Throughout medical history, treating *Lao Zhai* (chronic consumptive disease, similar to pulmonary tuberculosis) has often been effective using Shenling Baizhu San (Ginseng, Poria, and Atractylodes Macrocephala Powder), but one must understand the auxiliary and guiding herbs, the dosage weight, and modifications according to the season. If Sini Tang (Four Substances Decoction) is used exclusively, eight or nine out of ten will die; this is likely due to adhering rigidly to Zhu Danxi's words without understanding flexibility. *Lao Zhai* can be caused by excessive labor and hardship, or by sexual excess; it cannot be treated uniformly. Some use Li Dongyuan's method of tonifying the middle [Jiao], while others use Zhu Danxi's formulas for nourishing yin; treatment should be applied according to the specific syndrome. However, I have used the method of tonifying the middle more frequently, whereas the method of nourishing yin was used in only two or three out of a hundred cases. (Wu Huangchi)
I often observe that in treating *Lao Zhai*, people who specialize in using kidney-tonifying herbs are mostly mistaken. This disease should primarily be treated by cultivating and tonifying the spleen and stomach. When the spleen and stomach are robust, appetite improves; when food is ingested, essence and blood are transformed; over time, the kidney naturally becomes solid. To tonify the kidney without first tonifying the spleen and stomach is to ask how herbs such as Zhimu (Anemarrhena Rhizome), Huangbai (Phellodendron Bark), Dihuang (Rehmannia Root), and Guiban (Tortoise Shell) can generate essence and marrow. Such a treatment approach can be described as abandoning the root to pursue the branch. ○ Fire and Qi share the same substance but different names. When Qi flows smoothly without obstruction, it is called Qi; if it stagnates and fails to extend, or if Blood deficiency fails to anchor Qi, accumulation and agitation occur, transforming into Fire. Observing syndromes of Phlegm-Fire: rapid breathing and fever occur. If doctors use cold and cool herbs excessively, Yuan Qi gradually declines. The cessation of wheezing and cooling of the body are mistaken for signs of impending recovery. After five or seven days, sudden collapse of Yuan Qi leads to death. The cessation of wheezing indicates that Yuan Qi has sunk and cannot surge upward; the cooling of the body indicates that Yuan Qi has exhausted and cannot circulate to the skin and muscles. In such cases, one must guard against changes and urgently use herbs like Renshen (Ginseng Root) to tonify Yuan Qi for rescue; one must not lightly declare recovery. (Yu Fushan)
Illnesses resembling wind-cold coughs, if mistakenly treated as exterior pathogens with dispersing and cooling methods, will inevitably develop into *Lao Zhai*. Because Lung deficiency fails to protect the exterior and hair/skin, leading to wind-cold cough, one should use Wenfei Tang (Warm the Lung Decoction), focusing on consolidating Lung Qi. If cold herbs are used, Lung Qi becomes even more deficient; Lung deficiency fails to generate Kidney Water; when Kidney Water dries up, Ministerial Fire flares up; when Ministerial Fire flares up, the bone marrow is steamed dry, which is the origin of consumptive disease. There are also consumptive diseases without diarrhea, indicating that Yin deficiency has led to the withering of bone marrow. Those with a good appetite have excessive fire in the Stomach; it is not that eating more food can suppress the fire. ○ In deficiency damage diseases, Mingmen (Gate of Life) fire is hyperactive, and Kidney Water is insufficient. Yangming transforms into dry fire. Liver Qi is equivalent to Stomach Qi; Wood is dry and Earth is parched. Heart Fire flares upward, Metal receives no nourishment, and Water has no source. When the Five Fires blaze intensely, using Zhimu (Anemarrhena Rhizome) and Huangbai (Phellodendron Bark) to nourish Yin and descend Fire is like pouring a cup of water onto a red-hot dry pot; it agitates the fire, causing immediate cracking. If the pulse is slow and the Stomach Qi is not exhausted, regulation is still possible. Use Sijunzi Tang (Four Gentlemen Decoction) plus Shanyao (Dioscorea Rhizome) to guide the action into the Spleen Meridian, solely tonifying Spleen Yin. When the Spleen is robust, Earth generates Metal, Metal generates Water, Water rises, and Fire descends naturally; this is the "treating the third organ away" method. If the pulse shows tightness-rapid, shortness-rapid, or thinness-rapid, the condition is untreatable. (Zhou Shenqi)
