In individuals with a weak constitution, if they overexert themselves through labor and hardship, damaging the Qi and meridians (Jingluo, 经络), the meridians will become disharmonious. Consequently, the whole body may present with edema (Swelling) that is painless. Acupuncture is contraindicated as it would further damage the blood vessels; instead, internal administration of tonic and regulating herbs and external application of mild topical preparations should be used, treating the condition gradually to achieve success.
Leprosy (Mafen, 麻风), in its initial stage, invariably presents with numbness and loss of sensation at the affected sites. The pores are closed with no sweating. Manifestations include red-rimmed patches slightly swollen like cloud patterns, or white skin with scaling resembling tinea (Xuan, 癣). In some cases, pricking with needles yields black blood.
First, draw an isolation boundary circle with ink at the junction between healthy and diseased skin. All affected areas, whether divided into separate circles or combined, must be clearly demarcated. Next, within the circled area, mark black dots sequentially, maintaining a slight distance between each dot. Take nine No. 2 needles, thread them through the eyes, coil them into a circular shape, wrap them tightly with cloth strips, ensuring the needle tips remain exposed. Prick each black dot thirty-six times. The isolation boundary circle is also pricked thirty-six times. Immediately prepare a mixture by grinding musk (Shexiang, 麝香), borneol (Bingpian, 冰片), and Calculus Bovis (Niuhuang, 牛黄) (each in small amounts, e.g., fen) into powder, and mix it evenly with ginger juice and rice vinegar. Grind this mixture into ink and apply it to the pricked areas, covering the black dots. Then, prick the black boundary circle three more times, applying the medicinal liquid simultaneously. If not cured, continue pricking the interior of the circle as before. For unpricked areas presenting with foot paralysis and swelling, use bian stones (Bianshi, 砭石) to puncture and drain all turbid fluid until cessation. If the skin is already broken with bleeding and weeping serum, the condition is incurable. After pricking, avoid consuming salt and soy sauce for one hundred and twenty days to prevent recurrence.
During prolonged droughts without rain, excessive and fierce qi circulates throughout heaven and earth, particularly severely during summer months. River and well waters are also subjected to dry heat. This environment steams and consumes the human body. At such times, the interior is affected by toxins from stagnant water sources, while the exterior is pressured by pathogenic heat. This facilitates the emergence of seasonal epidemics (Shiyi, 时疫) which spread through contagion. Furthermore, those whose Yuan Qi (Original Qi, 元气) is already depleted due to overwork and injury, compounded by hunger which depletes the zang-fu organs, and whose emotional state is disturbed by adverse circumstances, inevitably suffer from prior deficiency in their Yin aspect. In the initial treatment using dispersing formulas, add moistening herbs slightly to curb the hyperactive pathogenic trend. Subsequently, employ sweet and cold herbs to vigorously nourish Yin fluids. If one recklessly administers warm and dispersing agents, seizing Yin and causing complications, symptoms such as dry tongue, thirst, delirium, coma, restlessness, and mania may arise, rendering the case likely untreatable. One should understand that heat pathogens first affect the Lung and Stomach. Metal of the Lung abhors dryness, while Earth of the Stomach prefers moisture. Since the disease originates from Yang pathogens, treatment should utilize Yin-moistening methods. Originally, Yang can sufficiently injure Yin, but Yin can also restrain Yang. When Yang declines, heat dissipates; when heat dissipates, Yin generates. Thus, hyperactive Yang can harmoniously interact with gentle Yin. Consequently, the Qi of Great Harmony (Taihe Zhiqi, 太和之气) stirs within the body, and the illness recedes suddenly.
