Syphilis in women, affecting the Yangming channel, causes deep ulceration of the breasts resembling a depression, with no swelling or pain. There is intermittent discharge of foul-smelling clear fluid, while lactation remains normal; this distinguishes it from other conditions.
When syphilitic toxin sinks downward into the Three Yin channels (Taiyin, Shaoyin, Jueyin), the legs and shins become ulcerated, turning black or dark purple. Foul fluid oozes intermittently, with vertical granulation tissue protruding, creating an uneven surface resembling embedded pearls of varying sizes. This is termed "Luodi Jiedu" (Deep-set Condensed Toxin) and is extremely difficult to cure completely.
In regions where warfare has persisted for a long time, malignant pestilential Qi inevitably permeates the area. Combined with horse urine flowing into rivers and wells, individuals with deficient constitutions or those suffering from overwork are prone to infection. The toxin enters the Lung and Stomach, spreads through the muscles, and reaches the skin. It initially presents as small red papules, gradually enlarging to the size of plums or dates, becoming foul, ulcerated, and everted, with vertical granulation tissue inside. This is known as "Ye Yangmei" (Wild Bayberry). Not transmitted via essence or sexual contact, its treatment is identical to that of standard syphilitic chancres and yields good results easily.
If the penis ulcerates segmentally until detaching down to the root, apply Zhuhuang San externally and take Baidu Fang internally; gradual recovery is possible. If there is swelling, induration, and foul ulceration unresponsive to treatment, healing is unlikely. If the swelling resembles the color of pig liver, it is incurable.
In cases of Huoluan (Cholera-like syndrome), there is no fever on the forehead or throughout the body, only dizziness, irritability, vomiting, and diarrhea. If there is no vomiting or diarrhea but severe abdominal pain, it is classified as "Jiaochang Sha" (Intestine-Twisting Sha). In seasonal epidemic febrile diseases where the head, face, body, and abdomen all present with fever, the physician must palpate the patient's head and chest/abdomen with the right hand to confirm the diagnosis. For female patients, female relatives may perform the palpation.
For carbuncles and sores that have ulcerated and discharged pus for a long time, if the wound intermittently discharges thin pus or clear fluid and fails to heal, or heals briefly only to recur, it indicates that pus and toxin have stagnated and formed a fistula tract. Insert Jiangyao Tiao (Descendant Drug Strip) into the tract and apply a plaster. After seven days, the tract will naturally expel itself, after which application of wound-healing agents can effect a cure. Jiangyao is refined from mercury; its nature is equivalent to arsenic trioxide (Pishuang), being fierce, corrosive, and painful. It should not be used lightly. Young and robust individuals may use it in small amounts. However, in children, the elderly, or those with deficient constitutions, its use may cause adverse transformations. If pain is severe, causing floating fire to attack upward, leading to erosion of the mouth, tongue, and gums, Shengmai Yiyin Jian (Decoction to Generate Mai and Nourish Yin) (see formula below) should be used. It is absolutely contraindicated in the mouth, around the eyes, ears, nose, epigastrium, lumbar region, penis, areas of red vein aggregation, hemangiomas, and qi tumors, as misuse can be fatal. Historically, only its benefits were known, not its harms. A method to transform danger into safety now exists: mix 3 fen of Jiangyao with 7 fen of Gypsum Fibrosum (Shigao), pounded together with rice gruel juice to form strips. This can also extract necrotic tissue and promote healing, accompanied by internal administration of tonic formulas. If external ulceration and pus discharge have persisted for more than ten days, using Baijiang Tiao (White Descendant Strip) can achieve complete success easily.
