liver. Cured.
Brother Xue Si from Songlin, official in Jiangxi, had intermittent dysentery for 2 years. Returned home in Autumn. I advised Spring Equinox treatment, suggesting pills for slow action. Formula: Chuanlian 1 liang, Taiwuyao 1.5 liang, Jiaomaoshu 3 liang, Guangmuxiang 1.5 liang, Zexie 1.5 liang, Dan Huangqin 1.5 liang. Powdered, pilled with rice drink. 5 qian per dose, total 7 liang consumed. Cured. Red in stool indicated damp-heat in Liver.
Pregnant women with dysentery are hard to treat. Ancients had Five Prohibitions and Three Examinations. Three Examinations: check body heat, fetal movement, waist pain. Five Prohibitions: 1. Banlang, Houpo break Qi (Qi broken leads to abortion); 2. Zhijun breaks Blood (Blood broken leads to abortion); 3. Huashi, Tongcao open orifices (Orifices open leads to abortion); 4. Fuling, Zexie drain water (Draining water damages Yin, fetus unsafe); 5. Renshen, Shengma block Qi (Dysentery stagnates, fetus hits Heart). Method: Cool Blood, benefit Qi. Zhou from Jitou Mountain, 7 months pregnant, dysentery, Gaobu doctors failed. I used previous method, cured in 2 doses. Doctors said it wasn't a dysentery formula. Indeed, it treats pregnancy dysentery specifically. Zhou Qixiang’s two daughters-in-law, pregnant, simultaneous dysentery. Cured in 2 doses each. Friend Zhu Gutang, poor scholar, wife 8 months pregnant, dysentery. Used standard drugs (Houpo, Zhike, Gualou, Huren) not following ancient pregnancy dysentery rules. Invited me at dusk. Severe waist/abdominal pain, impending labor. Pulse floating-large, tongue clean. Fetal movement and labor will injure mother and child. Recalled *Jingyue Quanshu* method for pregnancy dysentery impending labor: Danggui Bu Xue method. Mi Zhi Huangqi 1 liang, Chao Danggui 3 qian, Chao Nuomi 1 he. Pharmacy nearby, fanned fire to boil fast. Swallowed, pain stopped after an hour. Guan/Chi pulses still large, feared labor at dawn. Gave another dose for dawn. Next noon, Zhu said doctor is immortal, labor pains started at dawn, stopped after swallowing, took another dose. I said stop when illness responds; overdose blocks. Dysentery won't heal before birth. Post-partum severe watery diarrhea. I said dysentery turning to diarrhea means healing. Mother needs diagnosis. Gave Wuling San, cured.
Zhou from Dayun Bridge, 23 years old. Front door opposite Ren family back door. Had dysentery, disliked Ren’s aggressive treatment, hired Yao. Yao used Danggui, Baishao, Huangqin indiscriminately for 10 days, no cure. Reluctantly hired Ren for Spleen method. 10 more days, worse. Finally hired me. Early September. Pulse string-large, white floating tongue, gray face, sore throat, thirst, frequent discharge. Disease in Upper Jiao, Lung/Large Intestine exterior-interior. Used Lung Damp-Heat method. Sore throat/black tongue resolved, but dysentery persisted. Daily visits, couldn't find key. One day critical, bedridden, mass in lower abdomen pushed up, liked thick cotton pad pressed firmly, stool passed abundantly. Others said scrotum retracted into abdomen. Room full of women, didn't avoid strangers. Saw decoration like bride beside bed, asked who. Mother said new wife, married March, sick in shop Aug 6, returned Aug 7, worsened Aug 8. I accompanied day/night. Said you probably didn't accompany on Aug 7 night. No answer. Realized lack of caution caused scrotal retraction, pathogen entered Liver.