Chronic deficiency damage diseases all belong to Spleen deficiency. Spleen deficiency causes the Lung to suffer first. Lung disease fails to govern the whole body; Spleen disease renders the four limbs useless. One must focus on preserving Yuan Qi and always treat from the perspective of the Spleen and Stomach. Do not use Blood-tonifying herbs. Even if there is Fire, do not clear it; even if there is Phlegm, do not resolve it. ○ In damage diseases, if all six pulses are rapid, accompanied by hoarseness, mouth ulcers, and day-and-night fever, it is critical. The *Classic* states: "Rapid pulse indicates Spleen Qi deficiency and True Yin deficiency." This is the third stage (critical phase). If Sijunzi Tang (Four Gentlemen Decoction) and Baoyuan Tang (Preserve the Origin Decoction) are ineffective, switch to Sijunzi Tang plus Huangqi (Astragalus Root), Shanyao (Dioscorea Rhizome), Lianrou (Lotus Seed Flesh), Baishao (White Peony Root), Wuweizi (Schisandra Fruit), and Maidong (Ophiopogon Tuber). Discard the first decoction and consume the second and third decoctions. This is a secret method for nourishing Spleen Yin. After taking it for over ten days, fever gradually subsides, and mouth ulcers heal. If made into pills, such as Shenling Baizhu San (Ginseng, Poria, and Atractylodes Macrocephala Pill), discard the first decoction, dry and powder it, and form pills with old rice paste. Discarding the first decoction removes dryness, resulting in a sweet and bland flavor; blandness nourishes Stomach Qi, sweetness nourishes Spleen Yin. This is transmitted wisdom among masters; do not neglect it. (Hu Shenrou)
The term "Xu Lao" (Deficiency Consumption) is generally used loosely by the world, unaware that syndromes differ and treatments may be contradictory. I heard Shenrou teach: Damage diseases transmit from top to bottom; Consumption diseases develop from bottom to top. Damage diseases transmitting to the Spleen and then Kidney are untreatable; Consumption diseases transmitting to the Spleen and then Lung are untreatable. Using consumption treatments for damage often causes diarrhea; using damage treatments for consumption inevitably causes panting. With such confusion between clear and muddy paths, treating them indistinctly as mere weakness is akin to heading south while intending to go north.
Zhu Danxi established the theory of Ministerial Fire, relying solely on Siwu Tang (Four Substances Decoction) to nourish Yin, obscuring the meaning of Yin and Yang for a long time. Xue Lizai emerged, and medicine underwent one transformation; Zhang Jingyue (Shenzhai) emerged again, and medicine underwent another transformation; finally, Hu Shenrou collected the methods of sages, dividing deficiency damage and consumption into two categories, bringing brilliance to the medical field. Furthermore, the *Essay on Original Qi* divides Yin and Yang based on Pre-heaven and Post-heaven, establishing treatments accordingly. I believe that while Pre-heaven can be damaged, Post-heaven damage is necessary to cause illness. Treating Pre-heaven is essentially treating Post-heaven; how can one abandon Post-heaven to treat Pre-heaven? (Shi Ruizhang)
In all consumption diseases, if there is Phlegm-Fire, panting, and coughing in the Upper Jiao preventing lying down, combined with loose stools in the Lower Jiao, it is mostly untreatable. Using cold herbs to moisten the Lung will cause Middle Jiao Stomach Qi to sink and worsen diarrhea; using sweet-warm herbs to tonify the Spleen will make Upper Jiao Lung Qi drier and worsen coughing. Treating the upper hinders the lower, and treating the lower hinders the upper; constrained in this way, medication is difficult. However, if symptoms are isolated—panting/coughing with hard stools, or loose stools without panting/coughing—it is treatable. Why? Hard stools tolerate cold herbs; absence of panting/coughing tolerates warm tonics. Doctors can apply their skills, and medicines can exert their effects. Also, in treating Phlegm-Fire coughs, symptoms stabilize when awake because movement aligns with Fire and does not oppose it; symptoms worsen when sleeping because stillness opposes Fire. In all consumption diseases, trial administration of Renshen (Ginseng Root) indicates treatability if tolerated; if coughing and panting worsen after taking Ginseng, it is untreatable due to excessive Upper Jiao Fire. (Wang Shuangquan)
