There are throat conditions that are difficult to distinguish and must be carefully differentiated. In cases where men or women have syphilitic toxins accumulated internally within the zang-fu organs, although no sores have yet appeared on the body, or in children who have inherited the toxin in the womb, sudden onset of sore throat occurs with mild redness and swelling. However, the epiglottis (Diding) around the four sides rapidly ulcerates, forming several deep holes with white putrid tissue. The patient, due to ignorance, is unaware of the condition; the physician must inquire whether the patient has been infected with syphilis. Only by identifying the root cause can treatment be effective. Internally administer bitter-cold herbs to clear and descend the fire; externally apply Bingpeng San (Ice and Borax Powder), adding a small amount of Qinfen (Calomel) and Shengyao (Red Sublimate), and insufflate into the lesion using a milk-blower instrument. If treatment is delayed, the epiglottis may fall off, perforating through to the nasal cavity, and spreading to ulcerate the lips and mouth, often leading to death. Alternatively, it may perforate the fontanelle, creating a "sky-opening" deformity, resulting in disfigurement. If the condition is caused by deficiency-fire burning the lungs due to consumption diseases (Laozheng), although there is throat pain, only the superficial skin ulcerates without deep holes, and it is mostly incurable. In contrast, for throat conditions caused by wind-heat, the ulceration is located only on both sides of the throat.
When carbuncles and sores have ulcerated for a long time but fail to heal, the wound edges become progressively convex, concave, or everted with flower-like growths. They may be small like plums or apricots, or large like bowls or plates, becoming stiff and painful, with a terrifying progression. The pus may be foul-smelling and repulsive, or white thick putrid skin may appear on the wound surface, followed by the growth of dense thorn-like projections on the skin surface, changing into all kinds of bizarre shapes rarely seen in the world. These are attributed to karmic retribution and are incurable, ultimately leading only to prolonged suffering and death.
Xiaoye observes that during summer, earthworm snakes often emerge. Outside their burrows, snake venom foam is present. If a dog's mouth or nose touches this venom, it develops rabies (madness). Madness can also result from eating carrion or extreme hunger. If a person is bitten, breaking the skin, or even if only clothing is bitten, the toxin invades the body, causing the victim to bark like a dog, leading to death within days. Some patients feel numbness throughout the body and mental confusion one or two months later, or exhibit fear upon hearing sounds related to food and drink. Although various treatments exist, none have succeeded in saving the patient.
When a nodule forms in the center of a woman's breast, initially resembling a plum and gradually enlarging to the size of an apricot, with little pain, it eventually suppurates after a long period. This is called Ruxin Ju (Breast-Center Carbuncle). If it occurs in widows or unmarried women, it develops into Ru Yan (Breast Rock/Cancer); it is also difficult to treat when occurring in men. It should be treated with frequent administration of Huajian Tang (Hardness-Dissolving Decoction).
