Flying frost and hail, morning mist and evening gloom, continuous drizzling rain, cold wind and dew, heat when it should be hot, cold before the time of coldness, seasonal dampness overflowing, springs emerging in fields, mountains collapsing and earth cracking, soil crumbling and rivers overflowing, sun dimming and moon obscuring—this is the Yin Jue (Yin Reversal: a pathological state caused by excessive Yin qi disrupting the normal flow) of Heaven and Earth; sudden aphonia and acute cold, whole-body stiffness and urgency, curling and spasms of the four limbs, cyan lips and black face, staring eyes and clenched jaw, fullness and pain in the heart and abdomen, trembling and bulging of the chin, heaviness in waist and legs, slurred speech, vomiting above and diarrhea below, numbness on left and right sides, free passage of urine and stool, sour fluid from the tongue, sorrow and grief, unpredictable anger and joy—this is the Yin Jue of humans. The pulse of Yin Jue: if light pressure (Ju) feels weak but deep pressure (An) feels large, survival is indicated; if both light and deep pressure fail to detect the pulse, death ensues; if the whole body is cold and sweat spontaneously flows from the forehead, death also ensues. This disease of Yin Jue, if untreated for more than three days, leads to death.
When Yang Qi ascends but does not descend, it is called Pi (Pi-Ge: obstruction/stagnation where Qi mechanism fails to circulate); when Yin Qi descends but does not ascend, it is also called Pi; when Yang Qi descends but does not ascend, it is called Ge (Pi-Ge: rejection/blockage); when Yin Qi ascends but does not descend, it is also called Ge. Pi and Ge refer to the failure of Yin and Yang to follow each other harmoniously. When Yang rushes upward, it scorches the Spleen and Lung, generating carbuncles and sores with yellow-red color, all arising from Extreme Yang (excessive Yang heat); when Yin wanders downward, it freezes the Kidney and Liver, generating Jue syndromes with blue-black color, all originating from Extreme Yin (excessive Yin cold). All arise from the blockage and lack of communication between Yin and Yang due to Pi and Ge. When Yang scorching occurs, treat with Water methods (cooling); when Yin Jue occurs, assist with Fire methods (warming); this is the way of mutual assistance between Yin and Yang.
Alternating chills and fever indicate Yin and Yang competing against each other; insufficient Yang leads to chills first then fever; insufficient Yin leads to fever first then chills; excess in the Upper Jiao causes fever; excess in the Lower Jiao causes chills; cold skin with dryness indicates insufficient Yang; hot skin with dryness indicates insufficient Yin; cold skin accompanied by bodily coldness indicates Excess Yin; hot skin accompanied by bodily heat indicates Excess Yang. Fever with red cheeks and excessive talking indicates Yin Evil within Yang; fever with blue face and excessive talking indicates Yang Evil within Yin; coldness with blue face and excessive talking indicates Yin Evil within Yin; if the patient does not speak, the disease is incurable; Yin within Yin results in nine deaths and one survival; Yang within Yang results in nine survivals and one death; Yin diseases are difficult to treat, while Yang diseases are easy to cure. Diagnosing the pulse: rapid pulse in the Upper position indicates Yang within Yang; rapid pulse in the Lower position indicates Yang within Yin; slow pulse in the Upper position indicates Yin within Yang; slow pulse in the Lower position indicates Yin within Yin; slow pulse in the Middle position indicates Cold in the Middle Jiao; rapid pulse in the Middle position indicates Heat in the Middle Jiao; Cold is treated by retrieving with Heat; Heat is attacked with Cold; the laws of reversal and compliance follow Heaven and Earth, rooted in Yin and Yang.