Wang Jiezhai stated: For those whose Lung and Kidney True Yin are damaged by excessive alcohol and sex, Renshen (Ginseng Root) and Huangqi (Astragalus Root) must not be taken excessively; excessive intake leads to death, fearing that Yang prosperity consumes Yin. Since this saying spread, practitioners treating Yin-deficiency coughs view Renshen and Huangqi as arsenic, regarding Zhimu (Anemarrhena Rhizome) and Huangbai (Phellodendron Bark) as spiritual elixirs, resulting in almost no survivors, which is truly tragic. The disease originates from sexual exhaustion, damaging True Yin, causing Fire to flare and injure Metal (Lung), leading to cough. One should first use Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) to tonify True Yin, allowing Water to rise and Fire to descend, followed by Renshen (Ginseng Root) and Huangqi (Astragalus Root) to benefit the Mother of the Lung (Earth/Metal relationship or Tonifying Source), promoting Metal-Water mutual generation, facilitating recovery. Practitioners using cold herbs are negligible. Among those who know to use Renshen and Huangqi, many fail to first strengthen Water to control Fire, instead hastily administering Renshen and Huangqi to tonify Yang, making Yang even stronger and injuring Metal further. This is a failure to understand the sequence. (*Zhao's Medical Canon*)
When True Yin deficiency causes fever, six or seven out of ten cases resemble exterior patterns. Fire rebelling upward causes slight head distension and pain; Fire heat congesting the Lung causes occasional nasal congestion; Yin deficiency with Yang sinking inward causes aversion to cold with shivering; Yin deficiency with Yang having no attachment causes floating to the surface and fever. However, fever occurs specifically in the afternoon, starting with chills then heat; at Yin-Mao hours (5-9 AM), night sweats occur, and the body cools. Some have heat without chills, but must show Kidney deficiency signs, such as red lips and malar flush, thirst, irritability, and rapid or empty-weak pulses. Large doses of Yin-nourishing herbs, such as Baoyin Tang (Protect Yin Decoction) or Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia), are appropriate. Mistaking this for an exterior pattern and inducing sweating leads to profuse sweating and a onset of various deficiencies. ○ Insufficient Yin Water allows Fire to insult the Lung; fluids condense turbidly, failing to generate Blood and instead generating Phlegm. One should use moistening agents to nourish Yin, allowing rebellious Fire to return to its home, clearing Phlegm naturally. ○ In deficiency consumption, patients who can tolerate Renshen (Ginseng Root) and Huangqi (Astragalus Root) have no Heat in the Lung; the Lung pulse is empty upon pressure and certainly not rapid. Thus, following the principle that robust Earth generates Metal, one should not rigidly adhere to Lung-moistening theories; ancients used these methods successfully. Now, however, Fire scorches Metal causing cough, steams fluids into Phlegm, and Ministerial/Emperor Fire is hyperactive causing Blood to rebel upward. Yet, some still cite "Yang generates Yin, deficiency Fire can be tonified," recklessly using Renshen and Huangqi, causing Yang Fire to intensify and Metal to suffer further injury. Hence Haogu warned "Heat in the Lung harms the Lung," and Jiezhai warned "Death follows Ginseng ingestion." ○ The *Classic* states: "The Kidney governs Water, receiving and storing the essence of the five Zang and six Fu organs." Essence is stored here, Qi transformation occurs here. Essence is Water within Yin; Qi is Fire within Yin. Thus, the Water and Fire of Mingmen (Gate of Life) are the sources of transformation for the twelve organs. Fire fears decline less than Water fears scarcity. Therefore, Baoyin Tang (Protect Yin Decoction), Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia), and Zuogui Wan (Restore the Left Pill) are sweet-cold herbs that nourish Water and add Essence, tonifying Yin to match Yang. This aligns with Wang Tai Pu's "Strengthen the Master of Water to control Solar Flare" and Danxi's "Nourish Yin so Fire descends naturally." It is like adding oil to a dying lamp flame; the flame stabilizes. However, large doses must be taken for a long time, as Yin cannot be rapidly tonified. ○ The *Jin Gui Yao Lue* discusses treating the Liver by tonifying the Spleen; this applies to Liver Yang deficiency, not Liver Yin deficiency with Fire hyperactivity. Liver Yang deficiency fails to ascend; if it ascends, the Stomach lacks ascending Qi, the Spleen lacks transportation power, Water is not controlled by Earth, and Kidney Yin-Cold rises to suppress Heart Yang. The whole body experiences Yin excess and Yang deficiency, purely descending. Lung Metal Qi prevails, further harming Liver Yang's generating Qi. One must tonify Earth Yang to control Kidney Yin-Cold. Then Heart Yang, unchecked by Water, becomes strong; strong Fire prevents Lung Metal Yin from circulating, stopping the descent of Yin and allowing the leftward ascent of generating Qi, thus Liver Wood Yang flourishes. Ancient formulas like Xiaoyao San (Free and Easy Wanderer Powder) treat Wood stagnation in Earth to promote Yang Qi, applicable to Liver Yang deficiency. Later generations used it for Liver Yin deficiency with Fire hyperactivity, causing excessive ascending without descending, leading to syncope. Can Yin and Yang not be clearly distinguished? For Liver Yin deficiency with Fire hyperactivity: Blood deficiency requires nourishing Water (tonifying the Mother); Fire hyperactivity requires bitter drainage (draining the Son); Qi rebellion requires descending Qi (using Metal to control Wood). These are opposite to the "treat Liver by tonifying Spleen" method; how can they be mixed? (*Collected Discussions of Wu Physicians*)
Regarding damage syndromes, Yue Man stated the meaning of upper damage following Yang and lower damage following Yin. For cases unresponsive to acupuncture and drugs, sweet herbs are used for regulation. The *Jin Gui Yao Lue* followed this by establishing Jianzhong Tang (Establish the Middle Decoction) to establish Middle Qi, increasing diet, strengthening Spleen and Stomach, generating Blood and Essence, and restoring Yuan Yuan deficiency. Only the sweet taste of Grain (Earth) is used; sour, acrid, salty, and bitter are excluded. However, this only treats Upper Jiao Yang deficiency damage, insufficient to cultivate Lower Jiao True Yin. The author extended this to Sancai Tang (Three Powers Decoction), Guben Wan (Solidify the Root Pill), Tianzhen Wan (Heavenly Innocence Pill), Dazao Wan (Great Creation Pill), Guizhi Longgu Muli Tang (Cinnamon Twig, Dragon Bone, and Oyster Shell Decoction), Maigui Jiajian Tang (Decoction for Reviving Pulse), and other securing formulas, along with balancing and tonifying the Three Yin meridians, treating five Zang deficiencies, assisting in damage syndromes. Even Li Dongyuan and Zhu Danxi, for stubborn damage, prioritized Renshen (Ginseng Root) and Baizhu (Atractylodes Rhizome), believing that substantial Essence-Blood is hard to restore, so urgently cultivating formless Qi is key. This expands upon Zhang Zhongjing's Jianzhong formulas. Chronic unrecovered deficiency is called Damage; extreme unrecovered damage is called Consumption; these three occur sequentially. Generally, Qi injured by mental/physical fatigue is treated via Upper/Middle Jiao methods: sweet-cool herbs nourish Lung/Stomach clear fluids; soft agents nourish Heart/Spleen Ying-Nutrient fluids; or sweet-warm aromatics establish the Middle Palace. Essence injured by sexual excess is treated via Lower Jiao and Eight Extraordinary Vessels; one must distinguish filling Essence-Blood from tonifying Qi-Energy, and warming Fire from nourishing Yin, or quietly gathering Ren Yin and warmly regulating Odd-Yang. If caused by dysregulation of other conditions spreading, identify the root cause and regulate, distinguishing constitution Yin/Yang as the key, considering Upper/Middle/Lower manifestations, and judging life/death by transmission to Pre/Post-heaven. ○ The *Classic* says: "For consumption, use warmth." Labor causes physical shaking, injuring Yang first. Here, "warmth" means gentle nourishment, not hot/acrid advancement. Chronic unrecovered labor-damage is Damage. The *Inner Classic* states: "For damage, benefit it." Benefit means tonification. All tonics have warm Qi and sweet flavor, cultivating the initial generating Yang; this is the main rule for treating consumption-damage. (*Guide to Clinical Practice*)
Deficiency consumption without Fire is named Deficiency-Cold; with Fire is named Deficiency-Fire. Though both are "deficiency," Deficiency-Cold and Deficiency-Fire are worlds apart. Treat Deficiency-Cold with warm tonification, avoiding Yin nourishment; treat Deficiency-Fire with Yin nourishment, avoiding warm tonification. However, Post-heaven tangible pathogenic factors are easier to treat; Pre-heaven intangible pathogenic factors are harder. For Pre-heaven insufficiency, distinguish True Yang deficiency from True Yin deficiency. True Yang deficiency is Yang emptiness without Fire; tonify Yang. True Yin deficiency is Yin emptiness with Fire hyperactivity; tonify Yin. Human Yin-Yang Water-Fire, balanced leads to life, imbalanced leads to disease, extreme imbalance leads to death. Yang deficiency leads to Yin excess; Yin excess further depletes Yang; if Yang perishes, solitary Yin perishes with it. Yin deficiency leads to Yang excess; Yang excess further depletes Yin; if Yin perishes, solitary Yang perishes with it. If True Yin is insufficient, taking Yin-nourishing herbs turns it into Deficiency-Cold, while warm tonics turn it into Deficiency-Fire; this is a death syndrome where both Yin Water and Yang Fire are exhausted, intolerant of Yin nourishment. If True Yang is insufficient, warm tonics turn it into Deficiency-Fire, while Yin nourishing herbs turn it into Deficiency-Cold; this is a death syndrome where both Yang Fire and Yin Water are depleted, intolerant of warm tonification. If Deficiency-Cold treated with warm tonics remains Deficiency-Cold until death, this is "Yin without Yang, solitary Yin cannot grow." If Deficiency-Fire treated with Yin nourishment remains Deficiency-Fire until death, this is "Yang without Yin, solitary Yang cannot generate." These are Pre-heaven Yin-Yang insufficiency and Water-Fire excess deficiency consumptions. Regarding Post-heaven consumption-damage, differences exist in Essence, Blood, and Qi, but fundamentally fall into Deficiency-Cold and Deficiency-Fire. Everyone knows Blood deficiency with Fire; everyone ignores Qi deficiency with Fire. Qi deficiency without Fire requires warm tonification of Qi; Qi deficiency with Fire requires adding cooling herbs to Qi tonics. Blood deficiency without Fire requires Blood tonification; Blood deficiency with Fire requires adding Fire-clearing herbs to Yin tonics. The *Classic* says: "Yin deficiency generates internal heat." Treatment should "strengthen the master of Water to control Solar Flare," not implying Deficiency-Cold can use warm tonics. Dongyuan said: "Deficiency Fire can be tonified with Renshen (Ginseng Root) and Huangqi (Astragalus Root)." This refers to Post-heaven dietary/labor fatigue causing deficient Yang fever, not Pre-heaven Kidney deficiency Fire. People misinterpret the *Classic*'s "For consumption, use warmth" and "For form insufficiency, warm with Qi," mistaking "warmth" for heat. "Warm with Qi" means gentle and harmonious nourishment to cultivate Yuan Qi, not using hot/acrid herbs. "Tonify insufficiency with flavor" means using Yin-nourishing substances to tonify Yin Essence, not rich/fatty foods. (*Diagnosis and Treatment Based on Etiology and Symptoms*)
All deficiency consumption diseases often feature fever; one must differentiate internal/external causes, Yin/Yang pulses, and timing to determine treatment. Using generic antipyretics punishes the innocent, leading to prolonged heat, dried Blood, and exhausted fluids, resulting in death; this is the doctor's fault. ○ All deficiency consumption diseases often involve Blood loss without sweat. If mistaken for Yang excess and sweating is forced, Blood is disturbed; this is "Lower Cold, Upper Exhaustion," killing the patient. ○ All deficiency consumption diseases must guard against Spleen Qi sinking. Excessive use of cold herbs causing clear watery diarrhea is the doctor's fault. ○ When treating consumption fever, act early while Stomach Qi can withstand medication. Urgently use potent formulas with Renshen (Ginseng Root) to guide Blood, open capsules, reduce fever, and move stasis, saving life. Delay allows Stomach Qi to weaken and emaciation to set; even good methods become useless. This is not the doctor's fault if they strictly follow rules without adapting to circumstances; silence and hoping for luck are the real faults. ○ Pediatric "Five Wasting-Temperature" corresponds to adult "Five Consumptions." Pediatricians know standard formulas but ignore the complexity of adult consumption. Early stages are treatable; Stomach deficiency leads to death. Herbs like Longdancao (Gentiana Root), Lvhua (Aloe), and Hulian (Coptis Rhizome) are extremely bitter and cold, unsuitable for children. Large adult formulas are similar; indiscriminate use of bitter-cold herbs for Five Zang deficiency/consumption is unforgivable. ○ Formerly rich then poor is called Tuoying (Depleted Camp); formerly noble then lowly is called Shijing (Lost Essence). Tuoying and Shijing lead to Guan Ge (Obstruction-Rebellion) or deficiency consumption. Gradually treat Qi knotting and Blood coagulation until flow is restored; success takes time. Doctors using tonic/clearing methods incorrectly result in emaciation and spirit loss. This is not just medical error, but negligence causing delay. (*Medical Door Laws*)
Pulse Diagnosis
For a healthy man, a large pulse indicates Consumption; an extremely empty pulse also indicates Consumption. (*Jin Gui Yao Lue*)
In chronic illness with flesh and muscle depletion, even if the pulse seems harmonious, it is mostly untreatable. The *Classic* says: "When flesh and muscle are depleted, even if the nine pulses are regulated, death ensues." (*Feng Shi Jin Nang*)
Consumption pulses are either wiry or large. Large and forceless indicates Yang deficiency; wiry and forceless indicates Yin deficiency. Large is easier to treat as Blood-Qi is not yet depleted and can be gathered and corrected; wiry is difficult as Blood-Qi is consumed, making tonification hard. Fine-rapid and soggy-scattered pulses are untreatable. (*Collected Compendium of Syndrome and Treatment*)
Generally, fine-rapid indicates Kidney deficiency; wiry-rapid indicates Liver deficiency; short-rapid indicates Lung deficiency; scattered-rapid indicates Heart deficiency. These are heavy disease pulses; presence of Stomach Qi means life, absence means death. (Zhou Shenzhai)
Deficiency damage pulses, regardless of floating/sinking, large/small, become promising if gradually slowing. Extreme wiriness means severe disease; extreme rapidity means critical disease. Wiry-fine with added tightness-rapid means no hope. (Zhang Jingyue)
Consumption pulses: forceful is treatable; forceless is difficult. Large and forceful indicates Fire; fine-rapid and forceless indicates no Fire. With Fire, although Yuan Qi is damaged, there is a foundation to work with; without Fire, Yuan Qi collapses, foundation is gone, and skill is useless. (Yu Fushan)
Chronic illness: pulses large/small, surging/fine, deep/floating, wiry/sliding, or cun floating/ch chi sunken, or chi floating/cun sunken. Any pathological pulse is relatively treatable; if floating/middle/deep are all harmonious with fatigue, death is certain. If complexion is lustrous, spirit is exposed externally, and cure is difficult. (Hu Shenrou)
Why does a fine-weak pulse become weaker after taking Renshen (Ginseng Root) and Huangqi (Astragalus Root)? Likely due to excessive Fire consuming Yuan Qi; tonics aggravate Fire, weakening the pulse and worsening disease. ○ Why can small-fine pulses not take Renshen/Huangqi, while large-floating pulses can? Pulse reflects Qi-Blood circulation. Small-fine indicates Blood deficiency, Qi has no attachment and declines. Root is Blood deficiency, Yin damage precedes; taking Yang herbs after Yin damage worsens Yin. Large-floating pulses are mostly Yang pulses; Renshen/Huangqi match the syndrome, proven repeatedly.
